She was initially managed according to the Pregnancy of Unknown Location (PUL) care pathway. However, she presented with throbbing abdominal pain and required an emergency laparoscopy which revealed an intramural pregnancy at the site of a previous uterine perforation. The ectopic site was successfully excised laparoscopically. A detailed history, good knowledge of various rare presentations and the skills to make tailor-made decisions when dealing with PUL are crucial to avoid complications. Dodampahala","doi":"10.4038/sljog.v44i3.8062","DOIUrl":"https://doi.org/10.4038/sljog.v44i3.8062","url":null,"abstract":"The incidence of gallstones is 10-15%, making it one of the most common causes of surgical admissions. The number of admissions and operations for gallstone disease is increasing. Commonly gallstones are formed in the gallbladder but can also develop in the biliary tree. Commonly, gallstones are made of cholesterol. Even though the exact cause of supersaturation of bile is not known, there are multiple associate factors for gallstone formation. Most patients with gallstones never experience any symptoms, but the risk of presenting with complications related to gallstones is 1%-4% a year. Patients with gallstones can present with pain or other more severe complications that demand surgical treatment and follow-up. Wedsinghe","doi":"10.4038/sljog.v44i2.8024","DOIUrl":"https://doi.org/10.4038/sljog.v44i2.8024","url":null,"abstract":"A 20-year-old nulliparous woman presented to our obstetric unit in spontaneous labour at 39+1 weeks gestation following an uncomplicated pregnancy. After a short labour she progressed to a spontaneous vaginal delivery of a live male infant. Immediately after delivery she experienced retrosternal chest pain, facial swelling and dyspnoea. On examination her chest was clear to auscultation, but extensive subcutaneous emphysema was noted over the chest, neck and face. She remained a diagnostic dilemma until a plain chest radiograph revealed a large volume pneumomediastinum and a CT chest and barium swallow excluded an oesophageal perforation leading to the diagnosis of Hamman’s syndrome. She responded well to supportive cares and was discharged home on day three postpartum. Spontaneous pneumomediastinum or Hamman's syndrome has been rarely reported in the literature as a complication of labour.
