{"title":"An Audit and Analysis of Causes of Treatment Default in Head and Neck Cancer: A Single Tertiary Cancer Centre Experience","authors":"S. Baral, S. This not only compromises the management plan but also makes it harder to measure our treatment outcomes. Method: We reviewed Outpatient department (OPD) clinical record files of head and neck cancer patients who were registered at our hospital in a year. Patients were chosen on the basis of availability of record files at the time of sampling. All patients' OPD files were evaluated for treatment completion and causes of treatment default were recorded from the OPD record file. If information was not available in the OPD record file, patients or patients' relatives (if a contact number was available) were called for further information and cause of default. The Google spreadsheet was prepared to record demographics and causes of treatment default. Results: 72.19% were male among 205 patients analysed for the study. Most common site was oral cavity (30.24%), followed by oropharynx (21.9%), larynx (20%) and others. Intent was curative in 175 patients (85.36%) and palliative in 22patients (10.73%). 112 curative intent patients (64%) and 11 palliative intent (50%) completed planned treatment, 27 curative intent patients (15.42%) and 7 palliative patients (31.81%) defaulted during treatment and 36 curative patients (20.57%) and 4 palliative patients (18.18%) defaulted before starting Primary treatment (Surgery or Radiotherapy). Treatment related toxicities in 20 patients (26.31%) waiting time for radiotherapy in 14 patients (18.42%) were major causes of default in this study. In curative intent cases, 44 patients (39.28%) had complete response, 41 patients (36.3%) had partial response, 10 patients (8.92%) had progressive disease and 6 patients (5.35%) had metastatic disease, 3-month post treatment. Conclusion: Among various reasons for noncompliance, few can be addressed immediately like arranging multidisciplinary team discussions at an institutional level to prioritize management. Methods : We conducted a survey over a period from December 27, 2022 to March 26, 2023 in the city of Ouagadougou. A total of 341 nurses working in the Health and Social Promotion Centers participated in the survey by completing a questionnaire. They were asked to produce a series of five words that came to mind spontaneously from the word “cancer” inducer then to rank these words in descending order of importance according to their judgment. The subjects surveyed were finally asked about their practices in front of signs suggestive of cancer. These practices have been categorized into “I refer” and “I do not refer” to a higher level of care. We compared these two groups through a prototypical analysis using the Iramuteq software. Results : In group 1, the central core was composed of 8 words which are: Pain, Incurable, Suffering, Breast, Nodule, Chemotherapy, Expensive, Tumor. The second periphery was composed of: Death, Fear, Metastasis, Chronic, Expenditure, Compassion, Serious. In group 2, the core was composed of 15 words which are Death, Pain, Tumor, Suffering, Expensive, Metastasis, Nodule. The first periphery was composed of the following words: Fear, Incurable, Chemotherapy, Chronic. The maximum tree of representation indicates that the words cited in both groups are organized around the words “death”, “pain”, “incurable” and “fear”. Conclusion : This difference in perception of the disease has a probable influence on their readiness to refer patients to health centers better equipped for patient management. Since one of the main factors determining the pathogenic effect of HPV infection is local immunodeficiency, a number of authors suggest using various cytokines and immunomodulators. Recently, peptide-based agents have been increasingly used in routine clinical practice. Objective . To study the effect of peptide immunocorrection on molecular and cellular factors affecting the course of grade II cervical dysplasia (CIN2) as a precancerous process and to evaluate the effectiveness of the proposed regimen. Materials and methods . The study included 2 groups of patients aged 28 to 44 years who were colposcopically and cytologically diagnosed with grade 2 cervical dysplasia. Clinical monitoring, determination of HPV by PCR, immunohistochemical studies of biomarkers of inflammation and proliferation, as well as counting of T-lymphocytes in cervical mucus preparations were performed before treatment, after treatment, after 6 weeks and after 3 months. Results . During the study of the expression level of pro-oncogenic interleukin IL-8 and anti-oncogenic IL-12, it was established that in both observation groups after the immunotherapy, a tendency to decrease the pro-oncogenic potential of cervical mucus was noted. Analysis of the level of expression of the cell proliferation marker Ki-67 in the clinical monitoring groups showed that the proposed therapy schemes reduced the proliferative potential of the cervical epithelium. The number of active cytotoxic lymphocytes in the cervical mucus in both observation groups predictably increased after the use of local immunotherapy. The study showed that immunotherapy of cervical dysplasia with the use of peptides demonstrated significantly better clinical results compared to the reference group. Conclusion . Methodology : This was a multicenter cross-sectional descriptive study, using retrospective data at the six CECAP sites from July 1 to Conakry. Three data collection tools: the individual form, a screening register, and a monthly summary sheet or reporting template, were used to track key single-visit service variables: the number of women screened using VIA, proportion of women screened positive for VIA and proportion of women screened VIA positive who received cryotherapy on the same day, KoBo Collect served the collections and the analysis was carried out by the software EPI info version 7.2 6. Results : during our three-month study, in the six pilot sites of Conakry, 786 women were screened, of whom 3.18% (25/786), had IVA+, 62.5% benefited from cryotherapy the same day, it was postponed in 37.5% of cases, the program did not train providers in the management of large lesions, which is one of the limitations of our study. Conclusion : Visual inspection with acetic acid is a simple test, requiring minimal infrastructure and expense, little provider training.
