We report a case of primary LCNEC of the tongue. A 60-year-old man was referred to our department because of impaired swallowing function due to pain while eating, as well as an ulcer on the left side of the tongue. Squamous cell carcinoma was suspected on biopsy. A whole-body examination showed no evidence of metastasis. Therefore, the patient was given a clinical diagnosis of squamous cell carcinoma of the left side of the tongue suspected (cT4aN2bM0: Stage IVa). After preoperative chemotherapy, we performed a tracheostomy, bilateral neck dissection, complete removal of the tongue, and reconstructive surgery using a rectus abdominis free flap, with the patient under general anesthesia. Histologically, the tumor was composed of solid nests of atypical large cells. Necrosis was remarkable. Tumor nests showed peripheral palisading and rosette formation. Immunohistochemically, the cancer cells were positive for chromogranin A and synaptophysin. We diagnosed this tumor as LCNEC, based on the criteria established for pulmonary tumors. The patient is alive without recurrence or metastasis 55 months after surgery. Since LCNEC is exceptionally rare in the head and neck region, especially in the oral cavity, the prognosis was unclear, and treatment has not been established. Fujii","doi":"10.5794/JJOMS.39.762","DOIUrl":"https://doi.org/10.5794/JJOMS.39.762","url":null,"abstract":"We have analysed 93 patients with mandibular angle fractures, with special re-ference to the treatment of wisdom teeth in the fracture line, during the past 6 years (1985-1990) at the Department of Oral Surgery, Ryukyu University Hospital.The results were as follows:1) Among 97 fracture lines in 93 cases, there were 89 wisdom teeth (91. 8%) and 31 of these were extracted.2) Erupted teeth tended to be extracted, while premature teeth tended to be preserved.3) There were few differences in prognosis of fractures between preserved cases and ex-tracted cases. It is a rare lesion of the oral cavity which has a warty or papillary appearance. It was first described by Shafer in 1971 with histopathological characteristics of verruciform growth and proliferation of xanthoma cells in the lamina propia.In Japan Kikuchi et al. reported the first case in 1977. Since then almost 40 cases have so far been published.This article described a case of Verruciform xanthoma together with its light, immunochemical and and electron microscopic fingings. After the bone is resected, the defect in the mandible is reconstructed by means of bone graft or ceramic implantation to maintain mandible form and function, but the effect on mandibular growth has not been studied in detail. So the author has studied experimentally the effects of bone graft and the implantation of 2 types of ceramics, hydroxylapatite (HA) and tricarcium phosphate (TCP), in surgically caused mandible defects growth.Ninety-six 4-week-old WKA rats were used in this study. Of these 36 rats were used for histological study and 60 were divided equally into the 6 following groups: Group 1. Bone was grafted into the defect at the mandibular body inferior border. Group 2. Bone was grafted into the defect at the mandibular ramus center. Group 3. Fibrin adhesion system (Tisseel ® kit) was filled into the bone defect created the same as in group 1. Group 4. Bone was grafted with fibrin adhesion system into the bone defect created the same as in group 1. Group 5. HA was implanted with fibrin adhesion system into the bone defect created the same as in group 1. Group 6. TCP was implanted with fibrin adhesion system into the defect created the same as in group 1. Sixty of the rats were sacrified at 15 weeks of age, decapitated, and the obtained specimens were evaluated by measurement, contact microradiography and radiography.Growth inhibition of the mandible after resection of the mandible inferior border was significantly improved by replantation of the removed bone itself to the resected site, but there was no significant effect on mandible growth when the defect at the mandibular ramus center was filled with bone. Experiments carried out following this concept are reported here. Different weights were applied to the mandible at the center of the mental region and the distribution of principal strains of bone were analyzed.It was found that force is transmitted through the mandibular bone along the bone radial structure.
