Methods: Differences between vancomycin and tigecycline in terms of their doseand time-dependent intracellular concentrations were determined with in vitro cell culture and the high performance liquid chromatography techniques. Results: The penetration of tigecycline was found significantly higher in cystic fibrosis cells compared to normal bronchial cells. The optimal dose for tigecycline was 30 μg/mL and the optimal time was 8 h in comparison to given dose and penetrating amount into the cells. Vancomycin dose needed to be given for minimal penetration into the cell was extremely higher than the minimum inhibitor concentration described in the guidelines. Conclusions: Penetration of tigecycline into cells of cystic fibrosis lung was found higher than normal lung cells, whereas cell penetration of vancomycin into both normal and cystic fibrosis cells was limited. These data suggest that tigecycline has a high therapeutic potential in lung infections of patients with cystic fibrosis. As the first case, a 31-year-old male who had a childhood in a family raising animals sought medical advice with the complaint of side pain. His radiologic examination revealed cystic lesions in lungs and liver. Lesions in the lung were resected. “Puncture, aspiration, injection, respiration (PAIR)” treatment was applied to lesions in the liver. The second case was the brother of the first one, a 36-year-old male. Cystic lesions in lungs, liver and spleen were detected on his radiologic imaging. Lesions in lungs were resected and PAIR treatment was applied to hepatic and splenic lesions. The last case was the 56-year-old mother. Her radiologic examination revealed cystic lesions in left lung and liver. Pulmonary lesion was resected and PAIR treatment was applied to lesions in the liver. All patients were followed up under albendazole treatment. In conclusion, family scanning is an important issue, especially in socioeconomically developing, endemic areas where farming is common. The aim of this study was to investigate CA-MRSA frequency and nasal carriage in patients with pyoderma. Methods: Between November 2011 and August 2013, we aimed to investigate the frequency of MRSA isolated from nasal swab and wound specimens of 38 patients with pyoderma according to the “community-acquired infection\" criteria of Centers for Disease Control and Prevention. Also, the criteria investigated included age, gender, number of people residing in the same household and their hospitalization frequency, chronic illnesses, and antibiotic use in the previous six months. The control group of 103 patients who were evaluated with the same criteria and with various skin diseases excluding pyoderma were examined by culturing their nasal swabs. Results: Totally, 38 specimens obtained from patients with folliculitis (n=18), furuncle (n=7), cellulitis (n=4), abscess (n=2), hidradenitis suppurativa (n=2), carbuncle (n=1), and impetigo (n=4) were investigated, and 2 (5.3%) CA-MRSA were isolated. One of these strains were isolated from a patient with folliculitis, and the other strain was isolated from nasal swab of the another patient of whose sample from folliculitis was negative for CA-MRSA. One (1%) CA-MRSA strain was isolated from the Özet Amaç: Son yıllarda, toplumdan kazanılmış metisiline dirençli Staphylococcus aureus (TK-MRSA) infeksiyonlarının giderek yaygınlaştığı ve bu bakterilerin en sık olarak deri ve deriyle ilişkili yapıların infeksiyonlarına neden olduğu bildirilmektedir. Çalışmamızda pyodermili hastalarda TK-MRSA sıklığının ve burun taşıyıcılığının belirlenmesi amaçlanmıştır. Yöntemler: Çalışmamızda, Kasım 2011-Ağustos 2013 tarihleri arasında, Centers for Disease Control and Prevention’ın “toplumdan kazanılmış infeksiyon” kriterlerine uyan 38 hastadan alınan deri lezyonu ve burun sürüntüsü örnekleri değerlendirilmiştir. Ayrıca hastalar, yaş, cinsiyet, evde yaşayan kişi sayısı, evde sık hastaneye yatış öyküsü olan birey olması, kronik hastalıklar, son altı aydır antibiyotik kullanımı ve poliklinik başvurusu gibi demografik özelliklerle diğer sağlık bakımıyla ilişkili risk faktörleri açısından da incelenmiştir. Kontrol grubu yukarıdaki kriterlere uyan ve pyodermi dışı deri hastalıkları olan 103 hastadan oluşturularak burun sürüntü örnekleri değerlendirilmiştir. Transesophageal echocardiography revealed a defect in intraventricular septum, and a loose mass image was observed on the septal cuspis of tricuspid valve. A total of 6 blood cultures were taken, one bottle of blood culture from each arm of the patient for three consecutive days. All bottles yielded growth of Neisseria animaloris. All isolates were identified by PhoenixTM 100 (Becton Dickinson, Diagnostic Instrument System, Sparks, MD, USA), and the identification was confirmed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) (Bruker Daltonik, Bremen, Germany). Antibiotic susceptibility results were studied by gradient diffusion method, and interpreted as resistant to penicillin G and susceptible to ceftriaxone by the European Committee on Antimicrobial Susceptibility Testing (EUCAST) 2013 standards for N. meningitidis. No risk factor such as animal contact or bite, and tooth or gum problems was detected in the patient evaluation because of the feature of the causative agent. Definitive infective endocarditis was diagnosed according to modified Duke criteria because of fever, infective endocarditis-prone cardiac condition and pulmonary embolism, vegetation in echocardiography and growth in consecutive blood cultures. Although there are some reports that N. animaloris causes wound infections in the cases bitten by a dog or a cat, there is no information about it as a causative agent for infective endocarditis. Our case is the only case of infective endocarditis that is caused by N. animaloris. Diagnosis of Candida infections should be established rapidly in order to initiate the efficient treatment early. Diagnosis of Candida infections is a very challenging process for clinicians due to comorbidities and complex clinical conditions in the critically ill patients. Growth of the agent in blood, sterile body fluid and tissue cultures or histopathological diagnosis are the main methods for the diagnosis. Methods: The study was planned retrospectively between May 2012 and April 2014. Patients who were followed in the oncology outpatient clinic and whose chemotherapy was initiated due to solid organ malignancy were included. IBM SPSS Statistics for Windows. Version 22.0 (Statistical Package for the Social Sciences, IBM Corp., Armonk, NY, USA) program was used for statistical evaluation. Results: As a result of the study, the data of 1476 regular follow-up patients were reached. 49.4% of the patients were male. The mean age was calculated as 60.6±13.3. Of the patients, 864 (58.1%) were screened for HBV infection. For screening, HBsAg and anti-HBs were used together in 53.8% of the patients, and only HBsAg was used in 4.3% of them. Anti-HBc IgG was not used in any of the patients. HBsAg positivity was 3.5% and antiHBs positivity was 37.9% of the screened patients. Of HBsAgpositive patients, 74.1% was started prophylactic antivirals (entecavir in 2 patients, lamivudine in 21 patients), whereas eight patients did not receive prophylaxis. Conclusions: It was determined that HBV serology was investigated in most of the patients who would receive chemotherapy but the serologic parameters used for investigation were not suitable in most cases. Further studies are needed to increase awareness on this issue.
