ABSTRACT Introduction: More than 24% of the world’s population, are infected with soil-transmitted helminth infection. Aim: To assess the helminthic load & efficacy and safety of various antiheliminthics. Methods: A total of 400 patients who met the inclusion criteria were recruited after proper consent. Subjects were divided into Four groups (100 each) randomly & receive different antihelminthic drugs. Cure rate and egg reduction rate was used as a primary outcome measure. Three weeks later two more stool samples were analyzed by two methods 1) Direct Smear method & 2) Concentration method. On follow-up, each group were randomly assigned a single dose of albendazole (single 400 mg dose), mebendazole (100 mg, BD×3 days), pyrantel pamoate (11 mg/kg orally once) and ivermectin (0.2 mg/kg orally once) using a randomization procedure.The data was statistically analysed. Observations: Among 400 patients, 63% were 5-19 years of age, 68% were male, 73.5% were from rural areas, 71% were literate, 62% were in the low income group, 58% reported closed sanitation, 6.75% were having low BMI, 26.25% were having High Absolute Eosinophilic Count before treatment, 77.5% were non-anaemic. 92.5% did not have icterus. In the present study it was seen that the difference among the four drugs was statistically significant as per demographic variables. Significant decrease of worm load after treatment were comparable with each other. However, Presence of Worm across papillae seen after upper GI endoscopy and persistent biliary ascariasis was comparable in all the four drug groups. Overall, the efficacy of anthelmintic drugs was excellent and statistically significant (p<0.001). Conclusion: Albendazole and mebendazole having cure rates of 90% and also reducing the mean egg count by 90%. Also treatment of school children every 4 monthly may be necessary in high endemic areas, like ours. OGH Reports. 2018;7(1):38-40. ABSTRACT Antithyroid drug therapy is routinely used for treatment of hyperthyroid patients. Therapy induced changes in hyperthyroidism can cause serious diagnostic dilemma on FNAC and have been rarely documented. We present a case of 45-year-old female patient who clinically presented with diffuse thyroid swelling and features of hyperthyroidism. FNAC from thyroid swelling showed features of Hashimoto’s thyroiditis / Grave’s disease / Colloid goiter with follicular hyperplasia and at places raising the suspicion of malignant transformation. Further clinical evaluation of patient, it was found that she had carbimazole therapy. We present the case of a middle aged cirrhotic who presented with severe fatal oro-facio-brachial dystonia secondary to intravenous levofloxacin use. Levofloxacin has been very rarely shown to be associated with such severe involuntary movement disorders. ABSTRACT Peutz-Jegher polyps are hamartomas of the small bowel which occur as part of the PeutzJegher syndrome, a rare autosomal dominant condition characterized by gastrointestinal polyps and mucocutaneous melanotic pigmentation. Here we report the case of a young male suffering from this syndrome who presented with acute intestinal obstruction due to intussception. Exploratory laparotomy was done which revealed jejuno-jejunal and jejunoileal intussusceptions for which resection and anastomosis was done. There were multiple intraluminal polyps in the jejunum which on histopathological examination, showed features of Peutz-Jegher Polyp. ABSTRACT We present an extremely rare case of Mantle cell Lymphoma with concomitant alcoholic liver disease and pancreatitis in a 48 year old male patient. There was absence of abdominal lymphadenopathy. The complete blood count showed absolute lymphocytosis which on subsequent peripheral blood examination showed small to medium sized atypical cells raising suspicion of prolymphocytic leukemia. The flow cytometry and Immunohistochemistry examination confirmed Mantle cell lymphoma. The morphology of cells in mantle cell lymphoma are known to mimic prolymphocytes on peripheral blood. ABSTRACT Hepatic angiosarcoma is a very rare disease, accounting for only 2% of primary liver malignancy. They are rare tumours with poor prognosis, with patients usually dying within 6 months. Metastases mainly occur in lymph nodes, spleen, lungs, bones and adrenals. There are some studies that reported the use of Fluroine -18 Fluorodeoxyglucose (FDG)-positron emission tomography (PET)/ computer tomography (CT)(F 18 FDG PET/CT) in angiosarcomas (hepatic, cardiac, venous and cutaneous angiosarcomas) in the literature. Bilateral adrenal metastasis from angiosarcoma of liver is a rare phenomenon.
