Mahi","doi":"10.35248/2329-6925.19.7.384","DOIUrl":"https://doi.org/10.35248/2329-6925.19.7.384","url":null,"abstract":"Predominantly found in the descending thoracic aorta, Penetrating Atherosclerotic Ulcer (PAU) is uncommon in the infra-renal abdominal aorta. It is defined as atherosclerotic lesions with ulceration of the aortic intima and media, and rupture of the internal elastic lamina. PAU is considered as a rare serious clinical entity that may induce aortic dissection, aortic rupture, and secular aortic aneurysm typically occurs in elderly hypertensive patients with severe systemic atherosclerosis disease. The treatment of PAU remains a subject of considerable controversy. With the development of percutaneous techniques of stent placement, stenting has emerged as a new option in the treatment of abdominal aorta PAU. We report a 68 years old man with PAU being diagnosed with symptoms of intermittent claudication of both lower limbs; that underwent intraluminal stenting. The technical success rate was 100%. We obtained excellent short and medium term clinical success without finding any complications related. Although open surgical repair is an effective therapeutic option, endovascular treatment of abdominal aortic PAU is a safe attractive alternative to surgery therapy that should be considered as the treatment of choice. Vascular manifestations or angio-Behcet are dominated by venous thrombosis (80%), arterial involvement is rare, but they are often multifocal revealed much more by aneurysms than thromboses, whose risk is the rupture that can engage the life-threatening. Medical therapy with corticosteroids and immunosuppressants should be considered before and after any surgical treatment. We report the case of a 35-year-old man with a history of Behcet’s disease, admitted as part of the emergency for a preoperative carotid bulb aneurysm, who had undergone a flattening and closure of the internal carotid artery by prosthetic patch reinforced by pladjets. TH is practised mainly in the treatment of adult cardiac arrest and neonatal hypoxic-ischemic encephalopathy. Despite the improved benefits of mild TH, there are commonly overlooked complications associated during management. TH may cause an increase in insulin resistance and a reduction in insulin levels which result in hyperglycemia. Studies showed a significantly increased incidence of meningitis, pneumonia and wound infections. Hypothermia was also reportedly associated with increased blood loss and transfusions. In addition, electrolyte abnormalities such as hypokalaemia, hypomagnesaemia, hypophosphatemia, hypo- and hyperglycaemia have been described during TH. The procedure has also been shown to incite mild metabolic acidosis. We conducted this study to review the stenocclusive pathology of the subclavian artery, and to expose our experience in surgical and endovascular management. Material and methods: It is a retrospective and analytical study conducted within the department of vascular surgery of Mohammed VI CHU concerning four observations of patients hospitalized between September 2014 and October 2016 for symptomatic subclavian artery stenosis. Results: The average age of our patients was 78 years, 3 women and one man; atheroma was the cause of the stenosis in all cases. Surgical treatment consisted of subclavio-carotid transposition in 2 cases, carotid-bypass grafting in one case and angioplasty with stent placement in the latter case. Conclusion: It is important for the surgeon to have a thorough understanding of the anatomy and etiology of subclavian artery stenosis, while considering the surgical options. This study assessed the use of CKD-EPI and 4v-MDRD for early diagnosis of renal impairment in pregnant women with hypertension. Methods: This case-control study was conducted at Suntreso Government Hospital Kumasi, Ghana. In all, 220 pregnant women were recruited, 84 had Gestational Hypertension, 36 had Preeclampsia, with 100 normotensive pregnant women as controls. Structured Questionnaires were used to obtain socio-demographic and clinical information. 4 mL of venous blood was collected for estimation of electrolytes, urea, creatinine and uric acid; urine was collected for estimation of protein using dipstick. The Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) and Modification of Diet in Renal Disease (MDRD-4) equations were used to assess and classify renal impairments. Results: Prevalence of renal impairment using CKD-EPI and MDRD-4 was 4.1% and 0.5% respectively. CKDEPI identified 22.2% of women with preeclampsia as having renal impairment whereas MDRD-4 identified 2.8%. Using CKD-EPI and MDRD-4, eGFR was significantly higher in the controls compared to the cases, (p<0.001). Sodium, chloride, urea, creatinine, uric acid was significantly increased in the hypertensive women. Conclusion: Renal impairment is common in hypertensive pregnant women. On perioperative TOE we noted an echogenic structure projecting into the left atrium just above the anterior mitral valve leaflet. Approximately half of the patients have no complaints, and the diagnosis is, made incidentally on the chest radiograph done for different issues. However, symptoms may arise from compression of the mass on the trachea, the recurrent laryngeal nerve, the esophagus, the superior vena cava or other mediastnal structures. Invasive thymoma with transcaval extension to the right atrium is a rare cause of superior vena cava syndrome. We present a case of a 63-year-old man presenting with dyspnea on exertion and facial and upper extremity swelling. Physical examination showed marked edema of face, upper torso and distention of superficial veins of the anterior chest wall and jugular veins. On elaborate investigations, it was revealed to be a case of Type B1 Masoka IVA invasive thymoma.
