Peer support programs have shown a positive impact on care teams by focusing on second victim mitigation and support.</p><p><strong>Purpose: </strong>To determine the type of events and resource needs for a peer support program.</p><p><strong>Methods: </strong>This program evaluation piloted a 6-month peer support program and gathered prospective data on the volume, type, and length of debriefing events.</p><p><strong>Results: </strong>During the pilot, 192 referrals were made to the peer support program. This included 102 programmatic responses. To provide sufficient nutritional support, enteral nutrition (EN) is the preferred method for patients admitted to the neurology ICU.</p><p><strong>Purpose: </strong>The purpose of this study was to examine the clinical effectiveness of an evidence-based nursing protocol for preventing and managing mechanical complications of EN for patients admitted to the neurology ICU.</p><p><strong>Methods: </strong>A randomized controlled trial design was used. The control group received general care, and the test group received care per the evidence-based nursing protocol.</p><p><strong>Results: </strong>A total of 118 patients participated in the study (59 each in the control and test group). {"title":"Mobility Loss in Hospitalized Adults Predicts Poor Clinical Outcomes.","authors":"Stephanie E Rogers, Jade Mulvey, Roniela Turingan, L Maurine Coco, Colin C Hubbard, Sasha Binford, James D Harrison","doi":"10.1097/NCQ.0000000000000816","DOIUrl":"10.1097/NCQ.0000000000000816","url":null,"abstract":"<p><strong>Background: </strong>The Johns Hopkins Activity and Mobility Program is a systematic approach to measure and improve patient mobility.</p><p><strong>Purpose: </strong>The purpose of this study was to evaluate the relationship between mobility loss and quality outcomes.</p><p><strong>Methods: </strong>A retrospective cohort study design was used. Patients were categorized into 3 groups (gain, loss, no change in mobility) using the Johns Hopkins Highest Level of Mobility (JH-HLM) scores. The association between mobility loss and falls risk, in-hospital mortality, delirium, discharge to a facility, length of stay, and 30 day readmissions were assessed.</p><p><strong>Results: </strong>Those who lost mobility were more at risk of being a high fall risk, in-hospital mortality, delirium, discharging to a facility, and had 48% longer lengths of stay. {"title":"Early Identification and Referral of Patients With Diabetic Foot Complications in the Emergency Department.","authors":"AnChi Liu, Eleanor Stevenson, Michael Menchine, Sheila Mallett, Christina Castillo","doi":"10.1097/NCQ.0000000000000806","DOIUrl":"10.1097/NCQ.0000000000000806","url":null,"abstract":"<p><strong>Background: </strong>Prolonged length of stay (LOS) in the emergency department (ED) and lack of post-ED follow-up pose a risk of worsening infection and amputation among patients with diabetic foot complications.</p><p><strong>Local problem: </strong>Excessive ED LOS posed a risk of delayed foot care, and triage providers underutilized post-ED telehealth referrals.</p><p><strong>Interventions: </strong>A graphic icon on the ED dashboard, nurse-initiated order set, and staff education were implemented.</p><p><strong>Methods: </strong>A pre-/postimplementation design was used. Outcomes included usage of the graphic icon and order set, ED LOS, and telehealth referrals.</p><p><strong>Results: </strong>Use of the graphic icon and order sets significantly increased ( P < .001). The rate of telehealth referrals upon discharge also increased but was not significant ( P = .086). Interestingly, LOS increased after the intervention.</p><p><strong>Conclusion: </strong>Using the graphic icon and order set can streamline patient referral to telehealth care. {"title":"Implementing a Nurse-Driven Early Ambulation Protocol to Enhance Post-Transcatheter Aortic Valve Replacement Outcomes: A Quality Improvement Initiative.","authors":"Katie P Worthington, Erin Giannantonio","doi":"10.1097/NCQ.0000000000000835","DOIUrl":"10.1097/NCQ.0000000000000835","url":null,"abstract":"<p><strong>Background: </strong>Immobility in patients undergoing transcatheter aortic valve replacement (TAVR) is linked to adverse outcomes and prolonged length of stay (LOS). Advancements in TAVR procedures allow for early ambulation post-procedure.</p><p><strong>Local problem: </strong>In a large teaching hospital, ambulation occurred at an average of 16.9 hours post-TAVR procedures, higher than the goal of 6 hours.</p><p><strong>Methods: </strong>A pre/post-implementation design was used. Data on ambulation times, LOS, and discharge disposition were collected and analyzed.</p><p><strong>Results: </strong>Time to the first ambulation significantly decreased from a mean of 16.9 (SD = 13.1) to 8.9 hours (SD = 5.3), P < .001. Patients meeting the goal of ambulation 4-6 hours post-procedure had a significantly lower LOS, P < .005. Fewer patients needed a higher level of care at discharge, P = .064.</p><p><strong>Conclusions: </strong>Early ambulation reduced LOS and had a positive impact on patient discharge disposition.
