E-Mail: varlik.erel@gmail.com ORCID ID: 0000-0002-0539-661X Abstract Aim: Sepsis is a clinical problem that still has high mortality and morbidity despite advances in supportive care in intensive care. The study’s aim was to investigate the protective effect of Bacillus clausii, an important probiotic with a strengthening effect on the gastrointestinal barrier, on septicemia. Material and Method: Twenty-four rats were divided into three randomized and double-blind groups, as Group 1 (control group, n=8), Group 2 (sepsis group, n=8), and Group 3 (sepsis+probiotic group, n=8). Lipopolysaccharide extracted from serotypes (0111:B4) of Escherichia coli (E.coli ) was injected intraperitoneally to all group for sepsis model at a dose of 15 mg/kg and rats were sacrificed at 48 hours. Serum oxidant and antioxidant parameters were measured. Results: The C-Reactive Protein (CRP) value of Group 1 was significantly lower than Group 2 [CI:(-44.16)-(-28.52), p=0.000], whereas the CRP values of Group 3 were significantly lower than Group 2 [CI:(7.4)-(34.7), p=0.005]. MDA values were found to be significant between Groups 1 and 2, CI:(-0.3)-(-0.7), p=0.045. The thiol values of Group 2 were significantly lower than Group 1 [CI:(9)-(366), p=0.041], whereas the thiol (SH) values of Group 3 were significantly higher than those of Group 2 [CI:(-659)-(-98), p=0.016]. Discussion: B. clausii is one of a group of probiotics that is inexpensive and effective. Sepsis is still a high-risk clinical problem. E-Mail: varlik.erel@gmail.com ORCID ID: 0000-0002-0539-661X Abstract Aims: The extension of spinal anesthesia by extradural injection has been identified as a modification of the combined spinal-epidural anesthesia. Epidural volume extension (EVE) is a rescue strategy that can raise the level of insufficient post-spinal sensory block. Material and Method: After approval of Adnan Menderes University Ethics Committee (Decision 2016/834/37), the data of 455 patients who had undergone insufficient simultaneous combined spinal epidural without catheter between 2010 and 2016 were retrospectively analyzed in terms of ASA scores; demographic and hemodynamic data; surgery type; preoperative and postoperative sensory and motor block levels; operation onset times after anesthesia; need for peroperative additional anesthesia, vasopressor (ephedrine) and atropine; and postoperative pain onset times. Results: Of the 455 patients, there were two groups. In the first group, there were 238 patients who had undergone cesarean section and in the second group there were 217 patients who had undergone surgery for inguinal hernia. There was no mortality and morbidity in any group. There was a statistically significant decrease in heart rate, systolic, diastolic, and mean arterial pressures during the peroperative period in both groups (p<0.05). Discussion: The simultaneous combined spinal-epidural technique (sCSEA) without catheter may be considered as an alternative to conventional methods for appropriate surgeries. It can be safely used with local anesthetic combinations instead of saline for EVE. {"title":"Changes in renal functions of sickle cell anemia patients during and after painful crisis","authors":"C. Huzmeli","doi":"10.4328/aemed.119","DOIUrl":"https://doi.org/10.4328/aemed.119","url":null,"abstract":"Aim: We aimed to investigate glomerular filtration rate (GFR) observed during the painful crisis and 15 days after the painful crisis of sickle cell anemia (SCA) patients. Material and Methods: A total of 38 patients with SCA were included in the study. 25 of these patients were male, and 13 were female. The amount of creatinine and albumin in urine in 24 hours and serum creatinine level were determined during and after 15 days from SCA painful crisis. In the patients, the creatinine clearance was measured during and after 15 days from SCA painful crisis. Results: In SCA patients, the average creatinine clearance during the painful crisis was determined as 136.0 ± 80.0 mL / min, after 15 days from the painful crisis, the average creatinine clearance was determined as 124,8 ± 53,4 ml/min (44,9-238,3). In SCA patients, there was no significant difference between the creatinine clearance calculated during the painful crisis and 15 days after the painful crisis (p>0,05). In SCA patients, during the crisis, the amount of albuminuria was 199.5 ± 408.3 mg/day (2.6-1943) while it was determined 167.0 ± 399.7 mg/day (5.7-1944.9) after 15 days of the crisis. There was no statistically significant difference between albuminuria during and 15 days after the crisis. Discussion: In SCA patients, there was no significant difference between the creatinine clearances calculated during and 15 days after the SCA painful crisis. {"title":"Remodeling capacity of femur diaphysis fracture with non-surgical treatment in preschool children","authors":"S. Çamur","doi":"10.4328/aemed.106","DOIUrl":"https://doi.org/10.4328/aemed.106","url":null,"abstract":"Aim: We aimed to evaluate the healing - remodeling capacities of femoral diaphysis fractures treated with non-surgical methods in preschool children. Material and Methods: Thirty-six patients aged between 0-6 years with closed femoral diaphysis fractures who were treated with non-surgical methods were evaluated. Two-way graphics of the patients were taken after plastering. Limb lengths were recorded. Patients were divided into 2 groups according to their follow-up periods: 0-60 months (group A) and over 60 months (group B). The two-sided bilateral femoral graphics taken during their final examinations were used as the basis for radiographic evaluation. Results: The mean age of the patients was 4.2±1.94 years. The mean amount of shortness of the patients measured when the plaster was removed was 1.61±0.84 cm and it was measured at a mean of 0.8±0.74 cm at the last follow-up. Angulation on the coronal and sagittal planes after treatment was significant (p<0.01). The decrease in shortness is statistically significant (p<0.01). Angulation improvement on the coronal plane in patients in group B was statistically significantly higher than in group A (p<0.05). There was no statistically significant difference between groups A and B when the amounts of angular improvement on the sagittal plane were compared (p>0.05).
