The patient had no classic exposures or ingestions of common causes of pancreatitis, and underwent an extensive workup with prolonged hospitalization until the antibody investigations revealed acute SLE.\u0000\u0000 Conclusion: This case illustrates the importance of keeping autoimmunity as a potential cause of acute pancreatitis particularly in a patient with no other classic cause of pancreatitis. Despite SLE being described as a rare and uncommon cause of pancreatitis in patients with known SLE diagnosis, this case sheds significant light on pancreatitis being a first-time presentation in a patient with no past diagnosis of SLE. It causes sensory, motor, and autonomic dysfunctions that can be bilateral, unilateral, or asymmetrical and that can change rapidly. The clinical presentation of TM is mainly represented by bilateral weakening of the lower limbs (LL), but it varies depending on the level of the spinal cord involved. Early diagnosis and treatment can benefit recovery and prevent a recurrence.\u0000\u0000 Case Report: We report the case of a 50-year-old man, an engineer by profession, who reported that in the second dose of immunization against SARS-CoV-2, he began to have a tingling sensation (dysesthesia) in the lower limbs. After four months, between the second and third doses, about five months, a new clinical picture of paresis had erupted, associated with tactile, thermal, and painful hypoesthesia.\u0000\u0000 Conclusion: Due to the significant impact of COVID-19 on health, especially about neurological impacts, disorders associated with the vaccine should always be considered when approaching patients after vaccination. Drug-induced peripheral neuropathy (DIPN) is a persistent condition, most often associated with anticonvulsants, chemotherapy, cardiovascular, psychotropic, and antimicrobial drugs, such as Metronidazole. In this article, we report the case of a patient who developed polyneuropathy secondary to the use of Metronidazole. Case Report: D.D.S.L., a 45-year-old female, previously healthy, presented with abdominal discomfort. Entamoeba histolytica was detected after investigation, and she started a 7-day cycle of 500 mg of Metronidazole three times a day for seven days, without clinical response. A new therapeutic approach was attempted, with three cycles of 2 grams of Secnidazole single dose and Tinidazole for four weeks. However, the patient presented dysesthesia in the distal third of the thighs, followed by allodynia in the four limbs and trunk. After new abdominal discomfort, three months after the first medication cycle, another pharmacological cycle was started. Neurological examination and electroneuromyography (ENM) examination suggested axonal sensorimotor polyneuropathy in all four limbs. Conclusion: Polyneuropathy can be caused by many factors, including some frequently prescribed drugs such as Metronidazole and other medications in the 5-nitroimidazole group. Therefore, although the relationship between these drugs and polyneuropathy is not fully elucidated, their neurotoxicity is indisputable, even in rare cases, but with significant variability in terms of the dose-dependent potential for this disease.

His injury was initially missed, but he was later found to have a left cuboid dislocation. He underwent an open reduction in the operating room and his midfoot was stabilized with Kirschner wires.\u0000\u0000 Conclusion: Cuboid dislocations are rare due to the soft tissue constraints around it. As in this patient, this is often a missed injury with delayed time to diagnosis and treatment. Imaging should be scrutinized, particularly with an inversion/plantarflexion force to cause injury. Hence, its growth in cultures has to be interpreted within a clinical context. This case illustrates the challenges experienced when diagnosing Candida parapsilosis necrotizing pneumonia, and the importance for considering candida pneumonia as a differential diagnosis for an immunocompetent patient. After a thorough literature review, we would like to present the first case report of C. parapsilosis causing necrotizing pneumonia in an immunocompetent patient.\u0000\u0000 Case Report: We present a case involving a middle-aged smoking male who presented with respiratory and metabolic abnormalities and was found to have necrotizing pneumonia. He was managed for severe sepsis with lactic acidosis, respiratory failure, and severe acute kidney injury (AKI), which improved with broad spectrum antibiotics and fluids. These conditions improved; however, his respiratory distress did not despite a prolonged course of antibiotics. This led to a workup for other causes of necrotizing pneumonia, after which cultures revealed the growth of C. parapsilosis. He was then started on antifungals and subsequently improved.\u0000\u0000 Conclusion: Candida necrotizing pneumonia is a rare disease for an immune-competent individual; however, chronic lung damage in the setting of a smoking history may make individuals more susceptible. This case illustrates the challenges associated when dealing with such a case, and it is the team’s hope that publishing this case will add to awareness. The pathology and its management are discussed. A rapidly growing extramedullary plasmacytoma infiltrating the cervical spine. Case Report: A 53-year-old male presented to our emergency room with a one-month history of worsening left-sided neck pain and an enlarging lateral neck mass. Evaluation with cervical imaging and laboratory analysis were performed. The patient underwent a soft tissue biopsy with subsequent spinal cord decompression and an occipital-cervical fusion. Pathology revealed the diagnosis of plasmacytoma. The patient was referred to oncology and radiation therapy for adjuvant treatment. Conclusion: This case represents the unusual presentation of a rapidly growing extramedullary plasmacytoma to the soft tissues of the neck with subsequent erosion of the base of the skull and upper cervical vertebral spine. It was treated effectively with posterior cervical decompression, arthrodesis, and adjuvant radiation and chemotherapy.

Imageries showed a 5 cm peri-ampullary duodenal mass. We couldn’t have a diagnosis by endoscopic ultrasound (EUS), so we did a laparoscopic transduodenal tumorectomy and removed the entire mass, which came back as gangliocytic paraganglioma on pathology and immunohistochemistry. Gangliocytic paraganglioma is a benign tumor that most commonly occurs in men and is localized in duodenum. Neuroendocrine tumor, ganglioneuroma, paraganglioma, and schwannoma are the differential diagnoses. Histologic diagnosis is difficult to make. Immunohistochemistry is indispensable for the diagnosis. Often than not, we cannot make diagnosis by a simple biopsy, because the tumor has a submucosal location and three types of cells need to be present for diagnosis: spindle cells, ganglion cells, and epithelial cells. That’s why en-bloc resection of the tumor is frequently needed for accurate diagnosis. Endoscopic resection or laparoscopy is used depending on the characteristics of the tumor. In our case, the 5 cm peri-ampullary tumor warranted a laparoscopic resection. Generally, there is no role for adjuvant therapy in duodenal gangliocytic paraganglioma (DGP). But sometimes, radiotherapy, surgery, somatostatin analog are used for treatment depending on the features of DGP. There is no consensus on follow-up management, but experts agree on the necessity of frequent follow-ups.\u0000\u0000 Conclusion: We believe it is important to include gangliocytic paraganglioma (GP) as a differential diagnosis in patients who present with duodenal masses, weight loss, and general state alteration.\u0000
