In view of the chronology of events and course of the disease, the connections between protein peak and polyarteritis nodosa can be considered. Management of polyarteritis nodosa is discussed according to the existence of a peak which may originate in myeloma. We report a series of 33 cases observed during a 21 year period in the departments of pediatrics of the University of Lyon. Among the various etiologies, vaginal foreign bodies (10 cases, 30%) and vaginitis (10 cases, 30%) were the most frequent. There were 3 cases of benign vulvar tumors (9%) and 3 cases of malignant vaginal tumors (9%). In 3 cases the vaginal bleeding remained apparently idiopathic leading to the diagnosis of premature menarche. The group includes 35 females and 26 males, with a mean age of 62. The disease is usually discovered on routine blood tests (half of cases), and more rarely because of hemorrhagic or thrombotic manifestations. Splenomegaly is found in one-third of cases. Platelet count is permanently above 800 X 10(9)/l (mean : 1 500 X 10(9)/l); mild hyperleukocytosis (mean : 16 X 10(9)/l) with predominant neutrophil polynuclears is usual but myelemia is not constant (28% of cases) and always very moderate; red cell parameters are normal in three-fourths of cases, while the remaining patients have anemia, either due to iron depletion or not. Reticulinic myelofibrosis, usually minimal, is found in 40% of cases. Medullary karyotype is always normal, without chromosome Ph1. Platelet functional abnormalities are not constant and do not correlate with the magnitude of thrombocythemia. 51 patients (84%) received myelosuppressive therapy, mainly by busulfan or radioactive phosphorus. The gradual deterioration of somatic and psychologic tolerance capacities is often associated with multiple pathological conditions which may delay diagnosis and hinder the implementation of therapy. Major therapeutic risks are mainly ascribable to chemotherapy which requires strict application of contraindications and close monitoring during treatment. Under these conditions, carefully determined by a thorough workup prior to treatment, the elderly patient, although fragile, can nevertheless benefit from adequate and efficient therapy. Although seemingly aggressive in some instances, this active outlook is preferable to systematic palliative therapy chosen only because of age, often constraining and not always useful or safe. The most significant clinical sign is the discovery of an abdominal mass, and ultrasound is, today, a decisive diagnostic aid. Whatever the etiology - which is often difficult to establish - the essential part of the treatment consists in early and complete surgical drainage. Most cases were discovered on routine chest films. Six patients had radiologic changes produced by obstruction, while 43 had a rounded peripheral opacity. Fine calcifications were present in four cases. In no case was diagnosis achieved by bronchoscopy, whereas one of the more recent cases was diagnosed by percutaneous fine needle biopsy. Sinner has shown that this last technique can establish diagnosis in most cases.
