Herniation of the urinary bladder into the inguinal canal and scrotum can cause urinary retention and hydronephrosis, bladder necrosis, and renal dysfunction. This study presents a case that underwent emergency surgery for an incarcerated inguinal hernia. The hernia sac included the urinary bladder in addition to bowel segments. An attempt to save the ischemic bladder wall during partial bowel resection failed, and the patient developed a vesicocutaneous fistula. The fistula was repaired, and the ischemic bladder wall was resected. During the repair of an inguinal hernia, general surgeons and urologists must be aware of this rare condition and work together in terms of patient management. Our study's primary aim is to determine the COVID-19 vaccination rate among healthcare workers in a tertiary hospital in Mogadishu, Somalia.\u0000 Materials and Methods: This questionnaire-based cross-sectional was conducted between September 1st and September 20th, 2021.\u0000Results: A total of 210 healthcare workers were included. Among 210 healthcare workers enrolled in the study, 56.2% (n=118) were male. Most (53.8%, n=113) participants were aged between 18 and 27. Most (48.6%, n=102) of all participants were physicians. The COVID-19 vaccination rate of the healthcare workers in Somalia Turkiye Training and Research Hospital was 20% (n=42). Males had a significantly higher vaccination rate than females (26.2 vs. 12%, p<0,05). The most common reasons for not getting vaccinated were fear of potential complications and believing that vaccination would not be protective against the disease.\u0000Conclusions: Most healthcare workers were hesitant to be vaccinated despite the availability of the vaccine. This approach exposes physicians and allied health workers to a significant risk of COVID-19. It mainly occupies the lateral ventricle and has relatively characteristic imaging features that aid in its diagnosis. This tumor causes obstructive hydrocephalus and manifests with signs of increased intracranial pressure. Herein, we present a case of a 53-year-old female diabetic patient who presented to our neurology clinic with an eight-month history of diffuse progressive headaches. Her headache was associated with nausea but not vomiting. There was no motor or sensory deficits or paresthesia. Likewise, the examination revealed no diplopia, ophthalmoparesis, cranial nerve impairments, or papilledema. Magnetic resonance imaging (MRI) of the brain revealed a heterogeneous intraventricular mass measuring 45x41 mm in size with surrounding mild tissue edema, typical for a central neurocytoma. The patient was referred to the neurosurgery department for surgical removal of the tumor; however, the patient did not consent to surgery. Drugs that influence central dopaminergic neurotransmission and almost all neuroleptics, including newer atypical antipsychotics, are also associated with it. While uncommon, NMS remains a critical differential diagnosis for patients with fever and mental status changes due to the requirement for prompt resuscitation to prevent morbidity and mortality. We present a case of a 21-year-old man with schizophrenia who attended the emergency room with generalized muscle rigidity, high-grade fever, and disturbed mental status for 12 days. His serum creatine phosphokinase was elevated (CPK). The computed tomography (CT) of the brain was normal, and the CSF was clear and cell-free. The patient was given muscle relaxants, dopamine agonists, and biperiden. After three days, rigidity, fever, and consciousness improved. A few cases of antipsychotic-induced NMS have been reported. Early repair is required to retrieve the extremity and restore its function, As the advance of surgical repair and reconstruction technology, limbs damaged by high energy trauma that might have needed to be amputated in the past can now be saved by surgical reconstruction \u0000This is a case report of 36 year old man with left upper limb injury after high energy explosion trauma where a large soft tissue defect of the forearm with exposure of the vital structures and fracture of the ulna and radius and the fracture of radius and ulna were fixed by Elastic Nail to maintain their alignments and length, and put a cerclage on the fracture site of the radius to hold a fragment. The large soft tissue was reconstructed by a primary tendon and muscle repair, with vacuum assisted closure followed by split thickness skin graft. Despite that fact, it is necessary to ask the patients about their bad habits to unmask the etiology of the disease. A 30-year-old male who had been abusing alcohol and smoking heavily for the last six years presented with lower extremity edema, fatigue on exertion, and heart palpitations within a month. Previously there was no known chronic disease and immunosuppression drugs use, nor any documented family history of any heart disease. Upon inspection, He had a chronically ill-looking appearance but was oriented to time, place, and person. His vital signs were a blood pressure of 115/80 mm Hg, heart rate of 110 beats/min, and body temperature of 36.5℃. Peripheral oxygen saturation was around 97% without oxygen support, despite having subjective dyspneic symptoms. Transthoracic echocardiography showed LV global hypokinesia with severe reduction in LV systolic function (as demonstrated by an LV ejection fraction of 30%). Empirical oral Thiamine 200mg replacement was initiated emergently as a thiamine test was not available. Within ten days of thiamin supplement and heart failure medications, including furosemide 20mg, Aldactone 25mg, and carvedilol 6.25mg, the ejection fraction improved up to 45% also, the fatigues and dyspnea were also highly improved. Therefore, the purpose of this study was to evaluate medical doctors’ awareness of radiation exposure in selected diagnostic radiology centers in Mogadishu. \u0000Methods: This was a descriptive cross-sectional study of the awareness of doctors about radiation exposure in diagnostic radiology investigations in Somalia. Online questionnaires were distributed to 200 medical doctors working in 3 state and 7 private hospitals in Mogadishu. The study was conducted online during the COVID-19 pandemic. Our data analysis was descriptive with a quantitative method, with univariate level analysis only for all variables. \u0000Results: Of the 200 participants, 62% had no formal training on radiation exposure. 86% of our respondents had no idea of the quantity of radiation. Anthrax is usually caused by bacteria that enter the body through a wound from slaughtering sick animals. 95% of anthrax cases are skin-related. After 1 to 7 days, the spores in skin abrasion become vegetative. We present a 42-year-old woman with a cat scratch history who had small itchy blisters and swelling around a painless skin ulcer with a black core, low-grade fever, and hypotension. She had anemia, hypotension, and bilateral lower limb ulcers. Wound culture showed gram-positive bacilli compatible with B. anthracis. Blood transfusion, Ciprofloxacin 200mg/100ml twice daily, Meropenem 1g three times daily, and Clindamycin 600mg three times daily was given. Her amputated gangrene-infected finger helped her recover. Early diagnosis, antibiotic therapy, and preventive measures like immunization help control epidemics. It is usually divided into allergic and irritant contact dermatitis. In this case report, we present a 2-month-old infant who presented with egg-associated allergic contact dermatitis, which is complicated by bacterial superinfection.
