This case report describes scleral lens fitting for advanced keratoconus in combination with additional glaucoma surgery. Material and Methods. The implantation of a drainage implant was followed by the bilateral fitting of scleral lenses. For contact lens fitting, it was essential to work with a scleral lens system that allows individual zone size and edge clearance in the scleral alignment zone to prevent provocation of the postoperative area. Results. Due to the individualized design of the scleral lenses, the patient was able to wear scleral lenses again postopera- tively with successful implantation of a drainage implant. A prerequisite for this was also that the surgeon considered the necessity of a later postoperative fitting with scleral lenses when choosing the surgical method. Conclusion. Scleral lenses can also be successfully fitted in cases of complex corneal shapes and surgical procedures on the eye. This requires not only a great clinical experience in fit- ting such contact lenses, but also an individual manufacturing of the contact lenses based on the contact lens parameters specified by the contact lens specialist. The case report describes the follow up fitting with scleral contact lenses for visual rehabilitation in advanced keratoconus and the influence of the scleral fitting characteristics on the daily wearing time as well as the wearing comfort of the contact lenses. Material and Methods. Scleral contact lenses were fitted using Scheimpflug images and analysis of optical coherence tomography cross-sectional images. Based on this data an optimization of the contact lens design could be performed. Results. In the present case, a significant improvement in visual acuity, daily wearing time and wearing comfort was achieved due to scleral lens re-fitting with individual scleral lens sagittal height calculation and custom fitting according to corneo-scleral profile measurement data. Conclusion. Scleral lenses can be a good alternative to other contact lens treatments to achieve visual rehabilitation in complex corneal situations. Analysis of the corneo-scleral profile by Scheimpflug technique and accurate evaluation of the sagittal height are of great benefit, in addition to the assessment of the fitting and landing zone of scleral lenses by anterior segment OCT. This results in significant time savings for the fitting and leads to a more physiologically compatible contact lens treatment result. This review paper aims to summarize the outcomes of the different studies investigating the correlation between corneal and scleral profiles, outline the relationship between corneal and scleral curvatures and asymmetry, and describe the correlation between corneal astigmatism and scleral toricity in regular and irregular corneas. Material and Methods. Literature was reviewed from PubMed. A total of 48 articles were specifically selected for the current study. Results. The scleral and corneal radii are directly correlated; the greater the corneal radius, the larger the scleral radius. The cornea and sclera become gradually flatter and asym- metric as the distance from the corneal optical line increases. Similar to the central cornea, the limbal area is more likely to be spherical. The horizontal asymmetry between the nasal and temporal areas is greater than the vertical asymmetry, with the nasal region of the ocular surface being the flattest. In eyes with keratoconus, the sclera is steeper, more asym- metric, and irregular. Cone decentration predicts asymmetric sclera. Corneal and scleral toricity are more likely correlated when corneal astigmatism is greater than 2.00 D. Conclusion. Some correlations were found between corneal and scleral topography; however, performing both corneal and scleral topography are necessary when fitting contact lenses. Further investigation is needed with larger groups matched in age, refractive error, and biometry and comparing smaller or steepest eyes (i.e., Asiatic population) with larger or flatter eyes. It would also be interesting to compare such correlations in primary and secondary ectasia, investigating whether the scleral shape changes in secondary ectasia. An optimal lens fit is necessary to ensure patient comfort and visual quality. However, it is not straightforward to estimate the appropriate scleral lens parameters for a particular patient. This review paper aims to summarize the current state of knowledge in predictors of scleral lens parameters based on corneo-scleral shape. Material and Methods. Literature was reviewed from PubMed. A total of 33 articles were specifically selected for the current study. Results. Even though not all available corneo-scleral meas- urements may be helpful in the fitting experience, the re- fractive state of the cornea, corneal flattest and steepest keratometry, scleral toricity, and axial length have proven to influence the scleral lens parameters and, consequently, the quality of scleral lens fit. Conclusion. The usefulness of corneal, scleral, and corneo- scleral measures is reviewed and critically evaluated. A special effort was made to highlight the clinical implications of the findings. Non-syndromic hereditary ocular diseases can lead to severe functional limitations of vision at all ages. Therefore, molecular genetic research into the causes of hereditary ocular diseases is of paramount importance. Due to the increasingly optimised pathophysiologically adapted treatments and gene therapy options, a rapid determination of the genetic cause of hereditary ocular diseases is neces- sary, also to increase the chances of treatment at early stages of the disease. In this context, the authors retrospectively analysed the database of the “rare diseases” consultation hour of the PraxisKlinik Augenärzte Halle, which has been in existence since 2016, for patients with a suspected hereditary ophthalmological disease after molecular genetic diagnostics, counselling and therapy options. Material and Methods. 367 index patients were treated dur- ing the period under consideration. In 221 (60.2 %) of them, the molecular genetic cause of the underlying disease could be found. In addition to a large group of patients with cor- neal dystrophies (304 patients, detection rate 61.2 %), we treated 23 index patients with hereditary retinal diseases (detection rate 69.6 %), 28 index patients with complex mal- formations (detection rate 50 %) and 15 index patients with optic atrophies (detection rate 33.4 %). In addition to the ophthalmological-clinical examination and diagnostics, ge- nealogical analysis and molecular genetic testing were always performed. Results. On the basis of 3 case studies with a molecularly ge- netically proven hereditary retinal disease (X-linked recessive inherited juvenile retinoschisis, hemizygous missense muta- tion in the RS1 gene; enhanced S-Cone syndrome, compound heterozygous mutation in the RS2E3 gene; oculocutaneous albinism, homozygous mutation in the TYR gene), the inher- itance, the clinical picture, the course and the therapeutic and rehabilitative options were explained. Conclusion. Ophthalmogenetic counselling and diagnostics in cases of suspected hereditary eye diseases are crucial for a prompt and a sufficient possible therapy and optical reha- bilitation in affected families. Thorough history analysis and diagnostics can optimise the detection rate and provide rapid and sufficient patient care. The aim of this clinical article is to describe the surgical treatment of tear-related retinal detachment in the context of the individual retinal findings, the lens status and in consideration of the surgical and postoperative risks, taking into account the specific situation of each patient. Material and Methods. Rhegmatogenous retinal detachment is the result of a retinal tear caused by traction of the vitre- ous on the retina. Liquefied vitreous enters the subretinal space and separates the sensory retina from the pigment epithelium-Bruch’s - membrane complex. As a result, the retina loses contact with the choroid supplying the outer retinal cell layers. The damage caused by the reduced supply, especially to the foveal retina, determines the urgency of surgical treatment. Two very different surgical procedures are available for thera- py: buckling surgery and pars plana vitrectomy. Both surgical methods have different indications and risks and are ex- plained on the basis of our own clinical experience, together with other publications on the same subject, also in their historical context. Results. The treatment goal of rhegmatogenous retinal de- tachment is to bring the retina closer to the retinal pigment epithelium, which ‘pumps out’ the fluid from the subretinal space, and reestablishing the retinas contact to the choroi- dal supply. The entry of fluid into the subretinal space must be permanently prevented by induced scarring. Whether a buckling surgery or a pars plana vitrectomy is indicated depends on the respective retinal findings. The greatest risk after any surgery for a tear-induced retinal detachment is the development of a proliferative vitreoretinopathy and a resulting renewed retinal detachment. Conclusion. Early detection of retinal detachment by the symptoms flashes, floaters, shadows and early binocular oph- thalmoscopy is important to prevent progression of retinal detachment into the fovea and to organize vitreoretinal surgery as soon as possible. The visual prognosis of retinal de- tachments with foveal or macular involvement has improved significantly in the last decades. This case report describes the optometric manage- ment of a peripheral retinal tear that was discovered during an optometric eye examination. It is intended to sensitize optometrists and ophthalmic opticians to the topic of retinal tears and retinal detachment. Material and Methods. During an optometric eye examina- tion, the fundus was examined using an ultra-widefield laser scanning ophthalmoscope. A retinal tear was detected in the fundus periphery. Results. After the comprehensive optometric examination, the patient was informed about the finding of a peripheral retinal tear and its consequences and referred to an eye clinic. At the eye clinic, the diagnosis was confirmed. Treatment consisted of preventive laser retinopexy to reduce the risk of subsequent tear extension and retinal detachment. Conclusion. The retinal tear presented in this case report was detected as an incidental finding. The patient was referred to an eye clinic for specialist diagnosis and preventive treat- ment. The case shows that a comprehensive optometric eye examination not only enables the management of relevant refractive errors, but can also make a contribution in terms of preventive health care. Optometric involvement in eye care has the potential to lead to earlier diagnosis, allowing treatment strategies to be implemented at an earlier stage. Together, earlier diagnosis and earlier intervention can lead to better long-term health outcomes. To compare the intraoperative OCT (optical coherence tomographer) of the femtosecond laser LenSx (Alcon Pharma GmbH, Freiburg) with a Scheimpflug-tomographer and an anterior segment OCT and thus to investigate a possible influence on the anterior segment of the eye when docking the patient’s interface. Material and Methods. A prospective cross-sectional study was conducted with 57 subjects who underwent a planned femtosecond laser-assisted cataract surgery or refractive lens exchange. The eye to be operated on was measured prior to the operation in mydriasis with the Visante OCT (Carl Zeiss Meditec AG, Jena) and the Pentacam AXL (Oculus Optikgeräte GmbH, Wetzlar). During the surgery, images of the anterior segment of the eye were taken by the integrated intraoperative OCT of the laser, which were evaluated afterwards and compared to the values of the other devices. The parameters examined were the internal anterior chamber depth (ACD), nasal and temporal chamber angles, central corneal thickness (CCT) and central lens thickness (LT). Results. The comparison of the intraoperative OCT and the two other devices showed clinically relevant differences in all parameters. The intraoperative OCT showed a 9.19 ± 27.55 μm larger CCT, 0.96 mm (median difference) larger LT, 0.05 mm smaller ACD than the Pentacam AXL and a 21.36 ± 25.50 μm larger CCT, 0.10 mm (median difference) smaller ACD than the Visante OCT. Conclusion. The comparison of the intraoperative OCT with the Visante OCT and the Pentacam AXL shows clinically relevant differences. Changes in the anterior segment of the eye may be due to errors in measurement or docking of the patient’s interface. The purpose of this study is to develop a new digital colour vision test. Material and Methods. Based on the principle of metamerism, a digital testing strategy was developed for efficient measurement of colour vision. Twentyfive subjects participated in the study, 21 of whom had normal colour vision and four of whom had a congenital colour vision disorder. Differences in colour vision were examined by monocular presentation of halfquadrants of different hues and degrees of saturation, and the algorithm calculated the colour vision defects that occurred based on the subjects’ responses. The following colour vision values were assigned from the data: a colour scale range of 2.76 – 7.18 for normal colour vision and greater than 7.18 for colour sense disorders. A second new colour scale assessed the type of colour vision disturbance: in the range of 0 – 0.3 were values for a deuteranomaly, in the range of 0.6 – 1.0 were values for a protanomaly, and a value of 1.0 corresponded to a tritanomaly. Results. The difference in colour vision between subjects with normal colour vision and those with a colour vision defect was confirmed (p < 0.001) by measuring the type and the extent of the colour vision disorder was determined. In the group of subjects with normal colour perception, a mean colour scale value of 3.41 ± 0.52 was determined. The extent of colour sense disturbance for the subjects with colour vision impairment was values of 7.18 – 14.33 according to the colour scale, indicating greater variability. Conclusion. The developed algorithm provided meaningful results regarding the colour perception of the test persons. It was possible to differentiate between normal colour vision and colour vision disorder could be shown. Furthermore, the variation between subjects with normal colour vision could be evaluated. The prevalence of myopia is increasing worldwide and its long-term effects can have major impacts on eye health. Paediatric onset of myopia leads to a higher risk for developing high levels of myopia in adulthood. Slowing down or stopping myopia progression is an important task for the future. Material and Methods. Based on a literature review, the latest developments in the field of myopia control for children were summarized. The current study situation for orthokeratology, soft contact lenses, atropine therapy and spectacle lenses for the management of myopia is discussed in detail. Results. Results on the safety and effectiveness of orthokeratology and modified soft contact lenses are exceedingly consistent. Soft multifocal contact lenses, as well as newer spectacle lenses, recently received regulatory approval for myopia control. Studies on the administration of atropine recommend a dosage of 0.01 - 0.05 %. All methods slow down myopia progression and decrease axial length growth of the eye. Side effects, adverse events or discontinuation of treatment only occur in the rarest of cases. Orthokeratology and atropine continue to deliver the best results. Conclusion. From today’s perspective, carrying out myopia control can be recommended with great certainty. The choice of method depends on the individual requirements of each patient, with all methods offering success in reducing myopia progression. In the future, even more advanced contact lens geometries, more precise atropine dosages or improved optics of spectacle lenses will further increase the success of the treatment. Keywords.
