The current cross-sectional observational study was undertaken to compare upper airway ultrasonography and capnography for confirmation of endotracheal tube placement in patients undergoing elective surgery. Methods: 75 adult patients (18-60 years of age) belonging to ASA physical status I or II, scheduled for elective surgery under general anaesthesia requiring oral endotracheal intubation were enrolled for the study. After intubation, proper placement of endotracheal tube was confirmed by chest auscultation, end tidal capnography and upper airway ultrasonography simultaneously. Sensitivity, specificity, positive and negative predictive values of both capnography and ultrasonography were computed. Results: Using capnography, the position of endotracheal tube was found to be in the trachea in 72 patients (96%) and not in the trachea in 3 patients (4%). Ultrasonography was able to detect all 3 esophageal intubations but failed to confirm 2 endotracheal intubations. Capnography had sensitivity, specificity, positive and negative predictive values all at 100%. Ultrasonography had sensitivity of 97.22%, specificity of 100%, positive predictive value of 100% and negative predictive value of 60%. The kappa value was 0.737 which indicates a good correlation between capnography and upper airway ultrasound. Conclusion: Ultrasonography provides a real time dynamic visualization of the endotracheal tube. This study was able to demonstrate that ultrasonography is an acceptable, faster and safer method for confirming endotracheal tube placement. To increase the duration of sensory block opioids are used along with bupivacaine, but data on the effect of pentazocine as adjuvant with bupivacaine in SCBP block is still lacking. The study aimed to compare the duration of postoperative analgesia, sensory and motor block between 0.5% bupivacaine and 0.5% bupivacaine with pentazocine as an adjuvant in SCBP. Methods: The study was conducted on 60 consenting patients, posted for upper limb orthopedic surgery involving the forearm under SCBP block. Patients were randomly divided into two groups. Group B received 19 ml of 0.5 % bupivacaine with 1 ml of normal saline; Group BP received 19 ml of 0.5 % bupivacaine with 1 ml pentazocine (30 mg). Block characteristics, duration of postoperative analgesia, and side effects if any were recorded. Statistical analysis was done using the student t-test and Chi-square test for continuous and categorical variables respectively. Results: The onset of sensory (11.47 ± 1.57 vs. 16.8 ± 2.23 min) and motor (8.17 ± 1.14 vs. 13.9 ± 2.44 min) block was significantly faster in the BP group. Duration of sensory (392.33 ± 9.92 vs. 357.2 ± 8.76 min) and motor (379.27 ± 9.28 vs. 347.27 ± 9.13 min) block was also prolonged in group BP (p < 0.0001). Duration of postoperative analgesia was more (p < 0.0001) in the BP group (407.43 ± 10.46 vs 367.3 ± 8.74min). The paper aims to point out the essential elements of IC intended for a patient involved in research and to present novelties in the legislation that will affect the nature of information concerning the subject and the process of obtaining consent. The fundamental goal of the IC content is to understandably present to the potential participant all the relevant data related to the nature of the procedures they would be involved in, the risks and benefits; alternative possibilities; and to assess their understanding of all presented information. The process is more complex when it concerns vulnerable patient populations, that is, people whose willingness to participate can be influenced by expectations or benefits from research or by fear of the reactions of superiors if they refuse. This is particularly important in the case of involving persons who cannot consent independently by themselves for any reason. In such a case, a clear justification for their inclusion should be provided, as well as a detailed description of obtaining consent from parents, relatives, or guardians, i.e., legal representatives. The aim of our work is to analyze the status of oral health expressed by an adequate indicator in the population of people with disabilities who come to the national special rehabilitation hospital in Serbia for prosthetic rehabilitation. Material and methods: The study was performed as a prospective observational cohort study that included 51 subjects who had at least one limb amputated and who were hospitalized for prosthetic rehabilitation after limb amputation. After the examination of the oral cavity, the Decayed, Missing and Filled Teeth (DMFT) index was determined, which represents the sum of the total number of carious, filled and extracted teeth. Results: Patients had an average DMFT index of 22 (median) with an interquartile range of 17 to 27. The high value of the DMFT index was most heavily influenced by the number of extracted teeth. The median number of extracted teeth was 16 (interquartile range: 10 - 25). There were significantly less carious teeth, 2 (0 - 3), while there were the least teeth with dental fillings, 0 (0 - 2). This narrative review addresses and discusses facts, frequent questions, beliefs, controversies, unsolved issues, open safety questions and existing knowledge gaps related to efficacy and safety of ultrasound guided regional anesthesia (USGRA), based on the available literature. We discuss evidence-based advantages and limitations of ultrasound guidance, as well as unresolved questions, with respect to successful anesthesia, undesired side effects and patient safety, like nerve injury and local anesthetic systemic toxicity (LAST). We aimed to compare the effect of adding clonidine or fentanyl to low dose intrathecal bupivacaine as opposed to intrathecal bupivacaine alone. Materials and methods: A prospective, double-blind study was conducted in ninety elderly patients undergoing lower limb surgery under spinal anaesthesia. After randomly allocating the patients to three groups, Group BC [Bupivacaine + Clonidine]: 9 mg bupivacaine (0.5%) + clonidine 15 µg + saline, Group BF [ Bupivacaine + Fentanyl] : 9 mg bupivacaine (0.5% ) + fentanyl 20 µg, Group BS [Bupivacaine + Saline] : 9 mg bupivacaine (0.5%) + saline. The time for onset of sensory and motor block, highest sensory level achieved, time taken to achieve peak sensory and motor blockade, duration of analgesia and side-effects were compared between the three groups. The relevant statistical analyses were done. Results: The time taken for the sensory level and motor block to recede was the longest in group BC. The duration of analgesia was maximum in the group BC group and minimum in group BS. Incidence of hypotension and use of ephedrine was maximum in Group BC. The objective of this study is to provide a current update on the role of women in anesthesiology in Belgrade, Serbia. Methods: A cross-sectional observational study was conducted during September and October 2013 amongst anesthesiologists at ten tertiary health care institutions in Belgrade. The study population was 272 anesthesiologists. Participants' consent and ethical approval were obtained. The questionnaire captured basic sociodemographic and work-related characteristic information: age, sex, level of academic and professional postgraduate education, the managerial position of the section or department. Sociodemographic characteristics were tested in relation to sex by Pearson's chi-squared test. Results: The response rate was 76.2%. Over two-thirds (70.7%) of respondents were women. One-third of female participants (34.5%) and 23.3% of male participants had obtained additional academic achievements; 35.2% of female participants and 40.0 % of male participants had been in managerial positions. Statistically significant gender discrepancies in the additional academic education and upper-rank leadership positions were not detected. Conclusion: Female anesthesiologists in Serbia have achieved parity with men in the highest academic ranks and leadership positions. The unique feature of ultrasound guided truncal blocks is that in all of these techniques, in contrast to peripheral nerve blocks, no nerve or plexus needs to be identified. Local anaesthesia is injected in a particular muscle plane, in which the injectate spreads and reaches the intended nerves. Transversus abdominis plane (TAP) block has become a common analgesic method after surgery involving the abdominal wall. Aim: 1. To assess the efficacy of 0.25% Bupivacaine and 0.5% Ropivacaine in pain relief after lower abdominal surgeries using ultrasound guided transversus abdominis plane block (TAP). 2. Requirement of first rescue dose. 3.To assess complications related to the technique and drug related adverse effects. Methods: A comparative, randomised, double blinded study was carried out on 60 ASA physical status grade I and II patients of either sex between 18-40 years of age, scheduled for elective lower abdominal surgeries. 60 patients were divided equally by using computer generated random numbers into two groups. Group B received 15 ml of 0.25% Bupivacaine. Group R received 15 ml of 0.5% Ropivacaine. The TAP block was performed at the end of the surgery using the ultrasound. Results: Mean duration of analgesia was 373.75 minutes with SD of 66.1512 in Bupivacaine group and 687 minutes with SD of 119.433 in Ropivacaine group. The difference was highly significant in Group R compared to Group B (p < 0.0001). Almost 40% of the patients do not reach hospital in time while more than quarter die in the first 24 hours after the dissection begins. Case Report: A 37-year old man was admitted to our hospital with severe anterior chest pain which had lasted for over a week. Suspected aortic dissection was rapidly confirmed using imaging modalities - MDCT chest scan and TTE, followed by an urgent surgical management - Bentall procedure. MDCT chest scan also discovered adrenal incidentaloma defined as malignant, pheochromocytoma like mass. Due to the critical state of the patient, there was not enough time for further endocrinologic testing. Discussion and conclusion: When treating patients with pheochromocytoma and acute aortic disection, it is crucial to obtain a stable hemodynamic state before the surgery, since they can trigger a severe hypertensive crisis due to high levels of cathecholamines induced chronic vasoconctriction. The most vulnerable periods are the induction of anesthesia and perioperative hemodynamic oscillations, so treating patients with short acting alpha- 1 adrenergic blocking agents preoperatively has proven to be helpful - Phentolamine.
