In addition to periodontal ligament fibroblasts, cells of the immune system such as macrophages are exposed to compressive strain. The aim of this study was thus to investigate the impact of histamine on the gene expression profile of macrophages in the context of simulated orthodontic compressive strain.</p><p><strong>Methods: </strong>Macrophages were incubated with different histamine concentrations (50, 100, 200 µM) for 24 h and then either left untreated or compressed for another 4 h. To assess the role of different histamine receptors, we performed experiments with antagonists for histamine 1 receptor (cetirizine), histamine 2 receptor (ranitidine) and histamine 4 receptor (JNJ7777120) under control and pressure conditions. We tested for lactate dehydrogenase release and analyzed the expression of genes involved in inflammation and bone remodeling by reverse transcription quantitative polymerase chain reaction (RT-qPCR).</p><p><strong>Results: </strong>Histamine elevated gene expression of tumor necrosis factor under control conditions and in combination with pressure application. Increased prostaglandin-endoperoxide synthase‑2 mRNA was observed when histamine was combined with compressive force. Interleukin‑6 gene expression was not affected by histamine treatment. In macrophages, compressive strain increased osteoprotegerin gene expression. Histamine further elevated this effect. Patient records and pretreatment panoramic radiographs were evaluated. The regions of interest selected for calculating fractal dimension were the bilateral mental foramen regions of the mandible. Fractal dimension was set in relation to orthodontic treatment duration using a linear regression model which was also adjusted for potential confounding variables. Total treatment duration was the outcome variable of interest used as a continuous variable. The predictor variables of interest included age, gender, type of dental and skeletal malocclusion, vertical growth pattern, extraction type, and fractal dimension.</p><p><strong>Results: </strong>The mean age, treatment duration, and fractal dimension were 14.56 years, 27.01 months, and 1.23 mm, respectively. Multiple linear regression analysis showed that the fractal dimension had a significant influence on overall treatment duration (P < 0.001). From the other variables, Angle class II malocclusion significantly influenced treatment duration (P < 0.01), age showed a significant negative correlation with treatment duration (P < 0.01), and treatment duration significantly increased for patients with tooth extractions (P < 0.001).</p><p><strong>Conclusion: </strong>There was a negative correlation between fractal dimensions at the mandibular mental region and total orthodontic treatment duration. Quantitative expression analysis of genes encoding the osteogenic markers alkaline phosphatase (ALPL), Runt-related transcription factor 2 (RUNX2), osteocalcin (OCN), and osteopontin (OSP), as well as the fibroblast markers vimentin (VIM) and fibroblast-specific protein 1 (FSP1), was performed after 2 and 20 days of cultivation. Metabolic activity was determined by MTT assay. For comparison with AscA, 50 ng/mL FGF1 was used for stimulation for 2 and 20 days. Cell number, percentage of Ki67-positive cells, and expression of osteoblast- and fibroblast-specific genes were examined. Alkaline phosphatase activity was visualized by NBT/BCIP and calcium deposits were stained with alizarin red. Cytokine (IL‑6, IL‑8, COX2/PGE2) expression and secretion were analyzed by qPCR and ELISA in 6 h mechanically compressed HPdLF cultured for 2 days with FGF1 or ascorbic acid.</p><p><strong>Results: </strong>Higher concentrations of FGF1 promoted cell proliferation upon short-term stimulation, whereas prolonged treatment induced the expression of osteogenic markers even with low concentrations. AscA promotes cell growth more markedly than FGF1 in short-term cultures, whereas FGF1 induced osteogenic cell fate more strongly in long-term culture. An open-loop spring was attached to the upper incisors of each rat to expand the premaxillae. A 5-day expansion period followed by a 12-day retention period was observed. The rats in the SO group received systemic administration of 20 mg SO/kg/day via the orogastric route for 17 days. Histomorphometric examinations were carried out to examine the amount of new bone formation, number of capillaries, and intensity of inflammatory cell response. Immunohistochemical analysis was conducted to examine the number of osteoblasts and osteoclasts. Leaf and root extracts of SO were also analyzed for antioxidant activity and phenolic compounds in vitro.</p><p><strong>Results: </strong>Statistical analysis showed that the following were higher in the SO group than in the control group: new bone formation, number of osteoblasts and osteoclasts, intensity of inflammatory cell response (neutrophils, lymphocytes, and macrophages), and number of capillaries. There are significant correlations between the increase of mandibular size and cervical vertebral maturation. The peak growth velocity of the mandible occurs after the third stage of cervical vertebral maturation. The location of the mandibular foramen (MF) and its changes subsequent to growth are of great interest to clinicians as they relate to the anesthesia of the inferior alveolar nerve and to mandibular surgical procedures. Therefore, the aim of the present study was to assess the influence of the mandibular growth spurt on the location of the MF in various skeletal growth patterns.</p><p><strong>Methods: </strong>Panoramic and lateral cephalometric radiographs of 98 (32 orthognathic, 50 retrognathic, 16 prognathic) patients before and after the growth peak were collected. For each subject, the maturational stage of the cervical vertebrae was defined on successive lateral cephalograms and the vertical and horizontal position of the MF was evaluated on two panoramic radiographs, one before and one after the growth peak.</p><p><strong>Results: </strong>The MF-Post/MF-Ant ratio (MF distance to the posterior border of the ramus/MF distance to the anterior border of the ramus) significantly increased after the growth peak in orthognathic and retrognathic subjects (P = 0.015 and 0.02, respectively). This ratio did not significantly increase in prognathic subjects (P = 0.882). No statistically significant changes in the vertical position of the MF were found in the three groups after the growth spurt.</p><p><strong>Conclusion: </strong>The horizontal position of the MF moves in an anterior direction in orthognathic and retrognathic subjects during the mandibular growth spurt. {"title":"Information on the Internet about clear aligner treatment-an assessment of content, quality, and readability.","authors":"Mehmed Taha Alpaydın,&nbsp;Suleyman Kutalmış Buyuk,&nbsp;Nehir Canigur Bavbek","doi":"10.1007/s00056-021-00331-0","DOIUrl":"https://doi.org/10.1007/s00056-021-00331-0","url":null,"abstract":"<p><strong>Purpose: </strong>The goal was to evaluate the content, quality, and readability of the information available about clear aligner treatment on the Internet.</p><p><strong>Materials and methods: </strong>The search terms \"aligner,\" \"clear aligner,\" and \"Invisalign\" (Align Technology, Tempe, AZ, USA) were analyzed in three search engines (Google [Google LLC, Mountain View, CA, USA], Bing [Microsoft, Redmond, WA, USA], and Yahoo [Yahoo, Sunnyvale, CA, USA]). The first 50 websites for each keyword in each search engine were screened. Duplicate websites, advertisements, links to scientific articles, videos, and other irrelevant websites were excluded. The quality of the remaining websites was analyzed using the DISCERN and Journal of American Medical Association (JAMA) benchmark instruments together with the Health on the Net code (HONcode, Health On the Net Foundation, Geneva, Switzerland). The readability of the websites was evaluated by the Flesch Reading Ease Score (FRES) and Flesch-Kincaid Grade Level (FKGL). Statistical analyses were performed by one-way analysis of variance, Kruskal-Wallis and Fischer's exact tests, with p < 0.05 accepted to be statistically significant.</p><p><strong>Results: </strong>Among 111 evaluated websites, most belonged to multidisciplinary dental clinics (n = 49; 44.2%), followed by aligner companies (n = 26; 23.4%), orthodontists (n = 26; 23.4%), and professional organizations (n = 10; 9%). The mean DISCERN score (sections 1 and 2) for all websites was 29.95/75. The average FRES and FKGL were 55.77 and 9.74, respectively. Professional organization websites had significantly higher DISCERN scores than others (p < 0.001), and together with multidisciplinary dental clinic websites, they showed better compliance with JAMA benchmark criteria. Professional organization websites' FRES and FKGL were also higher than other websites (p > 0.05).</p><p><strong>Conclusions: </strong>Overall, the quality of web-based information about clear aligners was poor and the readability of the data was insufficient. In the experimental groups, closed nickel-titanium closed coil springs with a 50 cN force were placed for 10 days between the maxillary incisor and molar. PBM and HFMV were applied daily over long- (15 days) and short-term (7 days) retention periods. The PBM groups received PBM with a single wavelength (650 nm) or higher wavelengths (532, 650, 940 nm) for 9 min per day. HFMV groups received HFMV of 10, 20, and 30 Hz for 10 min per day. Right and left maxilla were assessed using micro-computed tomography imaging and real-time polymerase chain reaction. The amount of tooth movement during the retention period, expression levels of cyclooxygenase‑2 (COX-2), osteoprotegerin (OPG), and receptor activator of nuclear factor-kappa B ligand (RANKL) mRNA gene expression levels, OPG/RANKL ratios, alveolar bone trabecular thickness (Tb.Th), trabecular number (Tb.N), and structure model index were analyzed. Kruskal-Wallis and Mann-Whitney U tests were used for multiple comparisons of the nonparametric distributed data and binary comparisons, respectively.</p><p><strong>Results: </strong>When using the long-term retention protocol, PBM and HFMV treatment increased Tb.N (p < 0.05) and decreased COX‑2 mRNA gene expression levels (p < 0.05) and Tb.Th (p < 0.05) compared to controls. For short-term retention, PBM and HFMV decreased the amount of relapse tooth movement compared to controls. Patient files including extraoral, intraoral photographs, hand-wrist, panoramic and cephalometric radiographs were randomly distributed. Each group presented their evaluations after 1 week. At the end of case discussions, students were given a questionnaire regarding their perceptions of case-based education. Differences in mean scores between groups were compared by independent samples t‑test.</p><p><strong>Results: </strong>The response rate was 100%. The majority of students strongly agreed that their interest and ability to interpret and diagnose the orthodontic data were increased. More than 60% strongly agreed that this education had a positive effect on their attention and effective learning. More than 45% strongly agreed that their communication with colleagues was increased, and women were significantly more inclined to group work than men (p < 0.05). More than 45% declared no additional burden.</p><p><strong>Conclusions: </strong>Orthodontic case-based education was well received by final year dental students.
