[A case of mediastinal seminoma undergoing extremely cystic degeneration]. 纵隔精原细胞瘤伴极囊性变性1例。

CT and MRI films revealed a cystic lesion, with irregular nodules in the right anterior mediastinum. A cystic teratoma was suggested. Antero-axillary thoracotomy revealed a cystic lesion originating from the right lobe of the thymus. The lesion was extirpated, along with the right lobe of the thymus. The cystic part of the lesion, filled with brown fluid, was occupied by several masses originating from the wall of the tumor. Pathologically, the lesion was diagnosed as a seminoma undergoing cystic degeneration. The patient was given post-operative irradiation of 20Gy. No apparent recurrence has been detected 33 months after surgery. Mediastinal seminomas generally occur as a solid tumor consisting of stroma and tumor cells. The patient later disposed of the pigeons and henhouse, where upon, she experienced dyspnea on exertion. In January 1995, the patient was admitted to our hospital with a progressive pulmonary interstitial shadow. Lung biopsy after thoracoscopic surgery revealed centrilobular fibrotic lesions, as well as, small honeycomb-like and granulomatous lesions. An antibody to the pigeon serum was detected in the patient's serum by the ouchterlony method. By then applying the normal limits thus obtained to CT images of COPD patients (n = 45), we examined the sensitivity for detecting abnormal emphysematous changes in the lung fields. To measure emphysematous alterations, we used the average value of lung CT densities (ROI), the maximally appearing value in a CT histogram (Hist. Peak), the relative area with low CT densities below -910 HU (%LDA) and the total cross-sectional area (Area) in each lung section. Regardless of the section thickness (10 mm or 1 mm), the lung volume level at which the breath was held or the site from which CT images were taken (upper, middle or lower lung field), no significant correlation was observed between the CT indices associated with emphysematous changes and the subjects' age. This allowed us to define, independently of the subjects' age, normal values and 95% confidence limits for the CT indices. Among the CT indices surveyed, %LDA was found to be the most sensitive indicator for detecting emphysematous abnormalities. A 30-year-old man was admitted to our hospital with a high fever. Multiple nodular shadows were revealed on chest X-ray. Echocardiography revealed a mural thrombus in the right ventricle. A diagnosis of multiple pulmonary septic embolisms was made, and antibiotics were administered. The mural thrombus soon resolved, but later recurred in the same location. On the 90th day after admission. The mural thrombus was surgically removed. Histologically, the thrombus consisted of numerous neutrophils and fibrin. Examination of the endocardium attached to the thrombus revealed nonspecific inflammation, with fibrotic change and necrosing granulation present. A diagnosis of cardiovascular Behçet's disease was made based on the following findings: oral ulcerations, acneiform nodules, thrombophlebitis, bilateral uveitis, and positive HLA-B51. After treatment with steroid hormones and cyclosporine, the thrombus resolved completely, and inflammatory signs diminished. A chest X-ray film taken at a regular health check-up suggested relapse of pulmonary tuberculosis in a 61-year-old woman. Chest X-ray revealed irregular shadow with calcification in the upper lobe of the left lung and pulmonary tuberculosis was initially diagnosed a despite a negative reaction for acid-fast bacilli on examination of her sputum and bronchial lavage. Chest CT revealed thickened bronchi and blood vessels and nodules in the lung field. Transbronchial by biopsy failed to reveal caseating epithelioid cell granulomas transbronchial lung biopsy. Past history of facial palsy, uveitis, high plasma levels of angiotensin-converting enzyme (31.6IU/L), and abnormal HRCT levels. Bronchoalveolar lavage analysis revealed an increase in lymphocytes and a CD4/CD8 ratio of 8.67. Sarcoidosis was diagnosed after a liver biopsy. Chest radiographs revealed a moderate bilateral pleural effusion. Examination of the effusion was in conclusive. Thoracoscopy and thoracic computed tomography (CT) failed to provide evidence pertaining to the etiology of the recurrent effusion. Elevation of serum CA-125 levels suggested the presence of an intrapelvic tumor, which was substantiated by pelvic CT. Laparotomy revealed a small amount of ascites. The resected specimen revealed a tumor of the left fallopian tube, which was histologically to be poorly differentiated adenocarcinoma. The pleural effusion disappeared after removal of the tumor and Meigs' syndrome was subsequently diagnosed. Furthermore, the relationship between the main caregivers recognition of the patients' mental state and the patients' CESD score was examined. Family APGAR scores for the main caregivers were significantly lower than those of the patients (p < 0.002), or the seniors' club members (p < 0.004). Since most the main caregivers were women, we compared family APGAR scores of the female main caregivers and the age-matched female members of the seniors' club, revealing that the satisfaction of the female main caregivers with the \"adaptation\" component of the APGAR score (the component dealing with support given when something troublesome happens), was significantly lower than that of the members of the seniors' club (p < 0.0001). The CESD of the patients whom the main caregivers recognized as depressed was significantly higher. Raynaud's phenomenon and mammary and axillary lymphadenopathy. She had received silicone augmentation mammoplasty 30 years ago, and had since noticed bilateral mammary and axillary lymphadenopathy that was stable in size. In the 2 years before admittance she had become aware of an exacerbation of the lymphadenopathy had begun to experience and exertional dyspnea several months before admission suggesting a connective tissue disease. Physical examination revealed symmetrical weakness of the proximal limb muscles and fine crackles in the base of both lungs. Elevated myogenic enzymes, inflammatory reactions, and positive anti-SSA antibody were noted. Based upon these findings, muscle and lip biopsy results, myogenic EMG, and an apple tree appearance on sialography, a differential diagnosis of polymypositis or sjögren's syndrome was made. Axillary lymph node biopsy findings were consistent with silicone lymphadenitis. In addition, chest roentgenogram and HRCT (which revealed decreased lung volumes and interstitial opacities with no honeycombing, present predominatly in the subpleural space), pulmonary function tests (decreased VC and DLco), bronchoalveolar lavage (elevated total cell count and neutrophil and eosinophil fractions), and transbronchial lung biopsy specimens (unevently distributed alveolitis with fibrosis) indicated concurrent interstitial pneumonia. The clinical correlation between exacerbation of silicone lymphadenopathy and the development of connective tissue disease with accompanying interstitial pneumonia strongly suggested human adjuvant disease (HAD) as the pathogenesis. Diffuse reticulonodular shadows were found on a chest radiograph obtained as part of an annual check-up eight years before admission to the hospital. She was given a tentative diagnosis of idiopathic pulmonary fibrosis and was observed. She was admitted to our hospital because she noticed dry coughing and shortness of breath. A chest CT scan revealed segmentally distributed honeycombing and bronchi-bronchioloectasis. Tests for IgA and IgG antibodies to extracts of pigeon droppings in serum samples and in samples of bronchoalveolar lavage fluid were strongly positive, as were tests for lymphocyte blastogenic responses to samples of pigeon serum Examination of lung-biopsy specimens obtained by video-assisted thoracoscopic surgery revealed bronchiolitis, alveolitis, and honeycombing in a centrilobular distribution. The patient was given a diagnosis of pigeon-breeder's disease. Chronic hypersensitivity pneumonitis without acute episodes might be misdiagnosed as idiopathic interstitial pneumonia or bronchiectasis, as happened in this case. The possibility of chronic hypersensitivity pneumonitis should be considered when patients are suspected to have pulmonary fibrosis. He was admitted to our hospital on June 21 with progressive dyspnea and bilateral diffuse infiltrates on chest X-ray. On admission, plain chest radiographs and chest CT scans revealed bilateral interstitial shadows predominantly in the outer zone, of the lower lobes. After an operation for rectal cancer, he had begun taking orally 300 mg of fluorouracil daily for 64 days. A drug lymphocyte stimulation test (DLST) was positive for fluorouracil. Fluorouracil-induced pneumonitis was subsequently diagnosed.
