Methods : Two patients were treated mainly with catgut-embedding acupuncture therapy and partly with other Korean medicine modalities. Catgut-embedding acupuncture therapy was applied once a week for three weeks, and follow-up observation was conducted by telephone survey two months after the first treatment. Results : In these two cases, the pain numerical rating scale(NRS) gradually decreased from 8 to 1, and the pain relief as well as the improvement in ambulatory function was maintained at two-month follow-up. Jeong, Seung-Ho Lee, Insoo Jang","doi":"10.13045/ACUPUNCT.2013051","DOIUrl":"https://doi.org/10.13045/ACUPUNCT.2013051","url":null,"abstract":"Objectives : The purpose is to review large-scale clinical researches, the acupuncture randomized trials(ART) and the acupuncture in routine care(ARC), concerning efficacy and effectiveness of acupuncture performed in Germany. Methods and Results : We have reviewed the studies that were searched by keyword ‘acupuncture’ as the title and ‘Witt CM’ as one of the main authors in PubMed. Sixteen clinical reports focused on the ART and ARC studies were analyzed among all thirty six were searched. Six studies were related with the ART and eight were focused on the ARC, and other two were on both. Finally, across all researches except osteoarthritis of knee trials in the ART, compared the acupuncture group with the control were significantly improved. On the other hand, ARC showed that additional acupuncture plus routine care was more effective than routine care alone and the response to treatment of the non-randomized group did not differ the one of the randomized acupuncture group. Conclusions : The results may suggest that acupuncture treatment has obvious clinical effectiveness. Methods : The subjects for this study were 21 patients who had peripheral facial nerve palsy. We analyzed about the correlation between the results of SEMG and facial nerve assessment scales that were carried out at the same time. The assessment scales used in this study were House-Brackmann scale, Yanagihara grading system, Sunnybrook facial grading system. Results : There are considerable correlation between the results of SEMG and the results of three kinds of assessment scales for facial nerve palsy. Conclusions : SEMG may be an objective examination for facial nerve palsy. Some research aimed at development of SEMG examination guideline for facial nerve palsy will be needed after this. {"title":"Retrospective Study on Factors Influencing Facial Nerve Damage of Acute Peripheral Facial Palsy Patients: by Electromyography","authors":"P. Methods : From October 2009 to September 2013, the total number of 581 patients of Bell’s palsy and Ramsay-Hunt syndrome visited Facial Palsy Center in Kyung Hee University Hospital at Gangdong for Traditional Korean and Western combined medical treatment. Of these, 453 patients of peripheral facial nerve palsy were selected for the research. After reviewing the medical records that have details of age, gender, diagnosis(Bell’s palsy and Ramsay-Hunt syndrome), onset, underlying diseases(DM, HTN), and HbAlc value, the analysis on the influence factors on the level of facial nerve damage was drew out. Results : The axonal loss rate of oris branch and nasal branch were significantly higher than the axonal loss rate of frontal branch and oculi branch. In addition, the frequency of becoming a major damaged branch was also high in the oris branch nasal branch. The factors by month, weather, smoking, and alcohol did not influence EMG axonal loss rate. Male rather than female and patient with Rasmay-Hut syndrome rather than Bell’s palsy had a higher axonal loss rate in all branches. Of those, front of branch of male was remarkably higher than female. Patient with DM as P/H had high axonal loss rate in all branches. Patient with HTN as P/H had high axonal loss rate in all branches except for oris branches. Patients with DM and HTN group had significantly higher value from the average of axonal loss rate than patients who are only with HTN and without DM/HTN. DM alone group had significantly higher value than patients who are without DM/HTN. However, HTN alone was not significantly high. By analysing HbAlc of the patients who were hospitalised regardless DM, axonal loss rate was high in the order of DM group, preDM group, normal group. Nevertheless, only DM group showed higher axonal rate statistically than normal group. Considering DM and HbA1c value, the patients can be divided into 4 different groups of hkDM, lkDM, hfDM and nDM. By analysing those groups, the average damaged value of the groups with diagnosis followed by treatment(lkDM, hkDM) were higher than the average rate of hfDM and statistically higher than the rate of the nDM. Conclusions : The influential factors of increasing the level of EMG damage are male(only for the frontal branch), age above sixties, HTN, DM, and HbAlc value above 6.5. Besides, the negligible factors are month, season, diagnosis, alcohol, and smoking. Methods : Sixty-three subjects got acupuncture at five acupoints which represent five different body parts ; head(GV20), abdomen(ST25), back(BL24), upper extremity(LU9), lower extremity(GB40). All subjests were asked to complete questionnaire rating the intensity of 13 kinds of acupuncture sensation(acupuncture sensation scale, ASS). We compared the subjective acupuncture sensation between the body parts. Results : Intensity of acupuncture sensation of GV20 was significantly lower than LU9(p=0.001) and GB40(p=0.000). Sum of acupuncture sensation of GV20 was also significantly lower than BL24(p=0.011), LU9(p=0.004) and GB40(p=0.033). Among the 13 types of acupuncture sensation scale, tingling and aching were well sensed at GV20 and ST25, aching, tingling and sharp pain were well sensed at LU9, GB40, dull pain, deep pressure and heaviness were well sensed at BL24. Conclusions : Head showed significantly lower intensity of acupuncture sensation than upper extremity and lower extremity.
