One such appropriate use of ICT in health is Telehealth, which is defined as ldquothe integration of telecommunication systems into the practice of protecting and promoting healthrdquo. The present research is an ICT needs assessment of a select group of rural health physicians in the Philippines called Doctors-to-the-Barrios (DTTB), and employs the self-administered questionnaire approach in gathering data. Most of the responses were described qualitatively. Results suggest that while mobile phones and personal computers have made considerable incursion into remote communities, this is not paralleled by landline telephone infrastructure and the Internet, with adverse consequences on DTTBspsila ability for making distance referrals. The other method such as MCG (Magnetocardiogram) has been researched for heart disease detection. However, ECG and MCG have their own feeble points and also limitations in system performance according to the increase of data in quantity. Thus, in order for improvement of ECG and MCG diagnosis methods, we propose an integrated-diagnosis system applying grid technologies which is termed as a Physio-Grid system. We conduct the experimental and evaluate proposed system to show improvements in diagnosis and system performance. The semantic interoperability issue arises when similar things are meant differently to the remotely sitting agents. Given such scenario, this paper aims to describe a framework that combines A) dialogue game with B) a decision support system (DSS) leading to resolution of such types of ontological mismatches. Under dasiaDialogue gamepsila argumentation mechanisms are used for resolving certain types of ontological mismatches. While, the DSS decides on the initiation, termination, structuring and sequencing the dialogues, as these may not be properly adjudged by the dasiaDialogue gamepsila. The framework is therefore context driven in nature, where concepts can be substituted with the similar kind of concepts (may not be accurate by searching and choosing equivalent concepts. With this system framework, a physician can securely access and carry the patient information from a mobile device, update the patient information offline on the mobile device and synchronize the data with the server at a later time. The system provides high security to the highly sensitive patient health records. It provides various layers of security and privacy controls to access the patient information. This framework also maintains security levels both at system level and user level to constrain any attacks on the system. Data on the mobile device also is protected from being tampered or hacked using password protections and encryption. This application framework demonstrates a multi-tiered SOA (service oriented architecture) involving mobile client, Web services, security agents, business logic layer, data access layer and database in secured environments. This framework uses SAML (Security Assertion Markup Language) security assertions for exchanging the secured user identification information between the server and mobile clients. Despite of lot of research effort, the accuracy and robustness of QRS detection still remain open problems. Here we propose a novel QRS detector, which uses the Discrete Wavelet Transform (DWT) and Cubic Spline Interpolation as preprocessor, together with an improved dynamic weights adjusting strategy to enhance the detection robustness in noise condition. The algorithm was tested against MIT-BIH arrhythmia database, and achieved average sensitivity of 99.68% and positive prediction of 99.59%. This personal area wireless network with a set of wireless nodes mounted on body is popularly known as body area network (BAN). A typical BAN has a set of thirty to fifty wireless sensors with a low transmit power constraint. Also, the wireless channel seen in by a wireless node is unique when mounted on the human body. It has been shown that UWB supports reliable connectivity at low transmit power for body mounted wireless nodes. Further, the UWB also supports co-existence with other wireless networks and supports higher data rate. Given that large number of nodes are placed in close proximity the BAN operates in a multi-user interference limited environment. Therefore, we discuss the performance of multi-user based detectors for body area networks and compare the performance with conventional receiver architectures. We use the recently proposed channel model for the standard IEEE 802.15.4a. {"title":"An empirical analysis of reduction of medical expenditures by the eHealth system: Further results","authors":"Y. Akematsu, M. Tsuji, F. The town office has been implementing it since 1994 and keeps receipts on medical expenditures of its approximately 4,000 residents paid by National Health Insurance for 5 years from 2002 to 2006. We select (i) users; and (ii) non-users of the eHealth system, and by comparing their medical expenditures, we examine: (i) difference in medical expenditures between two groups; and (ii) negative correlation between medical expenditures and the length of usage of the eHealth system. We find that total medical expenditures of users are larger than those of non-users, whereas by restricting to lifestyle-related illnesses such as high blood pressure, cerebral infarction, strokes, and diabetes, medical expenditures of users are found to be smaller than those of non-users. Userspsila average annual expenditures per person related to lifestyle-related illnesses is smaller than that of non-users by 15,302 yen (US$133.06), which is about 20.7% of average expenditures of non-users. If users utilize the system one more year, then the above expenditures would decrease about 13,719 yen (US$119.30) per year, which is about 18.7% of the average expenditures. In addition, we show (iii) the elasticity of reduction of medical expenditures with respect to the length of utilizing the system increases according to years of its use, and also verify (iv) medical expenditures of those who have diseases would be reduced larger than that of people who do not have disease.
