{"title":"Femoral fracture in an amputation stump.","authors":"A Khan, B McGlone, J Stow","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Presentation: </strong>We report a case of a 60-year-old male with bilateral lower limb amputations who sustained a fracture to his right residual femur after weight bearing on his prosthetic leg during therapy. This was attributed to his raised body mass index that fell in the morbidly obese category.</p><p><strong>Diagnosis: </strong>Patient was referred for orthopaedic management and reviewed by the multidisciplinary prosthetic rehabilitation team following the fracture recovery.</p><p><strong>Treatment: </strong>He was deemed unsuitable for further prosthetic usage and ultimately referred to the national weight management service and the bariatric team.</p><p><strong>Discussion: </strong>Fracture of the residual limb in an amputee is not a common occurrence. There is a scarcity of information regarding fractures in an amputation stump of obese individuals. {"title":"Implementing Faecal Immunochemical Testing in Primary Care.","authors":"N Abdullah, J Crowley, M Buckley","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim: </strong>The incidence of colorectal cancer (CRC) has increased globally, in particular patients under the age of fifty. This is a pilot study of a faecal immunochemical testing(FIT) service in primary care. The aim was to trial a FIT service for asymptomatic patients.</p><p><strong>Methods: </strong>We offered FIT kits to asymptomatic patients, aged between 40-75 years old during routine consultations. The number of FITs performed and the outcomes were reviewed. A cut-off of >10μg Hb/g faeces was defined as a positive result.</p><p><strong>Results: </strong>Overall 180 FIT kits were distributed, 7 duplicate tests were given. Of the 173 patients recruited, 142 (82%) samples were analysed in the lab. A total of 126 ( 88.7%) samples had a normal result, 8(5%) were rejected, 31 (17.9%) did not send their sample for analysis. A positive result was found in 8(5%), of these 6 (75%) underwent colonoscopy. No cancer was identified, 4 (50%) had polyps removed and 2 patients require follow up colonoscopy.</p><p><strong>Discussion: </strong>This study represents the first successful implementation of a FIT service in primary care in Ireland. {"title":"Proximal Renal Artery Stenosis Presenting as Uncontrolled Hypertension.","authors":"N Onwasigwe, T M Chia, H O'Neill, C Donagh, D Nagi","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Presentation: </strong>We present a case of 50-year-old lady who present with 1-month history of uncontrolled hypertension.</p><p><strong>Diagnosis: </strong>Magnetic resonance angiography showed stenosis of the left renal artery just beyond the ostium extending over approximately 7mm in length. Computed tomography angiography show focal narrowing of the proximal renal artery just distal to vessel origin, approximately 60% stenosis.</p><p><strong>Treatment: </strong>Anti-hypertensive medication was initiated in the ward. She was referred to vascular surgeon for renal bypass.</p><p><strong>Discussion: </strong>Renal artery stenosis is common cause of hypertension but may go unrecognised. The focus of this case is on the evaluation and necessity for a complete evaluation of the patient who is presenting with uncontrolled hypertension. {"title":"Sternal plating for traumatic sternal non-union.","authors":"J D Kehoe, S Barrett, P Higgins, J Dilworth, D Tuite, J Hinchion","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>Sternal non-union is a rare complication of sternal fracture but one which may impact patients symptomatically or cosmetically. Should conservative measures fail, the indications for operative intervention have become more defined in recent years and new techniques have been developed to restore sternal position and thus correct chest wall shape and function.</p><p><strong>Cases: </strong>We present two cases of aseptic sternal non-union. Case 1 involved a young patient with a protracted history of sternal pain owing to an injury induced by repeated low-grade stress. Case 2 refers to another young individual who presented with persistent pain 1 year after suffering a fractured sternum in a motor bike accident.</p><p><strong>Outcome: </strong>Despite polarising inciting events, both were found to have non-union injuries to the sternum with evidence of pseudoarthrosis. We examine their management with parallel locking compression plate sternal fixation.</p><p><strong>Discussion: </strong>We also evaluate the current literature with respect to sternal plating, novel operative and non-operative alternatives, including recombinant human parathyroid hormone, and peri-operative considerations, such as closed incision negative pressure therapy.
