{"title":"Application value of magnetic resonance 3D-SPACE full volume scanning sequence in direct imaging of lower extremity deep venous thrombosis","authors":"Jilin Nie","doi":"10.3760/CMA.J.ISSN.1008-1372.2019.12.014","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1008-1372.2019.12.014","url":null,"abstract":"Objective \u0000To evaluate the capability of magnetic resonance imaging 3D-sampling perfection with application optimize contrasts using different flip angle evolution (MRI 3D-SPACE) full-volume scanning sequence to detect lower extremity deep vein thrombosis (LEDVT), and to explore the characteristics of the images of LEDVT and surrounding soft tissue at different stages. \u0000 \u0000 \u0000Methods \u0000MRI 3D-SPACE full volume T1WI and T2WI scanning of lower limb were performed on 45 patients diagnosed of LEDVT via digital subtraction angiography (DSA) by Siemens Prisma 3T MRI scanner. 43 cases completed scanning. The location and quantity of vascular segment lesions were recorded, of which the diagnostic sensitivity, specificity and accuracy were calculated comparing to the gold standard of DSA results. The characteristics of magnetic resonance imaging signals of thrombus and thrombus surrounding soft tissue at different times were summarized. \u0000 \u0000 \u0000Results \u0000According to the location of thrombosis, there were 15 cases of central type, 17 cases of peripheral type and 11 cases of mixed type. In all imaging levels, 272 segments of embolic vessels were found in 3D-space T1WI and 261 segments in 3D-space T2WI. Compared with the DSA results, the sensitivity, specificity, and accuracy of the two groups in the diagnosis of deep venous thrombosis were: T1 WI: Common iliac vein 97.10%, 97.90%, 98.20%; External iliac vein 96.80%, 97.60%, 97.10%; Internal iliac vein 96.40%, 97.20%, 97.20%; Femoral vein 98.10%, 98.20%, 98.50%; Popliteal vein 94.40%, 93.50%, 93.70%; The anterior tibial vein is 94.30%, 88.50% and 90.40%. The posterior tibial vein is 93.40%, 85.20%, 85.30%. T2WI: Common iliac vein 97.10%, 97.90%, 97.10%; External iliac vein 96.50%, 96.20%, 96.00%; Internal iliac vein 96.20%, 96.60%, 96.20%; Femoral vein 98.10%, 98.20%, 98.50%; Popliteal vein 93.40%, 92.50%, 92.70%; The anterior tibial vein is 90.30%, 86.50% and 85.40%; Posterior tibial vein 89.40%, 84.20%, 83.30%. According to the onset time, 17 cases in the acute phase, the thrombus was equal or slightly higher and uneven signal in the two groups of sequences, the perivascular and muscle tissue gap signal increased; 21 cases in the subacute phase, the thrombus was high in both groups signal; 5 cases in the chronic phase, the thrombus showed equal or slightly lower signal in the two groups of sequences, the blood vessel wall was irregular, and the surrounding soft tissue was mild or no obvious edema. \u0000 \u0000 \u0000Conclusions \u0000MRI 3D-space sequence full volume scanning technology has high sensitivity, high specificity and high accuracy for the diagnosis of lower extremity deep vein thrombosis, and can reflect the characteristics of thrombus and soft tissue signal around thrombus in different periods. According to the pathological examination after operation, they were divided into adrenal adenoma group (86 cases) and adrenal metastasis group (34 cases). The difference of time-density curve between the two groups was compared, and the diagnostic criteria were evaluated by using receiver operating characteristic (ROC) curve. \u0000 \u0000 \u0000Results \u0000Dynamic CT and dynamic MRI were mainly type A, followed by type C, B, D and E. Dynamic CT and dynamic MRI were mainly type B, followed by type D/E, type A and type C. There were significant differences in type A, B, C, E dynamic CT and dynamic MRI between the two groups (P 0.05). The patients were divided into three groups according to the degree of stenosis (atherosclerotic group, atherosclerotic group with degree of stenosis 70%). General information, comorbidities and Pittsburgh sleep quality index (PSQI) of patients in each group were analyzed to compare the differences and analyze the risk factors of aggravated coronary artery stenosis. \u0000 \u0000 \u0000Results \u0000There were statistically significant differences in age, gender and diabetes among all groups (P 0.05). Spearman correlation analysis showed that PSQI score was positively correlated with the degree of atherosclerosis (P<0.01). In the multivariate regression model, poorer sleep quality (OR=1.75, 95% CI: 1.14-2.69, P<0.05), shorter sleep time (OR=1.64, 95% CI: 1.05-2.55, P<0.05) and more diurnal dysfunction (OR=1.45, 95% CI: 1.11-1.88, P<0.01) were correlated with increased coronary artery stenosis. Higher PSQI total score (OR=1.13, 95% CI: 1.06-1.20, P<0.01) and lower PSQI evaluation grade (very bad vs very good: 13.85, 95% CI: 1.56-122.82, P<0.05) were also related to the increase of coronary artery stenosis. \u0000 \u0000 \u0000Conclusions \u0000Poorer sleep quality, shorter sleep time and more diurnal dysfunction were independent risk factors for the aggravation of coronary artery stenosis. All patients were divided into simple varicose veins group (group A, 67 cases) and secondary varicose veins group (group B, 63 cases) according to the patency of deep veins. The data of sex, limb side, swelling and ulcer were analyzed respectively. \u0000 \u0000 \u0000Results \u0000There was no significant difference in the results of sex between the two groups (57.5% vs 63.5%, P>0.05). The incidence of left side in group A was lower than that in group B (59.3% vs 76.2%, P<0.05). And the incidence of swelling of lower extremities and ulcer in boot area in group A was lower than that in group B (31.1% vs 49.2%, P<0.05; 18.6% vs 31.7%, P<0.05 respectively). \u0000 \u0000 \u0000Conclusions \u0000Combined with swelling of lower extremities, ulcer in boot area or the varicose veins in left side had a higher incidence of secondary varicose veins. This case was diagnosed as cerebral infarction with right aortic arch and Kommerell's diverticulum. \u0000 \u0000 \u0000Results \u0000One month after the treatment of cerebral infarction, we successfully used the thoracic aortic stent to isolate the Kommerell's diverticulum. There were no operative complications occurred. Both groups received continuous epidural block combined with general anesthesia, sevoflurane group took sevoflurane combined with remifentanil to maintain anesthesia, while isoflurane group took isoflurane remifentanil to maintain anesthesia. The mean arterial pressure (MAP) and heart rate(HR) of different moments, awakening (open-eye time, extubation time, listening to instruction time, orientation recovery time, time of Aldrete score up to 9 minutes), the serum levels of tumor necrosis factor alpha (TNF-α), interleukin 1 (IL-1), aspartate aminotransferase (AST), alanine aminotransferase (ALT), glutamate dehydrogenase (GLDH), alpha glutathione S transferase (α-GST) levels at different moments were compared between the two groups. \u0000 \u0000 \u0000Results \u0000The MAP and HR at T1, T2, T3 and T4 in both groups were significantly lower than those at T0 (P<0.05). The HR at T1 and T4 in sevoflurane group were significantly higher than those in isoflurane group (P<0.05). The open-eye time, extubation time, listening to instruction time, orientation recovery time in sevoflurane group were significantly shorter than the isoflurane group (P<0.05). The levels of serum TNF-α, IL-1, AST, ALT, GLDH and α-GST in the two groups at T2 and T3 were significantly higher than those at T0 (P<0.05), and began to fall back to T4. Among them, 38 patients with malignant tumors and 40 patients with benign tumors were tested for CDUS and serum CA125. The characteristics of CDUS images and serum CA125 levels in patients with malignant tumors were analyzed by pathological examination as the gold standard. The diagnostic efficacy of CDUS combined with serum CA125 level detection and separate examination was analyzed. \u0000 \u0000 \u0000Results \u0000⑴ Characteristics of CDUS images: the size, location, texture, blood flow signal, resistance index, ascites, peritoneal implant nodule or enlarged lymph node of patients with malignant tumors were significantly different from those of benign tumors (P 0.05); the sensitivity of CDUS combined with serum CA125 level detection was 97.37%. The two were independently examined by 76.32% and 73.68% (P 0.05). \u0000 \u0000 \u0000Conclusions \u0000The serum CA125 level of patients with ovarian cancer is abnormally elevated. The characteristics of CDUS images are different from those of benign tumors, which can effectively identify benign and malignant tumors. Of 36 patients, 55.56% (20) had previously received chemotherapy with paclitaxel. The objective response rate (ORR) was 8.33%, the disease control rate (DCR) was 25.0%, the median progression free survival (PFS) was 106 days, and the median overall survival (OS) was 183 days. The main adverse reactions of grade 3-4 were hematological toxicity, including neutropenia [36.11% (13/36)], anemia and thrombocytopenia [5.56% (2/36) and 16.67% (6/36)] in patients without neutropenia fever. All patients were selected for cancer tissue, and 30 patients were randomly selected from normal gastric mucosa at 5 cm adjacent to the tumor. Immunohistochemistry was used to detect the expression of PKB/AKT and Syk protein. To compare the expression of PKB/AKT and Syk in gastric cancer and adjacent normal tissues, and to analyze the relationship between PKB/AKT and Syk expression and clinicopathological features in gastric cancer tissues, and the correlation between PKB/AKT and Syk and TNM staging of gastric cancer patients. \u0000 \u0000 \u0000Results \u0000The positive expression rate of PKB/AKT in gastric cancer tissues was higher than that in adjacent tissues (P<0.05). The positive expression rate of Syk was lower than that in adjacent tissues (P<0.05). PKB/Akt and Syk gene expression in gastric cancer were related to histological grade, tumor infiltration, TNM staging, lymph node metastasis and distant metastasis (P<0.05), and the expression of PKB/AKT was significantly increased with the increase of TNM staging in gastric cancer patients, and the positive expression of Syk was significantly decreased (P<0.05). \u0000 \u0000 \u0000Conclusions \u0000PKB/AKT is positively correlated with TNM staging of gastric cancer patients. Syk is negatively correlated with TNM staging of gastric cancer patients. The clinical expression of PKB/AKT and Syk can be used to determine the TNM staging of gastric cancer, which provides a strong basis for tumor treatment. According to the presence or absence of portal vein thrombosis (PVT), they were divided into thrombus group (33 cases) and control group (206 cases). According to the presence or absence of EGVB in thrombus group, they were divided into bleeding group (10 cases) and non bleeding group (23 cases). According to whether anticoagulant therapy was used in thrombus group, they were divided into anticoagulant group (10 cases) and non anticoagulant group (23 cases). The risk factors of each group and its control group were observed and compared. \u0000 \u0000 \u0000Results \u0000The thrombus group had a lower level of the albumin (ALB) , higher level of count of platelet (PLT), diameter of main portal vein (MPV), propotion of diabetes prevalence and history of splenectomy compared with the control group (P 0.05]. \u0000 \u0000 \u0000Conclusions \u0000⑴ PLT, ALB, MPV, and a history of diabetes or splenectomy are risk factors for cirrhosis combined with PVT, and PLT and MPV are independent risk factors. ⑵ The incidence of EGVB increased with the increasing severity of esophagogastric varicose vein.
