Khah","doi":"10.21761/jms.v8i02.01","DOIUrl":"https://doi.org/10.21761/jms.v8i02.01","url":null,"abstract":"Background: Acne vulgaris is a common disease in teenagers.It is a chronic inflammatory condition of the pilosebaceous units.Objective: To see the effect of different factors like age, skin type,diet, cosmetics, family history and pornographic material on acne.Methods: This case-control study was conducted in theDermatology Department of Nangarhar University TeachingHospital, Nangarhar from August 2021 to March 2021. Thestudy consisted on a sample of 120 subjects including 60 casesof acne and 60 controls. The data was entered in a structuredclose-ended questionnaire. Univariate and multivariate logisticregression was used to see the effect of different factors on acne.Results: Age was below 20 years in 68 percent of patients and38 percent of controls. Amongst patients 12% and amongstcontrols 7% were married. Logistic regression analysis showedthat age (p = 0.001), odds ratio (0.288), Confidence interval (0.136– 0.612) siblings history (p = 0.007), odds ratio (2.786), confidenceinterval (1.329–5.841), skin type (p = 0.000), dry skin (p = 0.173),odds ratio (0.449), confidence interval (0.142-1.422), oily skin (p= 0.002), odds ratio (3.925), confidence interval (1.684 9.146),cola drinks (p=0.048), daily drinks (p = 0.023), odds ratio (2.975),confidence interval (1.159-7.636), cosmetic used (p=0.004), oddsratio (4.219), confidence interval (1.577–11.285) and frequency ofapplication of cosmetics (p=0.010), odds ratio (3.509), confidenceinterval (1.347–9.1)had significant effect on acne.Conclusion: Acne is a more common disease in teenagers;age greater than twenty plays a protective role against acne.Carbonated drinks, positive sibling history, cosmetic treatmentand frequent use of cosmetics have a significant relationship withacne. Central venous catheterizations(landmark-based or ultrasound-guided) are associated withmechanical complications. This study aimed to assess, analyzeand compare the complication rate between two commonlyemployed catheterization routes: Internal jugular vein andsubclavian vein.Methodology: A prospective, observational study comparingtwo landmark-based central venous catheterization techniqueswas planned in a tertiary care teaching institute. 100 patientsrequiring central venous catheterizations were enrolledalternately into the IJV and Subclavian groups. Standardlandmark-based insertion techniques were employed understrict asepsis. All patients were observed for mechanicalcomplications in the first 24 hours following catheterization.Statistical analysis was done using the statistical packageSPSS 20.0. Data were expressed as either mean and standarddeviation or numbers and percentages. Categorical variableswere analyzed using proportions and percentages. Associationbetween categorical variables was established by Chi-squareand odds ratio (OR) with 95% confidence intervals (CI).Results: There were 22 complications observed. Incidence ofcomplications was higher in the IJV group with 17 complicationsversus 5 in Subclavian group. There were 4 major complicationsand 18 minor complications between the two groups. Majorcomplications comprised of hematoma formation (7/22) andarterial puncture (8/22). Incidence of complication was alsoanalyzed with regards to independent variables.Conclusion: Mechanical complications following centralvenous catheterizations was found in both groups studied.Internal jugular venous group had a higher mechanicalcomplication rate than the subclavian group. In the present study, we evaluated theimmunoexpression of Bcl-2 oncogene in oral squamouscell carcinoma (OSCC) tissue specimen and correlated itclinicohistopathologically.Material and Method: A total of 42 OSCC tissue specimenwere subjected to evaluation for immunohistochemicalexpression of Bcl-2 oncogene. IHC expression was scored as“-” (<5% positive cells), + (5–24% positive cells), ++ (25–50%positive cells) and +++ (>50% positive cells). Pathological Tstage, nodal status, histopathological grade, lymphovascularinvasion (LVI), perineural invasion (PNI) and mitotic index (MI)were noted. Data was analysed using SPSS 21 software.Chi-square, ANOVA and Kruskall-Wallis tests were used tocompare the data.Results: Mean age of patients was 49.05 ± 13.28 years,majority of them were males (83.3%). There was a dominanceof well/moderately differentiated (81%) cases. Lymphovascularand perineural invasion were seen in 16.7% and 26.2% cases.Majority had mitotic index 2 (66.7%), pT stage 3/4 (73.8%)and no nodal involvement (54.8%). Bcl-2 expression wasseen in 34 (81%) specimen. It was +, ++ and +++ in 6 (14.3%),15 (35.7%) and 13 (31.0%) cases. A significant correlationof Bcl-2 expression was seen with higher histopathologicalgrades, mitotic index and pT stage. There was no correlationof Bcl-2 expression with age, sex, LVI, PNI, MI and pNstatus.Conclusion: There was a high IHC expression of Bcl-2 oncogenewhich was significantly associated with histopathological gradeand pT stage. Inthis study, 70 patients were studied for 1.5 yrs from 1st ofNovember 2019 to 13th of April 2021. All these patients withintraoperative complications were evaluated preoperatively andpost-operatively for vision and anterior segment anatomicaloutcomes.Results: In this study, the commonest intraoperativecomplication was Injury to Iris/ Iridodialysis (52.9%). Basedon pre-operative vision, the patients had 5/60 and 6/36 visionwith 30.0%, each followed by 6/24 (14.3%) and 3/60 (12.9%).Corneal hazy/edema was reduced to 10.0% at one-monthfollow-up from 52.9% on post-operative day one. Also, DMfolds were reduced to 11.4% from 75.7% at day one.Conclusions: In this study, visual outcome of the cataractsurgery was much better, and there is a significant progressionin the visual acuity. However, concerns regarding neurocognitivetoxicity after radiotherapy are being raised. Newer radiotherapytechniques can deliver radiotherapy with better precisionplanning and delivery. Effective hippocampal sparing is possiblewith IMRT which governs the neurocognitive functions. Thepresent study is done to compare whether hippocampal sparingis possible in brain tumors by 3D Conformal Radiotherapyand Intensity Modulated Radiotherapy and their effects onneurocognitive functions.Materials and Methods: Twenty-two patients with brain cancerwere recruited from November 2019 to April 2021. Patients weretreated with 3D-CRT technique and alternate IMRT plans weregenerated. Dosimetric parameters of PTV, organs at risk alongwith hippocampus were evaluated and compared for 3DCRTand IMRT plans. Neurocognitive functions were evaluatedusing MMSE score.Result: In study group, there were 16 males and 6 females withmedian age 45 years. Brain tumors were commonly located infrontal lobe (36%) followed by parieto-occipital lobe (18%) andfronto-temporo-parietal lobe (13.63%). PTV parameters werebetter for IMRT and statistically significant. The OARs did notshow significant difference except in both lens though they arewithin tolerance limits. There is no significant difference in thedosimetric parameters of hippocampus in 3D-CRT and IMRTplans. The p-value for Dmax is 0.79, Dmean is 0.26, Dmin is0.18 between hippocampal sparing radiotherapy plans.Conclusion: Anatomic location of tumor plays a key role indeciding hippocampal sparing. Patients whose NCFs showedimprovement on subsequent visits, highlights the fact thatprimary tumor control is a key factor in deciding decline orimprovement in NCFs. Ithas also proved to be valuable for the lymphnode (LN) stagingof lung cancer,as well as for the primary diagnosis of unclearmediastinal lesionsMaterial and Methods: EBUS-FNAs from mediastinal lymphnodes were retrospectively reviewed from January 2020 toJanuary 2022 and their findings were correlated with thehistology.Results: A total of 60 cases were obtained. Out of which, 3cases were non diagnostic, and 57 cases were diagnostic.Three cases were reactive hyperplasia, seventeen weregranulomatous lymphadenitis and thirty-seven cases werepositive for malignant cells. Goyal","doi":"10.21761/jms.v7i01.2","DOIUrl":"https://doi.org/10.21761/jms.v7i01.2","url":null,"abstract":"Introduction: Cesarean delivery is a major obstetrical surgicalprocedure aiming to save the lives of mothers and fetuses.The incidence of cesarean section varies between 10.0% and25.0% in most developed countries.1Material and Methods: 80 cases for elective cesarean sectionadmitted in ward/labour room of Department of Obstt andGynae, SRMSIMS were studied to determine the incidenceof infectious morbidity, i.e., UTI, SSI, puerperal sepsis andendometritis on different occasions that are immediatepostoperatively, 48 hours post-operative, on discharge (4–5thday) and upto 6 weeks post-partum.Preoperative routine blood and urine examination were doneto recruit low-risk patients.Preoperative part preparation with betadine was done the nightbefore surgery, followed by intraurethral catheterization with 3swabs under all aseptic precautions.Results: There is a relationship between the timing ofadministration of prophylactic antibiotics and infectiousmorbidity. On comparing demographic factors concerninginfectious morbidity, a statistically significant p-value wasobserved with the women's age, parity and BMI.Conclusion: Single dose of prophylactic antibiotics shouldbe effective if given within 30 minutes of skin incision in anelective cesarean section, especially in women with mean age26.2 ± 4.2 years, low parity and with mean BMI of 24.9±2.4(kg/m2). The neonatal outcome is not much affected on givingprophylactic antibiotics before cord clamping. The most common causes appear to be respiratory tract infections. Overuse of antibiotics is common and accelerates the development of drug resistance\u0000 and hospital acquired infections. In some recent studies, both C-reactive protein as well as Procalcitonin levels have been shown to be useful in differentiating bacterial etiology of exacerbations and thus helping in guiding the treatment as well as\u0000 in prediction of outcome. Evaluate sensitivity and specificity of C-reactive protein and Procalcitonin as a marker of bacterial infection in patients with acute exacerbation of COPD. MATERIAL AND METHODS: The present hospital-based observational\u0000 study was carried out at Department of Pulmonary Medicine, SRMSIMS, Bareilly. 50 patients from patients of COPD with acute exacerbation attending/admitted to Pulmonary OPD/IPD were included in the study excluding those below 40yrs old or presenting\u0000 with acute breathlessness due to comorbid condition. Demographic information, relevant clinical data and lab investigations were recorded from all patients including C-reactive protein and Procalcitonin on admission following which the patients were\u0000 started antibiotics as per guidelines. Reassessment of S. Procalcitonin and C-reactive protein was done on 3rd and 7th day of hospitalization. ROC Curve was applied to compare sensitivity and specificity. RESULTS: Sputum culture was found positive in\u0000 27 (54.0%) patients. At all the three intervals, CRP levels had ROC area under curve (ROCAUC) values above 0.70. Area under curve value was maximum at day 3. For Procalcitonin, the area under curve values were >0.8 at day 1 and day 3 but on day 7\u0000 this value was only 0.624. On evaluating the correlation between S. The DECAF and BAP 65 score was derived for accurate prediction of mortality and risk stratification to inform patient\u0000 care. We aimed to validate these scores, and to compare them. Comparison of DECAF, BAP-65 scores in predicting in hospital mortality in AECOPD MATERIAL AND METHODS: 106 patients of AECOPD, admitted during 6 months period were scored at admission\u0000 using all 2 scores and their ability to predict in-hospital mortality was analysed. RESULTS: On receiver-operator characteristic curve analysis, the area under curve for prediction of in-hospital mortality was 0.791and 0.885 respectively for DECAF\u0000 and BAP-65 scores respectively. Thus, among the two scoring systems BAP-65 had maximum area under curve while DECAF had minimum area under curve. Sensitivity and specificity values for prediction of in-hospital mortality were 83.3% and 54.3% for\u0000 DECAF and 83.3% and 84.0% for BAP-65. MATERIAL AND METHODS: 100 term antenatal\u0000 patients, 50 with occipito-posterior and 50 with occipito-anterior position were included in the study admitted in labour room from December 2015 to March 2017. After recuirtment of patients, detailed history, examination: general physical, per\u0000 abdominal and per vaginal was done. All patients underwent ultrasonography to confirm the fetal position at the onset of labour. They were then followed up until birth to determine outcome. The primary outcome measures including mode of delivery\u0000 which was categorized into vaginal delivery, instrumental delivery and Cesarean section, duration of labour and neonatal outcome. RESULTS: Of the total 100 cases, the proportion of vaginal delivery was more in occipito-anterior group (74%) in\u0000 comparison with occipito-posterior group (42%). 40% patients of occipito-posterior group landed into cesarean delivery against 18% patients of occipito-anterior group. Eight percent patients had face to pubis delivery. The mean duration of labour is\u0000 prolonged in study group i.e. 473.2±1.84 minutes in first stage, 29.4±7.67 minutes in second stage and 5.70±1.75 minutes in third stage versus i.e. 376.8±1.26 minutes in first stage, 24.79±9.77 minutes in second stage and 5.20±3.22 minutes in third\u0000 stage of labour of control group. The neonatal outcome was analysed by Apgar score at 1,5 minutes of delivery, presence of caput succedaneum and moulding and found to be comparable in both the groups. CONCLUSION: We found that malposition group\u0000 showed prolongation of labour in comparison to occipito-anterior position. We found high rate of cesarean delivery in study group because of preference for cesarean over instrumental delivery. Neonatal outcome were comparable in both the groups.
