{"title":"Growth modification treatment of mandibular retrognathism using twin block apparatus (case report)","authors":"Chrisni Oktavia Jusup, Endah Mardiati","doi":"10.35856/mdj.v11i3.665","DOIUrl":"https://doi.org/10.35856/mdj.v11i3.665","url":null,"abstract":"Interceptive orthodontic treatment, which is growth modification treatment using removable appliances such as twin block, is effective in treating skeletal retrognathic mandibular abnormalities in growing patients. A 10-year-old female with class II den-toskeletal malocclusion with retrognathic mandibular was treated using a twin block device for 8 months of active period and 4 months of retention period to obtain class I skeletal relationships, class I molar and canine relationships and normal overjet and overbite. Treatment begins with recording the bite by instructing the patient to advance her lower jaw as needed, then ma-king a twin block tool design. Devices with maxillary and mandibular bite blocks are worn for at least 20 hours a day with con-trol intervals every 2 weeks. The treatment results were very good with the patient's skeletal relationship being class I. Ocular prosthesis is part of a whole rehabilitative treatment after the surgery. This article describes the procedure to make a permanent ocular prosthesis to improve appearance, physical and psychological post surgery. A 3-year-old female patient came to Halimah Dg. Sikati Makassar Dental Hospital that was referred by an ophthalmologist after losing her right eye-ball to enucleation procedure for stage 2 tumor at the tender age of 8 months and wishes to have a permanent ocular prosthesis to restore appearance and self-confidence. Ocular prosthesis can help ease the psychological trauma and burden of losing the eyeball. Impovement in management of head and neck cancer have resulted in decreased mortality, increased survival rates and patient’s quality of life. The World Health Organization defines quality of life are include three things, physical function, mentalstatus and ability to socially interact. It is reported a 19-year-old male with a history of right infraorbital adenocarcinoma who had undergone hemimaxillectomy,chemotherapy and radiotherapy. The patient complains painful swallowing and soreness in the oral cavity. Extraoral examination face and eyes was asymmetrical,there was keloid tissue in the right infraorbital and dry lips. Intraoral examination found white plaque on the upper labial mucosa, the left buccal mucosa had severalerythematous lesions and brownish macules. Yellowish white plaque on the anterior two-third of the tongue’s dorsum and there are fissures. Necrotic area on right hard palate with yellowish margin. The teeth at first region are edentulous, whereas regions 2, 3 and 4 most of them are radix. The patient’s quality of life was assessed using the oral health impact profile (OHIP-14). Based on the assessment, the patient's quality of life at the first visit was poor with score 45, the next visit showed improvement with score 39, reduced in pain, improved physical and psychological abilities. Some natural materials can be used as antibiotics, anti-inflammatory, and antifungal. Antibiotics and antifungalsare needed in the patient's dentures be-cause acrylic resin denture base can be a breeding ground for S.mutans and C.albicans colonies. This literature review explains the utilization and potential of natural materials in prosthodontics. Only 1% of cases reportedly occurring in those younger than 20 years of age, so that this case is quite rare. The diagnosis of temporomandibular joint disorder (TMD) was also present along with the trigeminal neuralgia and confirmed by head and TMJ MRI. This article describes a case of a 17-years-old females who sought help because of pain in the right side of her face, from her temple area down to her cheek and TMJ, continuing to her lower jaw and sometimes at the back of the ear. She also reported increased pain during chewing or otherfunctional activity. The pain already lasted for almost two years and debilitating. This case report describes compounding with a combination of hydrocortisone,chemicetin, lanolin and vaseline for the treatment of allergic contact cheilitis. Patients with com-plaints of lower lip feeling dry, cracked, sore and bleeding;dryness for 4 days ago. Dry lips getting worse for 2 days ago until bleeding. Objective examination revealed fissures, multiple, well-defined, irregular edges, 1-2 mm long; crusts, regular edges, clear borders, blackish red color; desquamation, multiple, irregular edges, clear boundaries, white color, rough surface. The pa-tient was prescribed a combination of drugs of hydrocortisone, chemicetin, lanoline and vaseline and was instructed on how to use the drug and control it 7 days later. Treatment of allergic contact cheilitis with compounding combination drugs aimed at accelerating healing. The inside of the bone that has been infected will become soft and experience swelling which causes the tissue to press against the outer wall of the bone and the blood vessels in the marrow whichcauses blood flow and oxygen supply to the bone decrease drastically. Usually surgical therapy is needed to treat osteomyelitis, but if the patient's physical condition does not allow it, hyperbaric oxy-gen therapy can be done to increase the supply of oxygen to the hypoxic bones. Hyperbaric oxygen therapy is an adjunctto conven-tional antibiotic or surgical treatment of resistant osteomyelitis, using large, multiposition or monoposition pressure tubes for one patient. The patients are given 100% oxygen with a pressure dose of 2-2.4 Atm for 90-120 minutes per day; five or six times per week for 8 weeks or more. T.conoides has been studied for its many benefits, including relieving fever in children, insect repellent, fertilizer, and also as an antifungal and antibacterial agent. The components found in the extract of T.conoides were investiga-ted to be able to inhibit C.albicans and also have antibacterial properties. Guava are often usedbecause the leaves have been proven to treat various diseases. Guava leaves contain active ingre-dients, including tannins, flavonoids, essential oils and saponins.Lime is often processed by the community as a fresh drink, such as warm lime juice. Lime fruit has useful ingredients such as essential oils, citric acid, amino acids and saponins. The aim of this study was to analyze differences in fracture resistance after root canal irrigation between 60% GL extract and 100% lime juice. This is a laboratory experiment using extracts of GL, LJ, maxillary central incisor with a post-test only control group design, which was tested statistically by using one-way Anova followed by a post hoc test. Fracture resistance in the 60% GL extract group averaged is 45.07±9.80; while the 100% LJ group is40.86±12.71; the differ is 4,280.
