Some suffer from insomnia because of ongoing medical issues; however, many suffer from primary insomnia. A few elderly adults seek treatment for insomnia by using hypnotic medications that may have more adverse effects than benefits. Several of these adverse events, like falls with subsequent injury or fracture, can be life changing. In this article, we review the various modes of insomnia treatment with a literature search on both pharmacologic and nonpharmacologic treatments. Conclusion Cognitive behavioural therapy may be an effective long-term treatment for insomnia in the elderly and is a nonpharmacologic approach. Pharmacologic treatment may be effective and efficacious in the short term. Many of the commonly used hypnotics have not been studied over a longer term. Sankhala","doi":"10.13172/2052-0077-2-11-885","DOIUrl":"https://doi.org/10.13172/2052-0077-2-11-885","url":null,"abstract":"Introduction Angiokeratoma is a broad term that describes various conditions of asymptomatic hyperkeratotic vascular disorders with a histologic combination of hyperkeratosis and superficial dermal vascular ectasia. Here we are describing a case of recurrent angiokeratoma scroti. Case report A 34-year-old patient presented to the surgery department with multiple small papules over the scrotum with a previous history of two recurrences. They were slowly progressive, itchy and not associated with any bleeding. Examination revealed multiple soft compressible papules of 3–4 mm diameter with excoriation marks. Systemic examination and routine investigations were normal. Histologically, they were composed of numerous dilated thin walled congested capillaries mainly in the papillary dermis. Epidermis revealed variable degree of acanthosis and elongation of rete ridges and mild hyperkeratosis. Electrocauterisation of lesions was done and the follow-up period of three months was uneventful. Conclusion Although angiokeratoma scroti is not an uncommon entity, good treatment modality with minimal adverse effects in terms of complete resolution of diffuse lesions, recurrence, scarring, crusting, bleeding or cosmetic results is still not available. Introduction The term angiokeratoma is used to describe several distinct entities characterised by vascular nodules in the skin, with a histological picture of dermal vascular ectasia surmounted by hyperkeratosis of the epidermis. Angiokeratoma of the scrotum was originally described by Fordyce in 18961 reporting a case of 60-year-old man with multiple warty lesions confined to the scrotum only. Only a few cases of recurrent angiokeratoma had been reported since then. We describe here an adult male presenting with multiple papules over the scrotum with a previous history of two recurrences. Case report A 34-year-old male presented in the surgery department with moderate itching and multiple papules arranged diffusely over the scrotum. There was history of two recurrences five and three years back with the same complaints. Electrocauterisation of lesions was previously done two times, although no previous pathological reports were available. Each time, the follow-up period of four to six months had been uneventful. Even after complete post-operative healing, the patient continued to experience persistent discomfort, pruritis and psychological distress. Mere contact of the affected area with his underwear resulted in constant irritation. There was no history of trauma, episodic bleeding, similar lesions somewhere else on his body, inguinal hernia, genitourinary tumours, thrombophlebitis or varicocele. Physical examination revealed multiple, soft, bluish-red, compressible papules of 3–4 mm diameter with excoriation marks and focal thickening of skin, which might be due to previous surgical intervention. Reports on the existence of variations in the formation, branching and communication of median nerve are seen in the literature. However, there are very few reports on the involvement of two additional lateral roots in the formation of the median nerve. Such variations are of considerable importance for surgeons, physiotherapists and other clinicians. The details regarding the formation of the median nerve, developmental considerations and its importance have been discussed here. \u0000Case report:Apart from the normal roots, two additional lateral roots were given off from the lateral cord at the level of the second part of the axillary artery. These roots then coursed superficial to the axillary artery from the lateral to medial side to join the medial root, on the medial side of the axillary artery. The medial root then crossed on to the lateral side of the axillary artery to join the lateral root to form the median nerve. Normally, it takes origin from the medial epicondyle of humerus and is inserted into the palmar aspect of the base of second metacarpal bone and gives off a slip to the third metacarpal in the same aspect. \u0000Case report:Here we present a case with most of FCR (except for few muscle fibres close to the origin) being replaced with tendinous portion. \u0000Discussion:The action of FCR is in combination with the other muscles acting at the wrist joint. Some of the actions include flexion, abduction and circumduction that involve FCR. Any change in the normal anatomy of the muscle would affect the normal actions at the wrist. The knowledge regarding the variations in this muscle would prove beneficial to physiotherapists and also clinicians. Other main clinical implications of FCR is in grafts. Patients may be asymptomatic or complain of oedema, diarrhoea, hypoalbuminaemia and other deficiencies. This article reports a case of chronic diarrhoea and an abdominal mass. Case report We present the case of a 70-year-old woman who came to our gastroenterological clinic for diarrhoea. Blood and stool tests were within the normal ranges, except for a mild eosinophilia and a high C-reactive protein. The ultrasound scan showed a cystic retropancreatic mass, and the following computed tomography scan confirmed the presence of the mass without sure continuity with the dorsal pancreas. The fine needle aspiration performed during an echoendoscopy, permitted the complete aspi ration of the fluid and the cytological analysis that showed numerous lymphocytes. This scarcity reflects the fact that Foetal and Perinatal Pathology is a shortage specialty. In this Editorial, the reasons for the dearth of this kind of published literature will be discussed, the need for well-documented case reports in Foetal and Perinatal Pathology will be highlighted and their clinical and educational value will be explained. Editorial Developmental, Foetal, Perinatal and Placental Pathology are disciplines that form a distinct subspecialty of Paediatric Pathology. Abbreviated as ‘Perinatal Pathology’, they are mainly based on the autopsy of the foetus and infant and the examination of the placenta. In practice, a Perinatal Pathology unit covers embryonic, foetal, perinatal, neonatal, placental and infant pathology. The perinatal pathologist is called to perform the autopsy of the demised foetus or infant in cases of spontaneous abortion, intrauterine death or termination of pregnancy. In all instances, the autopsy report is meant to shed light not only to the cause of death but also to the aetiologic factors and pathogenetic mechanisms of the adverse pregnancy outcome, addressing all the clinical questions and clarifying the significance of pathological findings.
