Abstract Moyamoya disease is a blood vessel disorder that occurs in the brain on the internal carotid arteries and its branches. The blood vessel in the carotid artery is narrowed and blocked, reducing and preventing blood flow to the brain. The condition results in the nar rowing of the vessels distributing oxygen-rich blood to the brain. This report explores the results and effects of the disease, its pathophysiological, biochemical, genetics and scientific analysis of the disease. The first confirmed COVID 19 (+) case in Turkey was detected on March 11th, 2020. The first COVID-19 case in Antalya was detected on March 14th, 2020 and transferred by an Antalya 112 emergency ambulance to hospital. The aim of this descriptive, retrospective and cross-sectional study which is conducted between the dates of March 14th, 2020 and May 31st, 2020, is to evaluate the level of Personal Protective Equipment (PPE) usage, the risk analysis results and follow up measures of Antalya 112 ambulance teams during COVID-19 outbreak. There were 5344 possible and 787 confirmed COVID-19 (+) positive ambulance cases between the dates of study conducted. The majority of these cases were male (62%) and over the age of 65 (47%). The majority of cases were result in transferred to healthcare facilities (75.48%). The total 2361 ambulance team workers were taken to risk analysis measurements and then followed-up procedures. The majority of ambulance team members were assessed with no risk available. The Level 4 PPE (N95/FPP, Goggle/face protection, Gloves, Apron/coverall) were used most commonly (84.50%). The HCWs who use Level-4 PPE for confirmed COVID-19 (+) cases, were assessed no risk exposed. The using mask on patient and the level of PPE usage showed negative correlation with risk level of HCWs. There are only 2 HCWs diagnosed with COVID 19 by CT scan. The follow up procedures of two HCWs has been finalized, and both of them cured, follow ups ended. The studies about easy use, hightech PPE with maximum protection, are recommended for further investigations. The technique with the Lichtenstein Plug since 1989 has allowed to obtain a lower rate of complications and recurrences, as well as an early recovery of the patients usual activities. Its application, widely spread in elective surgery, can also be performed in emergency surgery. The aim of this work is to review the experience of our basic group of work in the surgical treatment of Femoral hernia using this technique. Methods A retrospective descriptive observational study was conducted In our basic work group from the surgery service of the General Teaching Hospital “Enrique Cabrera” between 2009 and 2018, to which this surgical technique was applied. We study the anatomical variants of hernias as well as post-operative complications and clinical evolution. Results The mean age of the patients was 58.7 years (19-92 years), being the female with the highest incidence 78%, as well as, the most frequent location the right, 67.5%. The prosthesis used in the hernioplasty was that of polypropylene. Local anesthesia was applied to 29 patients (63%) of them. The average surgical time was25 minutes, (15-65 minutes). Ambulation was early and the average hospital stay was lessthan 24 hours, in most patients. Only one infectionof the wound and one hernia recurrence in one patient was confirmed. Abstract The authors discuss a case of a rare complications of vascular access creation in a hemodialysis patient “ischemic monomelic neuropathy”. This is an ischemic axonal injury to the nerves supplying the left hand after arterio-venous graft placement for hemodialysis. J Emerg Med Trauma Surg Care 6: 027. Abstract Introduction: Gastrointestinal (GI) injuries with associated pelvic fractures caused by ballistic trauma result in local contamination. Recent literature has suggested that irrigation and debridement may not be necessary despite GI contamination. Although this approach my not alter the incidence of osteomyelitis, there is a lack of evidence that irrigation and debridement of bone fragments could alter the incidence of retroperitoneal and intra-abdominal abscesses. We hypothesize that contaminated and devascularized bone fragments from Extra-Articular Pelvic Fractures (EAPF) can become a nidus of recalcitrant intra-abdominal and retroperitoneal infections. The goal of this study was to examine the outcomes of contaminated EAPF in ballistic trauma managed without local irrigation and debridement. Methods: A 1-year prospective study of consecutive adult patients presenting to a Brazilian level 1 trauma center with abdominopelvic gunshot wounds and EAPF or GI injury that did not undergo operative debridement and washout of the soft tissue neighboring the fracture site. The main outcome measure was development of soft tissue infectious complication defined as peritoneal or retroperitone - al abscess. Results: A total of 82 patients were divided into 3 cohorts: A) EAPF with an associated GI injury (n=32, 39%), B) EAPF with a urological injury or no associated GI or urological injury (n=21, 26%), and C) isolated GI injury without an associated EAPF (n=29, 35%). Overall, 14 patients (17%) developed an infectious complication, 11 patients from cohort A (34%), 1 patient from cohort B (5%), and 2 patients from cohort C (7%). The odds of developing an infectious complica- tion in cohort A were 10.5 times higher compared to cohort B and 7.1 higher compared to cohort C. Cohort A was found to have significant - ly higher readmission rates and lengths of stay. Compared to other areas of the gastrointestinal tract, the odds of developing an infec- tious complication were 6 times greater for an injury to the rectum. Conclusion: GI injuries with pelvic fractures caused by ballistic trau- ma are associated with up to 10 fold increase in retroperitoneal and intra-abdominal abscesses. Herein, we report an 17-year-old boy who sustained a single buttock stab wound that was associated with isolated bladder injury. It is aimed in this report to remind the clinical and therapeutic features of this rare trauma together with a brief literature review on this issue. The quick Sequential Organ Failure Assessment (qSOFA) is an accepted tool to identify patients with suspected infection at risk of negative evolu-tion.This study aims to evaluate whether qSOFA can predict the risk of intra-hospital mortality in cirrhotic patients with suspected infection presenting in the Emergency Department; moreover this study aims to evaluated if a new score, T-qSOFA (that altered if qSOFA was ≥ 2 or body temperature >38°C), increases the accuracy of qSOFA. Methods: qSOFA and T-qSOFA were calculated in 108 cirrhotic pa- tients with suspected infection enrolled during 24 months. Results: qSOFA was ≥ 2 in 9 patients whereas T-qSOFA was al tered in 27.20 patients died (4 with qSOFA ≥2 and 7 with tempera ture >38°C). qSOFA had a high specificity to identifying patients with better prognosis but its sensitivity is low. “T-qSOFA” increases sensitivity and positive predictive value and became an independent predictor of mortality in the multivariate analysis.
