The approaches used in the reduction of orbital floor fracture are classified into three types: transorbital, transantral and intranasal approach. Each of them has limitations, such as hypertrophic scar formation, facial hypoesthesia and difficult access with manipulation. Contrastively, the prelacrimal approach is a minimally invasive procedure that provides a better surgical view of fracture site than the three types mentioned. It also provides direct access to the fracture site. It is commonly found in the mastoid or petrous apex, but the involvement of the nasal septum is very rare. Mucocele, which has pathogenesis similar to that of cholesterol granuloma, is a relatively common cystic lesion of the paranasal sinus; but rarely, it also occurs in the nasal septum. NMC predominantly arises in the anatomical midline locations of the body, with a high occurrence in the thoracic, head and neck regions. However, NMC remains a rarely reported disease, making its inclusion in the initial differential diagnosis challenging. This case involves a 4-year-old boy who presented with a neck mass initially diagnosed as a poorly differentiated carcinoma originating from the larynx with the cervical Lymph node metastasis. However, further tests confirmed the diagnosis of NUT midline carcinoma. It is often misdiagnosed due to its rare occurrence in the nasal and paranasal sinus; moreover, it has similarities with other cartilaginous tumors, showing the same radiologic and histopathologic features. A 62-year-old female consulted our clinic with the chief complaint of a left nasolabial mass of one-month history. The mass was located in the nasal cavity: from there it extended anteriorly to the anterior nasal spine, posteriorly to the end of the hard palate, and laterally to the maxillary sinus. The nasal septum was deviated to the right side due to the mass effect. Complete removal of the mass was conducted via the modified endoscopic Denker’s approach, and the diagnosis of CMF was established. These approaches are canine fossa approach (CFA), Caldwell-Luc operation (C-L op), Denker approach, and inferior turbinate (IT) swing technique. Our purpose was to analyze and compare the overall results of these surgical approaches to determine superiority.Subjects and Method Records of patients who underwent surgery of functional endoscopic sinus surgery (FESS) with CFA, C-L op, Denker approaches or IT swing technique in a single tertiary hospital from 2015 to 2023 were studied retrospectively. Patients who underwent additional surgery other than CFA, C-L op, Denker, and IT swing technique were excluded.Results A total 86 patients underwent FESS with combined approaches. IT swing technique was used for 30 patients, Denker op was performed for three patients, and C-L op or CFA was used for 53 patients. IT swing technique, Denker op, and C-L op were mainly used for nasal cavity tumor, such as inverted papilloma (IP). CFA was used for not only CRS w NP but also for IP. Recurrence was reported for the IT swing group and CFA group in IP. OETD is often not improved by medication nor by nasal washing or nasal spray. On the other hand, balloon Eustachian tube dilatation (BETD) is an effective surgery in OETD although repeating BETD is controversial as it involves additional procedure such as ventilation tube, middle ear steroid irrigation, and trans-tympanic Eustachian tube probing in some cases. This study aims to analyze the clinical differences between patients who received single BETD and repeat BETD, and to evaluate the efficacy of repeat BETD.Subjects and Method For the study, 110 single groups, and 16 repeat groups were selected from patients who underwent BETD from Nov 2019 to Aug 2022. Age, gender, underlying diseases, history of rhinitis and smoking, and Eustachian Tube Dysfunction Questionnaire-7 (ETDQ-7) were analyzed for the selected groups.Results There were no significant differences in gender, underlying disease, or history of rhinitis between the two groups. However, age was significantly younger in the repeat group (45.91 vs. 33.38, p=0.006), while smoking (5.64 vs. 1.66, p=0.01) was significantly higher in the single group. The rate of improvement in the ETDQ-7 was 81.3% in the single group and 69.2% in both initial and repeated surgeries in the repeat group.
