Prevention strategies using traditional risk factors miss many opportunities, because these risk factors do not identify the atherosclerotic plaques that are responsible for the target ischemic events. We present a new technology that could be used to develop a new prevention strategy by providing a low-cost tool for detection of coronary artery disease (CAD). This technology can be used in a decentralized primary care setting. This technology uses ultrasound for measuring minute local vibrations in the cardiac wall associated with acoustic markers of CAD. We have developed a programmable ultrasound platform that has enabled the rapid translation of this technology from the research laboratory to clinical evaluation. Preliminary feasibility studies indicate the promising potential of this technology for detecting and classifying CAD in a symptomatic population. For more effective secondary prevention of patients with diagnosed CAD, this technology can be used in conjunction with another new ultrasound-based method for measuring local variation of mechanical strain in the carotid artery wall as a surrogate for monitoring the progression or regression of CAD","PeriodicalId":164569,"journal":{"name":"1st Transdisciplinary Conference on Distributed Diagnosis and Home Healthcare, 2006. To this end, we developed a prototype wearable monitoring system based on interoperability standards that demonstrates plug-and-play wireless connectivity between the system components. The system utilizes both device-level (IEEE 11073, Bluetooth) and system-level (Health Level (HL7), CORBA) standards. The wearable monitoring system stores data in a local database, and these data are then sent to a remote database via HL7 messages. The remote data can be viewed and processed with a graphical user interface created in Java that employs the CORBAmed PIDS and COAS services as implemented by OpenEMed. The proposed system uses dynamic feedback to maintain constant low cuff pressure at 40 mmHg. Thus, long term continuous blood pressure waveform monitoring is possible. Additionally, the two blood pressure waveforms can be used to compute the system function, frequency response and impulse response of vessel system for system dynamic studies. Ten normal young males were recruited for this study. Continuous blood pressure signals from upper arm and wrist were obtained by the proposed system. During steady state, correlation coefficients of systolic pressure and diastolic pressure, between upper arm and wrist, are 0.83plusmn0.15 and 0.81plusmn0.14, respectively. During cold pressor, correlations decrease slightly. Additionally, the blood vessel system model was evaluated using autoregressive exogenous model (ARX) model. The dynamic changes of blood vessel system during task can be seen from the 3D time-varying impulse responses. In order to assist the observation of the dynamic characteristic change of blood vessel system during different physiological conditions, the 90% energy curve was proposed and used. Before the end of cold pressor, the 90% energy curve reviewed that system energy accumulates faster than steady state indicating the vessel system shifted to higher frequency. The approach is called DLC2 (Diagnostically Lossless Compression-2). To get lossless effect, we use lossy coding followed by error image coding. An NNVQ (Neural Network Vector Quantizer) is used for lossy compression and Huffman coding is used to code the difference image losslessly. It is an spatial domain technique. No frequency domain transformations are required, this makes the proposed scheme simple and computationally economical. Pulse transit time (PTT) is one of the potential parameters which can be utilized for non-invasive and continuous monitoring of BP. PTTp and PTTf are the time intervals measured from the R-wave of electrocardiographic signal to the peak and the foot of photoplethysmographic signal respectively. In this study, the relationship between systolic blood pressure (SBP) and PTTs on 12 healthy subjects, aged 25plusmn2 years, was investigated after exercises. Results showed that SBP was correlated with PTTp and PTTf wherein the means of individual correlation were r=-0.77 and r=-0.92 respectively. The means of the standard deviation of the difference between the measured SBP and the SBP predicted from regression lines in scatter plots of SBP~PTTp and SBP~PTTf were 6.6 mmHg and 4.3 mmHg respectively.
