Because of this, cobalamin—vitamin B12—is crucial for producing myelin and neurotransmitters. Additionally, subacute combined degeneration (SCD) is one of the consequences of B12 deficiency. Symptomatology often is paraplegia, spasticity, and contractures which are typically progressive. In addition, as the disease advances, they experience a distal sensorimotor loss that ascends proximally and causes spastic quadriparesis and generalized hypoesthesia. Case Report: Male, 33 years old, with a history of a fall in December 2015, when pain and discomfort started in the left lower limb. A year ago, she evolved with paraparesis, loss of sphincter control, sensory changes in the extremities of the limbs and sensory level at T10, in addition to a report of visual disturbance in the left eye. At that time, the dosage of vitamin B12 was very low—50 pg/mL (reference above 300 pg/mL). On physical examination: extrapyramidal syndrome with hypoesthesia at the distal level. Bilateral Hoffmann’s sign, spastic paraparesis, hyperreflexia in the lower limbs, clonus in the feet, and loss of sphincter control. Sensitive level at T10. Magnetic resonance imaging (MRI) showed cervical dorsal lumbar spinal cord and cone with hypersignal focus on the lateral and posterior cords and skull hypersignal focus in the topography of the corticospinal tracts, suggestive of combined subacute degeneration. Conclusion: Vitamin B12 deficiency, in addition to other pathologies, is also found in SCD. When treated early, these alterations found in the imaging exam can be resolved without signs of atrophy. Thus, neuroimaging plays a relevant role in the diagnosis, complementing the clinic.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"49 10","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136348176","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Case Report: This report presents a case of a fully erupted mesiodens in a 10-year-old girl causing incisor displacement. The patient presented for a dental checkup with dental caries and a mesiodens visible on clinical exam. Radiographs confirmed eruption of the mesiodens. Extraction was planned to prevent further crowding. Erupted mesiodens can still cause complications like malocclusion and cysts. Extraction is the treatment of choice. Conclusion: Early diagnosis allowed for removal before additional orthodontic problems. Monitoring and restorative care are needed after extraction. This highlights the importance of prompt mesiodens diagnosis and extraction even when fully erupted.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"8 3","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135479853","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Malignant mixed Müllerian tumor of the cervix, also referred to as carcinosarcoma of the cervix is an unusual entity. Hence, there is a dearth of literature on evaluation and management of such tumors, which is also reflected in the fact that there are no specific consensus guidelines on the same. We present the case of a 35-year-old woman who was diagnosed to have malignant mixed Müllerian tumor of the cervix. Case Report: Carcinosarcoma cervix is a rare entity, with postmenopausal women being more commonly affected. This makes our case a very unique one, as our patient is a premenopausal, 35-year-old young woman who presented with abnormal uterine bleeding and was subsequently diagnosed to have carcinosarcoma of the cervix. The clinical presentation and management of the case have been discussed. Conclusion: Malignant mixed Müllerian tumor is an aggressive and rare tumor which can affect the uterus, ovaries, or the cervix. A negative biopsy may not help to rule out the diagnosis of the same. Multimodality treatment including surgical intervention, chemotherapy with paclitaxel and carboplatin, and radiotherapy could benefit this subset of patients.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"21 11","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135973001","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Case Report: This case report presents clinical and radiographic findings of gemination affecting a mandibular left lateral incisor in a 20-year-old female patient. The patient presented for a routine dental examination, which revealed an abnormal crown morphology of the mandibular left lateral incisor. There was no associated symptomology. Radiographic analysis confirmed the presence of a single root and root canal beneath the bifid tooth crown. Based on the clinical and radiographic features, a diagnosis of gemination was made. The treatment plan options were discussed with the patient, who opted not to restore the anomalous tooth. Instead, the geminated tooth will be kept under observation with regular monitoring and preventive care. Conclusion: This case provides an example of the importance of proper diagnosis of developmental dental anomalies. It also underscores the role of patient preferences in determining appropriate management of incidental findings like asymptomatic geminated teeth.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"77 ","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135814866","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This degeneration process has a complex and multifactorial etiology, culminating in the motor disability of the carriers. Case Report: The case report discusses a clinical suspicion of motor neuron disease (ALS) in the patient NVF, male, 72 years old, with a report of falls and paresis in the left distal crural third for two years. His condition evolved with the four limbs and trunk involvement, but it does not show bulbar involvement. In addition, atrophy, fasciculations, and paresis have been identified from lower motor neuron injury in all four limbs. Regarding the pyramidal pathway, only lively but symmetrical reflexes. Conclusion: In the last two decades, there have been significant advances in non-invasive imaging techniques, which allow the evaluation of brain structure, as is the case with tractography. Although this technique does not diagnose ALS, it can help early detection.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"19 2","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136262538","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The common presenting symptoms in our patients were pelvic pain, dysmenorrhea, and deep dyspareunia. The cases were successfully treated with laparoscopic excision by a multidisciplinary team of doctors. One patient showed adenomyoma co-existing with endometriosis on histopathological examination of the tissue sample. Case Series: We present 3 cases of extra uterine adenomyomas in 3 different sites, each case representing a different theory of origin and all cases managed laparoscopically with successful outcome without any complications. First case represent the implantation theory following antecedent myomectomy. Second case represents origin of adenomyoma as direct extension from the uterus with background of severe diffuse adenomyosis. Third case represents origin from Müllerian remnants in the recto-vaginal septum with no adenomyosis or obliteration of the pouch Douglas. Conclusion: We propose the theory that adenomyoma which is a form of adenomyosis should be regarded as a form of deep endometriosis involving the uterus rather than a separate entity. We believe that multidisciplinary laparoscopic treatment is the way forward for accurate diagnosis and treatment of adenomyosis in patients requiring to preserve fertility. Future research needs to focus on studying endometriosis behavior and recurrence according to the tissue host to understand the disease and tailor the management according to patient symptoms.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"35 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136113610","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Conventionally, below knee amputation was used as its only management. Currently, due to numerous advancements, wide local excision following reconstruction is the treatment of choice worldwide. Case Report: This case report provides clinical and functional outcomes of using deep circumflex iliac artery bone graft for calcaneal reconstruction. The follow-up duration in our study was two years. Normal ambulatory status, post-procedure, was achieved within nine months with a good range of motion at ankle joint both actively and passively. No donor site morbidity was seen and radiological imaging confirmed a healthy and viable graft with negative oncological margins. Conclusion: Our choice of graft proved to be a suitable option for calcaneal reconstructions as the curvature of iliac crest resembles calcaneal tuberosity. The ability of its osseous flap to naturally fit the area of defect makes it the most appropriate choice for the reconstruction of the heel of the foot.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"26 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136013234","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Its etiology remains unknown and is present in more than 500,000 people throughout Brazil. In this article, there is a brief literature review for the basis of the autistic spectrum disorder and its finding in the patient regarding her wonder about the fairytale princess. Case Report: JAD, 17 years old, is a student without comorbidities. She had been diagnosed with Autistic Spectrum Disorder, Cognitive Deficit, and Epilepsy. At 14 months, she started convulsive crises that were difficult to control, about four episodes daily. About ten months ago, an obsession with a specific fairy tale character (Snow White) began. She put herself in the character’s place and suffered from “supposedly” not receiving visits from the entire group that made up the fairy tale. She had severe cognitive delay and disjointed reasoning. She was spoken very little, did not make long sentences. It presented alterations in neuroimaging. Conclusion: After understanding the definition of the disorder, which involves a change in neurodevelopment, whether in the brain anatomy or due to a deficiency in the neuronal circuit, it is noted that even though the diagnosis is clinical, in some cases—including the reported case—the diagnosis imaging can also help to understand possible behavioral changes due to an anatomical abnormality.","PeriodicalId":13833,"journal":{"name":"International Journal of Case Reports and Images","volume":"2 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135689782","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
