It is rare with the reported incidence between 0% and 1%. We present a case of a 51-year male who presented with breathlessness, cough and right-sided chest pain. Chest radiograph showed right-sided pneumothorax. After 2 hours of chest tube insertion, the patient developed tachypnea and severe hypoxemia. HRCT chest revealed consolidation, ground glass opacity and smooth interlobular septal thickening in the right lung as well as a small patch of ground glass opacity in the contralateral lung. Clinical observations and different imaging properties can help with early diagnosis and treatment. Vertebral endplate destruction, changes in the signal intensity of intervertebral disc and bone marrow, and involvement of the soft tissues surrounding the vertebrae are typical MRI findings of infectious vertebral lesions.Methods:To determine the diagnostic accuracy of MRI in patients suspected of having infective vertebral lesions, a cross-sectional study was conducted involving 52 individuals. The study compared the validity of MRI diagnosis with the diagnosis obtained through CT-guided fine needle aspiration cytology for infective vertebral lesions.Results:By comparing the MRI diagnoses with those of fine needle aspiration cytological diagnosis of vertebral lesions, the sensitivity, specificity, positive and negative predictive values (PPVs and NPVs), and ultimately diagnostic accuracy of MRI, were computed. Its causes are numerous, but they can be divided into two categories: congenital and acquired. The congenital type develops intrauterine, whereas the acquired type develops early in life, usually before the age of two. Cerebral hemiatrophy syndromes are characterized by a wide range of neurological symptoms. Childhood epilepsy, mental retardation, and neuropsychiatric disorders are common, whereas movement disorders such as highly asymmetric parkinsonism or hemidystonia, as well as neuropsychiatric problems, have been reported in adults. Here we present a 22-year-old patient who presented with refractory seizure. The gastroenterology team performed ERCP with sphincterotomy to retrieve stone in common bile duct but failed. Open cholecystectomy with common bile duct exploration was done after few days. T-tube was placed in common bile duct. In the 8th post operative day, tube manipulation was done due to reduced drain output and pain abdomen. We performed T-tube cholangiogram on 12th postoperative day which showed duodenal migration of T tube. It can display minute anatomical and pathological details by providing a direct visual window into the temporal bone.\u0000Methods: This is a prospective cross-sectional study done at the department of Radiodiagnosis and imaging of Birat Medical College, Teaching hospital over 1 year with a total of 50 patients enrolled in the study. The findings were compared with the post-operative findings and data were entered. HRCT was done on 64 slices of multidetector CT scan. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of various parameters were calculated.\u0000Results: Out of 50 patients were enrolled in this study. Among them 52%(26) were females and 48%(24) were males. HRCT showed 100% sensitivity for the lateral semicircular canal and sigmoid plate erosion. The sensitivity and specificity of cholesteatoma were 79.06% and 42.85% respectively. It also offered a sensitivity of 75% and 95% for the erosion of the facial nerve canal and tegmen tympani respectively\u0000Conclusions: High-resolution computed tomography is a reliable diagnostic tool for the pre-operative evaluation of chronic otitis media. During surgery, anatomical variances are frequently encountered by surgeons, making operation complex and occasionally challenging. The purpose of our study was to evaluate the hepatic artery and its variants, including the length and diameter of the right and left hepatic arteries. This information is vital for patients undergoing major hepatic surgeries and hepatic transplantation\u0000Methods: A total of 120 patients who underwent contrast-enhanced Triphasic CT of the abdomen were evaluated with the consent of the ethics committee at Chitwan medical college. Study subjects were selected on a random basis, who had no previous abdominal or hepatic surgeries, and without the known hepatic disease.\u0000Results: Out of 120 participants, 63 (52.5%) were male and the rest 57(47.5 %) were females. Normal anatomy was observed in 104 cases (86.7%) and the rest of the 16 cases showed an anomalous hepatic arterial pattern, which consisted of Michel’s type III in 6 cases (5%), Type IV in 1 case (0.8%) and Type X in 3 cases (2.5%); and 5 cases (4.2%). did not fall under Michel’s Classification. The main objective of this study was to investigate the morphology and morphometry of the coccyx on lumbar spine MRI images in asymptomatic individuals among Nepalese adults.\u0000Methods: This study was conducted retrospectively on the lumbar spine images of 190 adult population without a history of trauma in the coccyx region, from April to September 2019. The coccygeal vertebrae count, the number of bone segments, and intercoccygeal and joint fusions were determined from the sagittal plane images. In addition, the length and angles were also measured.\u0000Results: One hundred and ninety patients, with a mean age of 43.91 years, were enrolled in the study. Among the 4 types of coccyx; the most common type was type II (46.3%), followed by type I (40.5%). The coccyx was formed from 4 vertebral segments in 21.1% (n=41) individuals, 3 vertebral segments in 45.3% (n=86) individuals, 2 vertebral segments in 31.1% (n=59) individuals and 1 vertebral segment in 2.1% (n=4) individuals.\u0000Conclusions: In our study, type II coccyxes predominated. {"title":"Assessment of Liver Stiffness by Shear Wave Elastography in Healthy Liver in a Tertiary Center","authors":"K. Adhikari, K. Devkota, Sapana Koirala","doi":"10.3126/njr.v12i2.54595","DOIUrl":"https://doi.org/10.3126/njr.v12i2.54595","url":null,"abstract":"Introduction: The main aim of this study was to determine the mean value of liver stiffness in healthy adults using S-Shear wave elastography and compare liver stiffness with gender, age and BMI.\u0000Methods: A hospital-based cross-sectional study was carried out in the Department of Radiology, BPKIHS over 6 months after taking ethical approval from the institutional review committee. Ninety-six adults referred for ultrasonography of the abdomen for various indications were included in the study\u0000Results: Mean shear wave velocity of the healthy liver in the study subjects was 1.267±0.151m/s.The corresponding mean value of liver stiffness was 4.91±1.15 kPa. The mean liver stiffness was higher in males compared to females (5.283±1.08 kPa vs. 4.689±1.13 kPa) which were statistically significant (p=0.0013). Mean liver stiffness was highest in the age group of 70-79 years (6.7±0.80 kPa) compared to other age groups. There were significant differences between the mean liver stiffness values among different age groups (p=0.01). Patients in the obese class I category showed higher mean liver stiffness value (5.54±0.42 kPa) than other BMI categories. However, mean liver stiffness values showed no significant difference among different BMI groups (p=0.322).\u0000Conclusions: Liver stiffness values measured using S-Shear wave elastography in healthy adults ranged between 2.4 kPA to 7.4 kPa.There were significant differences in mean liver stiffness values between men and women and among different age groups. MRI is routinely used before arthroscopic examination and surgery. Arthroscopy is considered the reference standard for the diagnosis of injuries of cruciate ligaments and meniscus of the knee and hence the efficacy of MRI needs to be compared with arthroscopic findings.\u0000Methods: This was a retrospective observational study at Bheri hospital, Nepalgunj.
