The 2013 Carotid Revascularization Endarterectomy versus Stenting Trial concluded there was no difference between the two techniques as carotid endarterectomy increased the prevalence of a nonfatal myocardial infarction while a higher incidence of strokes was observed after carotid stenting. The inclusion of myocardial infarction as part of the primary endpoint has drawn criticism; however, in this dispute, a key component is missing: the patient's perspective. A patient's values and fears are crucial to the choice between therapies carrying different risks. As such, visits to an OBGYN may be the best or only opportunity for a young to middle-aged woman to be screened for cardiovascular disease (CVD) and CVD risks, uniquely positioning these providers to address the gap in CVD prevention. However, CVD screening practices, barriers to screening, and knowledge gaps in CVD screening and treatment are largely unknown. A statewide survey was conducted to fill this gap in our understanding of the CVD-related needs and practices of OBGYN practitioners. We conducted an online survey of OBGYN-certified physicians and advanced practice nurses who provide care in Minnesota. A 25-item questionnaire about provider practices, needs, barriers, and knowledge gaps regarding CVD screening and treatment was sent to 794 providers. We present an interesting case of a 63-year-old male who presented with an uncomplicated type B aortic dissection, who within a week represented with a rupture and was successfully treated with endovascular therapy. Either basic approach has advantages and disadvantages relative to diagnostic and therapeutic decision-making. Improved patient outcomes have been achieved with an invasive anatomic assessment using coronary angiography followed by stenosis specific intervention guided by physiologic measurements using invasive fractional flow reserve. Fractional flow reserve computed tomography has been developed to mimic these invasive procedures using noninvasive coronary computed tomography angiography and computer modeling with computational fluid dynamics.
