Balik","doi":"10.5835/JECM.OMU.33.02.003","DOIUrl":"https://doi.org/10.5835/JECM.OMU.33.02.003","url":null,"abstract":"Varangels are simple and uniquely-designed cable car-like vehicles commonly used for human and cargo transportation in the Eastern Black Sea Region of Turkey, a mountainous region. Varangels cause various accidents, resulting even in death. This study investigated the characteristics of the patients with injuries caused by varangel accidents. Medical records of 78 patients (55 males) admitted with injuries due to varangel accidents to Recep Tayyip Erdogan University Training and Research Hospital between January 2010 and December 2012 were retrospectively evaluated. Data including age, gender, site and type of the injury, accident type, and outcomes were recorded. No significant differences were determined among patients in terms of the time and type of the accident, site of the injury and outcomes according to gender or age groups. According to the type of the accident, significant differences were observed regarding the site of the injury and outcomes. Compression injuries appeared to affect the extremities more, whereas head-neck injuries were encountered more often due to falling from high. While patients with impact injuries were mostly treated in the outpatient units, those with compression injuries were treated both in the inpatient and outpatient units. Although no significant difference was determined according to the injury site, two patients who died had head-neck injuries. This study evaluating cable car-related accidents in Turkey is the most comprehensive study, with the highest number of cases analyzed. {"title":"Early period results after cataract surgery with phacoemulsification in monocular patients","authors":"O. Sayın, Ertuğrul Can","doi":"10.5835/JECM.OMU.33.02.004","DOIUrl":"https://doi.org/10.5835/JECM.OMU.33.02.004","url":null,"abstract":"In our study, the increase in visual acuity of monocular patients in the early period following cataract surgery with phacoemulsification (PHACO), the reasons for being monocular and the comorbid pathologies in the operated eye were examined. The files of 24 patients who had PHACO and intraocular lens (IOL) implantation were analyzed retrospectively. The patients who have 0.1 and worse best-corrected visual acuity (BCVA) in the other eye with Snellen chart and whose vision could not be increased by a medical or surgical treatment were accepted as monocular. All operations were performed with clear corneal incision and standard PHACO methods. IOLs were implanted into capsular bag for all patients. The postoperative visits were made on the 1st, 7 th and 30 th days. The mean age of the 24 patients included in the study was 65.0±14.1. Seven (29.1%) patients were female, while 17 (70.9%) were male. Right eyes of 4 (16.6%) patients and left eyes of 20 (83.4%) patients were operated. Comorbid disease was found in 16 (66.6%) of the eyes operated. When non-operated eyes of the cases were analyzed, the first three reasons for being monocular were age related macula degeneration (AMD), phthisis bulbi following penetrating injury and glaucomatous optic atrophy. The mean BCVA was 1.31±0.71 logMAR before the surgery, 0.32±0.57 logMAR on the 7 th day control and 0.14±0.16 logMAR on the first month control. 22 (91.1%) patients had better visual acuity after the surgery and on first month control 18 (75%) patients’ BCVA was equal to or better than 0.3 logMAR. Patients hospitalized with diagnosis of ISSHL and treated in our clinic between 2009 and 2013 were evaluated retrospectively. Individuals received the same treatment were classified into two groups after a month from diagnosis by a pure tone audiogram regarding Siegel’s criteria. Effect of patient’s age, gender, side of ear, degree of hearing loss, patterns of initial pure-tone audiogram, comorbidities such as systemic diseases, time of initial treatment, contralateral ear hearing, presence of vestibular symptoms and tinnitus on the prognosis of ISSHL was evaluated in this study. Eighty two patients (46 male, 36 female, mean age 47.9, age ranging between 21 and 85) were included in this study. Hearing recovery was observed in 50 patients (61%). Recovery rate was significantly better in patients less than 60 years and received a medical treatment in the first seven days. On the other hand, it was lower in patients presenting with profound audiogram and contralateral hearing loss. {"title":"Emotional and behavioral characteristics of childhood depression","authors":"Çilem Bilginer, S. The study was carried out in a child and adolescent psychiatry department of Karadeniz Technical University, Faculty of Medicine, between the dates of 03 March 2013 and 03 March 2014. Forty two children, ranging from 7 to 12 years, diagnosed with major depression, were choosen as study sample. Child Depression Inventory (CDI) was used to evaluate the severity of depression and the behavioral problems of children were evaluated with Child Behavior Checklist for Ages 6-18 (CBCL 6/18). According to the CBCL 6/18, the internalizing, externalizing and total behavior problem scores were found significantly higher (respectively p<0.001; p<0.001; p<0.001) in the study group. The internalizing and total behavior problems were significantly higher in girls, but there was no significant difference in externalizing behavior in both genders. In addition, 57.1% of the study group (n=24) had suicidal ideation and 19% (n=8) showed self-injurious behavior. Suicidal ideation was increasing with child’s depression severity but not related with behavioral problems. In addition, self-injurious was significantly associated with rule-breaking behavior, aggresive behavior and externalizing behavior problem subscales, but not associated with suicidal ideation or depression severity. Consequently, children diagnosed with major depressive disorder ( MDD) had significantly higher behavior problems than their healthy peers. The results suggest that externalizing behavior problems could be a part of childhood depression regardless of gender differences. Besides, the results suggest that self harming behavior in depressive children may not always be related with depression severity. Impulsivity and high comorbidity rates may be additional contributions to this behavior. The disease is thought to occur under the influence of predisposing factors in people with genetic susceptibility. Pemphigus may coexist with other autoimmune diseases such as myasthenia gravis, lupus erythematosus, rheumatoid arthritis, and pernicious anemia as well as with neoplasias such as thymoma, carcinoma, and lymphoproliferative diseases. Crohn’s disease is a chronic inflammatory disease which involves the whole gastrointestinal mucosa from the mouth to the anus. Etiopathogenesis of this disease includes autoimmunity. A 39-year-old Turkish man presented to our polyclinic due to the erythema on the trunk and the shoulders. The patient stated that his complaints had started a month before and were accompanied by pruritus. He had had a history of Crohn’s disease for 10 years. Histopathologic examination of the blister revealed pemphigus vulgaris. Physicians should be able to recognize Crohn’s disease might accompany with autoimmune skin disease. Sixty-five patients with lung cancer (57 with NSCLC and 8 with SCLC) and 57 healthy controls were enrolled. Polymerase chain reactionrestriction fragment length polymorphism (PCR-RFLP) method was used. Genotype distribution of monocyte chemoattractant protein 1 (MCP-1) -2518 A>G polymorphism in lung cancer patients was as follows: 30 for AA, 30 for AG and 5 for GG genotype. Frequency of minor G allele in patients and controls were found as 30.8% and 23.7%, respectively. In patients, genotype distribution of CCR2 V64I polymorphism was as follows: 47 for GG, 16 for GA, and 2 for AA. Frequency of minor A allele was found in patients and controls as 15.4% and 19.2%, respectively. Genotype distribution and allele frequencies of MCP-1 and CCR2 polymorphism were not statistically different between patients and controls (p values >0.05 for both polymorphisms). In lung cancer patients, there was no significant association between smoking status and MCP-1 and CCR2 polymorphisms. Similarly, no significant association was found between distant organ metastasis and both gene polymorphisms. The most common causes of intracardiac masses are thrombi, vegetations and primary or metastatic tumors. A 73-year-old male patient was admitted to internal medicine clinic with the diagnosis of acute postrenal failure superimposed on chronic renal failure. He also had fever and urinary tract infection. Eosinophil count was 1975/mm³ (22.7%). Transthoracic echocardiography (TTE) revealed a 12x10 mm mobile mass attached to right ventricular (RV) lateral wall. The patient was on bicarbonate, sulbactam+cefoperazone 2x1 g, enoxaparine 1x4000 IU and silodosin 1x8 mg, when the patient discharged. One month after discharge, the patient came back for control examination. Surprisingly, control TTE revealed complete disappearance of the RV mass. Eosinophil count was 500/mm³ (4.9%).
