{"title":"A study of evaluation of anterior segment changes in eyes with pseudo exfoliation syndrome including anterior chamber depth status of lens anterior chamber angle preoperatively","authors":"Girish Surlikar","doi":"10.26611/10091934","DOIUrl":"https://doi.org/10.26611/10091934","url":null,"abstract":"Background: Pseudoexfoliation syndrome is an important ocular manifestation of a systemic disease, found to be common in cataract patients as well as in 50% of glaucoma patients. Present study was intended to assess the profile of Pseudoexfoliation syndrome and evaluate the surgical outcome of Manual Small Incision Cataract Surgery in Pseudoexfoliative eyes. Material and Methods: Present study was prospective, observational, hospital-based study, conducted in patients with age more than 50 years, with Pseudoexfoliation, admitted for cataract surgery, posted for Manual Small incision cataract surgery(MSICS). All patients underwent a manual small incision cataract surgery, and visual outcomes of the procedures were recorded on the first postoperative day. Results: In present study, maximum numbers of patients were from the age group of 70-79 years (62.5%) and 56 (70%) patients were males and 24 (30%) were females. In this study of 80 patients with Pseudoexfoliation Syndrome, 60 patients had Bilateral Pseudoexfoliation i.e. 120 eyes (85.71%) while 20 patients had Unilateral Pseudoexfoliation i.e. 20 eyes (14.29%). In majority of the eyes 111 (79.29%) had involvement of lens with Pseudoexfoliation while 109 (77.86%) had pupillary involvement. Mean Anterior Chamber Depth in Pseudoexfoliative eyes was 2.50±0.22 mm. Majority of the eyes i.e. 74 (52.9%) were having IOP between 16 – 20 mm Hg. 61 eyes (43.6%) had IOP between 11 – 15 mm Hg. 80 eyes of 80 patients underwent MSICS, 17 (21.25%) patients developed Intraoperative Complications. Conclusion: There is significant association between Pseudoexfoliation syndrome and age, male preponderance and bilateral involvement of eyes. Method: All the school students between 3rd standard (8 years) to 10th standard (16 Years) were examined in their school using Snellens chart. Any child wearing glasses, who had difficulty in reading are having any other symptoms like headache watering were referred to our centre for dilated retinoscopy, fundoscopy and post mydriatic test and to prescribe the glasses. Results: A total of 1986 children were examined of which 194 students had refractive error of which 123 were myopic and 48 were hypermetropic and 23 were having astigmatism (>0.50) Myopia was higher in the female gender of the 194 students 178 students had Bilateral refractive error and 16 students had unilateral refractive error. To assess the reasons for noncompliance in these patients. Methodology: This study was an Observational study which was conducted in Ophthalmology OPD at Kodagu Institute of Medical Sciences, Madikeri, from March 2020 to June 2021. Patients selected were those on topical medications for primary open angle glaucoma of age > 18 years. 95 patients were selected and where given questionnaire. The formula used here to calculate the sample size was sample size n= Za2 p(1-p)/ e2. The prevalence of POAG is 3.51 %(1 Hence n=83. We have included 95 patients in our study. The statistical analysis was done using percentage, frequency and Chi square test. Approval from the ethical committee of the institution was taken before starting the study. Informed consent of the patient was taken. Patients were given the questionnaire and were asked to answer. The questionnaire included- mentioning the details of the antiglaucoma drug being used, duration of usage,missing of doses, reasons for missing the dosage etc was provided to the patients. Results: Total number of 95 patients took part in this study. Among them males and females were 45.2% and 54.7% respectively. Missing of dosage in the previous two weeks was considered as non compliant with regimen. Results showed that 28 out of 95 patients, (29.4%) were non compliant to the drug regime. Conclusion: In glaucoma patients, adherence to topical antiglaucoma medications is a major challenge As antiglaucoma therapy has to be used for lifelong and requires continuous care, awareness has to be created regarding the consequences of missing their medication and the importance of adhering to prescribed regimens. Different aspects about the cost of the medication, drug usage frequency, and side effects has to be considered and not just intraocular pressure control.
