Pervez, Sana Kundi, Bushra Aslam, Sohail Rashid, Tahir Malik","doi":"10.48036/apims.v19i2.704","DOIUrl":"https://doi.org/10.48036/apims.v19i2.704","url":null,"abstract":"Objective: To investigate the variations in normal Doppler indices of the portal vein during the post-operative period following living donor liver transplantation (LDLT).\u0000Methodology: This retrospective cohort study was carried out at Pakistan Kidney and Liver Institute and Research Centre from July 1 to December 31 2021. It included all adult patients over 16 years of age who underwent Living Donor Liver Transplant (LDLT). Triplex Doppler ultrasound of LDLT recipients was performed intraoperatively and postoperatively for 5 consecutive days. Subsequent scans were performed at 2 weeks, 4 months, and 6 months after the transplant. Portal vein velocities were taken at the extrahepatic part, anastomosis, and intrahepatic part. Statistical analysis was performed using SPSS version 20.\u0000Results: The study involved 91 patients, with ages ranging from 17 to 73 years and a mean age of 44.9 years. Among the recipients, 79% were male and 21% were female. The portal venous velocities varied between 31 cm/s and 357 cm/s. All patients had antegrade portal venous flow. {"title":"Risk Factors for Dengue Shock Syndrome in Children Admitted in Federal Govt. Polyclinic Hospital (FGPC) Islamabad.\u0000Methodology: A cross sectional study, conducted at Pediatric department of FGPC hospital from August 2019 to October 2019, included 102 children of less than 12 years of age. All the children with acute febrile illness, who tested positive for NS1 antigen visiting the outpatient and emergency department, admitted in Pediatric department were considered eligible for enrollment in the study. Demographic and clinical characteristics were reported and correlated with disease prognosis and clinical outcomes.\u0000Results: Out of 102 patients, 13 (12.7%) developed dengue shock syndrome (DSS). Patients with capillary refill time of more than 3 seconds were found to be 2.44 times more likely to develop dengue shock syndrome (OR=2.44, 95% CI 1.47 – 4.03, p<0.001). Similarly, patients with thrombocytopenia (platelet count less than 150x103 cells) and leukopenia (TLC less than 4000x103 cells) were found to be at increased risk of developing dengue shock syndrome as compared to others (OR=1.28, 95% CI 1.12 – 1.48, p=0.001 and OR=1.6, 95% CI 1.12 – 5.21, p=0.01 respectively). Out of 13(12.7%) patients with DSS 1 patient (7.7%) died but no mortality observed in DHF.\u0000Conclusion: Increased capillary refill time, thrombocytopenia, and leukopenia were found to be significant predictors of DSS in children with dengue fever. A total of 120 neonates were included in both groups of the study. The control group received standard treatment, while the intervention group received standard treatment along with glycerin suppository. The data was analyzed using SPSS version 26. The independent sample t-test was used to compare the time to achieve full feed and the time to achieve complete evacuation of meconium between the two groups. A p-value of 0.05 was considered significant.\u0000Results: The mean gestational age was 30.30±1.19 weeks. Out of 240, 127 (52.9%) neonates were male. All neonates included in the study have a very low birth weight range from 1 to 1.6 kg with a mean weight of 1.32±1.29 kg. Patients with type-1 or type-2 diabetes mellitus who visited the outpatient department and had a disease duration of at least six months were included in the study. Participants might be either gender and ranged in age from 18 to 70. Along with sociodemographic information, characteristics of diabetes mellitus were recorded including fasting and postprandial blood glucose levels, kinds of DM, drugs used, and family history of DM. All patients' frequency of missing diabetic mellitus prescription doses was also assessed. \u0000Results: In a total of 230 patients of DM, 148 (64.3%) were male. The mean age was a 45.00±12.48 year. Family history of diabetes was present in 113 (49.1%) patients. There were 215 (93.5%) patients who had type 2 DM while remaining 15 (6.5%) were type-1 DM. Mean duration of DM was 7.63±7.6 years. It was found that 105 (45.7%) patients were using both oral and insulin. It was noted that 30 (13.0%) patients were missing their DM medications daily, 43 (18.7%) most frequently, 21 (9.1%) frequently and 54 (23.5%) missed their DM medications/dosages occasionally. It was observed that statistically significant differences existed for blood glucose parameters and frequency of missed medications (p<0.0001). \u0000Conclusion: Vast majority of diabetes mellitus patients were found to miss their anti-diabetic medications. After evaluating scores of lesions, participants are provided with their respective preventive measures according to their division in the randomized control trial. Group one is provided with high fluoridated toothpaste at 1400 ppm that is commercially available in markets under the name Colgate Total twice daily for a period of 56 days. The other group is asked to visit OPD for the application of a paste of CPP-ACP once a week for a period of 56 days. Both groups are supposed to make three follow-ups at days 15, 34, and 56.\u0000Results: This study consists of 126 participants. Out of 126 participants, 48.0% were males and 57.94% were female. The mean age of the cases was 17.8 years. Out of total, 48% were males and 57.94% were females, with a mean age of 15.99±2.183. Sampling was done using the nonprobability consecutive sampling technique. Test analysis included a complete blood picture, RBC morphology and reticulocyte count, G6PD quantitative test, and serum bilirubin. Thus, to categorize G6PDD based on its enzyme and clinic-haematological correlation, study included patients of both gender with an age ranging from 0-76 years. Descriptive statistics were expressed as mean ± standard deviation (SD) and categorical data were presented as frequency and percentage.\u0000Results: Out of 120 study participants, 30 (25%) were females and 90 (75%) were males. The mean age of study participants was 10.83±12.75. G6PD PCR was detected among participants having G6PD deficiency level <1 U/g Hb and between 2-3 U/gm Hb. Hb levels below 8g/dL were found only in individuals with G6PD deficiency levels <1 U/gm Hb.\u0000Conclusion: GDPD deficiency can be diagnosed by blood analysis comprising of complete blood count and RBC morphology aided by clinical correlation. A total of 11 patients with an age range 21-58 years, lesions of the triangular fibrocartilaginous complex (TFCC), and MR arthrography were included. MR arthrography images of the wrist were evaluated to assess the integrity of the intrinsic wrist ligaments, including those of the TFCC, scapholunate, and lunotriquetral ligament. MR arthrography injection was performed into the wrist joint, likely to help visualize the ligaments and other structures within the joint. The injection was performed under fluoroscopic guidance, and the size of the needle used for the injection was 20-24G. The data collected from the MR arthrograms and arthroscopy was analyzed using SPSS v 23.\u0000Results: The patient’s mean age was 40.0±12.31 years. Out of 11 patients, there were 6 (55.5%) males and 5 (45.5%) females. Sensitivity of MR arthrography is 71.4% and specificity that of 75% for detecting TFCC defects, and a 100% sensitivity and 85.7% specificity for diagnosing intrinsic ligament defects. All these patients were full term. The diagnosis was confirmed on clinical examination supported by cross table lateral film in prone position after 24 hours and ultrasound evaluation of level of rectal pouch. Single stage reconstructive surgery was done under general anesthesia within 24 to 48 hours after birth. The outcome of single stage reconstruction of neonates with anorectal malformations is evaluated post operatively during an average regular follow up of 3 years.\u0000Results: All the 26 neonates are treated by single stage reconstructive surgery between 24 to 48 hours after birth during the period from June 2011 to Oct 2021 who were admitted with anorectal malformations in NICU after excluding associated congenital anomalies. All the patients had excellent cosmetic and functional outcome without significant problem of wound infection. Only 3 female patients with recto-vestibular fistula had mild superficial wound infection and were managed conservatively.\u0000Conclusion: Single stage reconstructive surgery to treat ARMs in neonates I effective, safe and feasible with good continence. It avoids morbidity and higher cost associated with three stage surgeries and colostomy. A total of 148 patients were enrolled after taking written consent and data was collected through predesign proforma sheets, including; clinical history, investigation and early outcomes in term of (renal failure, prolonged ventilation, and mortality). SPSS 23 was used to analyze data with statistically significant p-value < 0.05.\u0000Results: The findings showed that average age of research participants were 57.14 ± 3.07 (age range 30-73 years) and 121 (81.76%) male compared with 27(18.24%) female patients were enrolled with insignificant p-value of 0.730. In this study prolonged ventilation was found in 5(6.76%) obese and 8(10.81%) non-obese patients with insignificant p-value of 0.070.
