{"title":"Topical Coenzyme Q10 Eye Drops as an Adjuvant Treatment in Challenging Refractory Corneal Ulcers: A Case Series and Literature Review","authors":"K. Some patients may fail to respond to conventional therapy, making new alternative agents necessary to treat these resistant cases. In this article, we aim to present six challenging cases of refractory corneal ulcers that revealed healing response to the adjuvant treatment with coenzyme Q10 (CoQ10) eye drops and to review the literature examining of new therapeutic agents. Methods: Our study was designed as a descriptive case series demonstrating the use of novel coenzyme Q10 eye drops in refractory corneal ulcers. In our case series, CoQ10 eye drops were added to the existing therapies as an adjuvant agent in six cases: three cases with neurotrophic corneal ulcers, two cases with postinfectious corneal ulcers (one unknown etiology+one Acanthamoeba keratitis), and 1 case with Stevens–Johnson syndrome. All cases were monitored regularly and corneal images were taken at all visits. Results: All nonhealing corneas with conventional therapy revealed recovery after the addition of CoQ10 eye drops. Except for two cases that responded to the CoQ10 eye drops more rapidly (within 1 to 2 weeks), complete corneal healing was observed in four cases between weeks 4 and 8. No adverse events were reported in these cases throughout the follow-up period. Methods: Twenty cases of pseudophakic bullous keratopathy were randomly allocated into two groups. Group 1 underwent slow, single-pass 400-&mgr;m microkeratome head, whereas group 2 underwent the standard technique of DSAEK using a 350-&mgr;m microkeratome head. The primary outcome measures were best-corrected visual acuity (BCVA) at 6 months. Results: Groups were comparable in baseline characteristics. The mean central graft thickness (GT) at 6 months in group 1 was significantly thinner than group 2 (90.44±28.67 vs. 165.60±62.74 &mgr;m; P=0.003). The BCVA and contrast sensitivity were significantly better in group 1 than in group 2 (BCVA: 0.34±0.15 vs. 0.53±0.19 logMAR units, P=0.02; contrast sensitivity: 1.48±0.13 vs. 1.06±0.22, P=0.001). A significant correlation was found between both postoperative BCVA and contrast sensitivity, with the postoperative GT using Spearman rho correlation analysis (R=0.534, P=0.01 for BCVA and R=−0.522, P=0.02 for contrast sensitivity). The percentage endothelial cell loss was comparable between the 2 groups at the last follow-up (P=0.3). No major complications were observed during the study period. Conclusions: The use of a 400-&mgr;m microkeratome head instead of 350-&mgr;m head can improve the visual outcomes in DSAEK without increasing the risk of complications. {"title":"Effect of Lens Care Systems on Silicone Hydrogel Contact Lens Hydrophobicity","authors":"M. Methods: In this research, four silicone hydrogel CLs (senofilcon A, balafilcon A, comfilcon A, and lotrafilcon B) and one conventional CL (etafilcon A) and appropriate lens care solutions (ReNu MultiPlus, Biotrue, OPTI-FREE PureMoist, and AOSept Plus) were used. Lenses were immersed in each lens care system for 12 hr and then CL hydrophobicity was determined through water contact angle (CA) measurements using the OCA 20 instrument (DataPhysics). Results: Mean CA values obtained when CLs were removed from blisters (baseline value) were lower for etafilcon A (49.6°±3.0°) and comfilcon A (48.0°±2.8°) and higher for senofilcon A (78.9°±2.5°), lotrafilcon B (82.6°±1.7°), and balafilcon A (91.5°±0.7°). Depending on CL and lens care combination, it was detected a statistically significant reduction by 70% of the 20 combinations studied and any significant increase in the CA. When compared to baseline values, CA varied between etafilcon A: 2° and 11°, comfilcon A: 1° and 7°, senofilcon A: 2° and 28°, lotrafilcon B: 14° and 31°, and balafilcon A: 18° and 29°. Comfilcon A was the most stable lens material presenting only one significant decrease in CA. Conclusions: This study reveals the existence of interactions between CL material and lens solutions that can play an important role in CL surface wettability. The major changes were obtained in lenses with higher baseline CA, which showed significant changes with each of the solutions. Methods: Fifteen patients with UC (group 1) and 15 healthy subjects (group 2) were enrolled in this study. Tear volume measurement, Schirmer-I test with no anesthetic, tear break-up time, and conjunctival impression cytology (CIC) were evaluated in both groups. Results: Patients with UC showed lower tear volumes when compared with healthy subjects (P<0.05). There was a statistically significant difference in distribution of CIC grades between groups 1 and 2 (P<0.05). Tear break-up time and Schirmer-I measurements were significantly lower in group 1 compared with group 2 (P<0.05). Methods: We retrospectively reviewed 1,195 ophthalmic pathology specimens from 2004 to 2014 at Ichikawa General Hospital. Pathologic specimens of 5 patients with a final diagnosis of “conjunctival papilloma” were stained with hematoxylin–eosin, Ki 67, p53, human papillomavirus (HPV) 16 and 18 antibodies. Results: Of 1,195 patients, 5 patients (4 men, 1 woman; age range: 27∼57 years, mean age: 38.4 years) had a diagnosis of conjunctival papilloma, which constituted to 0.42% of the pathologic diagnosis made for the ophthalmology specimens. All specimens displayed multiple fronds of thickened conjunctival epithelium that enclosed cores of vascularized connective tissues. Three patients with recurrence after surgical excision demonstrated moderate to severe epithelial atypia, who also showed higher staining with Ki67 and p53 compared with patients with no recurrence. HPV16 and 18 antibodies staining did not appear to relate to recurrences. Method: Retrospective analysis on approximately 40 participants (Px) each in 5 trials. Lenses grouped into SiHy (delefilcon A, somofilcon A, narafilcon A) and Hy (omafilcon A, nelfilcon A). Participants attended follow-up visits at baseline, 2 weeks, 1 and 3 months. Subjective ratings (1–10 scale), adverse events (percentage of Px), physiological variables (0–4 scale), and wearing time were collected at each visit and compared between groups. Result: Trials enrolled 201 Px totally. No differences in age, sex, and lens wear experience were found between SiHy and Hy groups (P≥0.09). There was greater increase in limbal redness from baseline in Hy group (0.18±0.38 vs. 0.02±0.47, P<0.001), whereas conjunctival staining and indentation were less in Hy group (P<0.001). No differences in comfortable wearing time were found between groups (P=0.41), and comfort at insertion, during day, and end of day was also no different (P≥0.71). Incidence of corneal infiltrative events (SiHy vs. Hy: 6.7% vs. 2.5%; P=0.32) and mechanical adverse events (SiHy vs. Hy: 0.0% vs. 0.0%; P=1.00) were no different. Conclusion: Though some statistical significance was found between the groups, these differences were within measurement error. Neither material types showed superiority in comfort, and adverse event rates were low with both material types. {"title":"Vision-Related Quality of Life Before and After Deep Anterior Lamellar Keratoplasty","authors":"E. Methods: Thirty-five patients who were scheduled for DALK between March 1, 2013, and March 1, 2014 were asked to complete the NEI VFQ-25. NEI VFQ-25 was administered again at 6 months and at 1 year postoperatively. Data on patients' age, sex, preoperative diagnosis, preoperative and postoperative best-corrected visual acuity, and postoperative astigmatism were recorded. Results: Successful DALK with the Anwar big bubble technique was achieved in 23 of 35 (65.7%) patients. The indications for surgery were keratoconus in 15 patients (62.2%), stromal corneal dystrophies in 4 (17%), and corneal scar in 4 (17%). The mean preoperative NEI VFQ-25 composite score (55.2±19.7) improved significantly (76.9±11.6) at 6 months after DALK and continued to improve (84.3±6.6) at 1 year postoperatively (Friedman test, P=0.001). All NEI VFQ-25 subscale item scores increased significantly after surgery. The patients' age was significantly correlated with the NEI VFQ-25 subscale score of mental health at 6 month and at 1 year postoperatively (r=0.92, P=0.008 and r=0.94, P=0.005, respectively). There was a negative relationship between postoperative astigmatism at 1 year and NEI VFQ-25 ocular pain, social functioning, peripheral vision, and mental health subscale scores (r=−0.76, P=−0.07; r=−0.53, P=0.2; r=−0.53, P=0.27; r=−0.80, P=0.05). Methods: A case–control investigation was conducted. Case patients were soft contact lens wearers with laboratory-confirmed AK. Control were soft contact lens wearers ≥12 years of age, with no history of AK. Case patients were recruited from 14 ophthalmology referral centers and a clinical laboratory. Control were matched on state of residence and type of primary eye care provider (ophthalmologist or optometrist). Participants were interviewed using a standardized questionnaire. Univariable and multivariable conditional logistic regression analyses were conducted. Matched odds ratios (mORs) were calculated. Results: Participants included 88 case patients and 151 matched control. Case patients were more likely to be aged <25 years (unadjusted mOR 2.7, 95% confidence interval 1.3–5.5) or aged >53 years (mOR 2.5, 1.1–5.7), and more likely to be men (mOR 2.6, 1.4–4.8). Unadjusted analyses identified multiple risk factors: rinsing (mOR 6.3, 1.3–29.9) and storing lenses in tap water (mOR 3.9, 1.2–12.3), topping off solution in the lens case (mOR 4.0, 2.0–8.0), having worn lenses ⩽5 years (mOR 2.4, 1.3–4.4), rinsing the case with tap water before storing lenses (mOR 2.1, 1.1–4.1), and using hydrogen peroxide (mOR 3.6, 1.1–11.7) versus multipurpose solution. Significant risk factors in multivariable modeling included age >53 years, male sex, topping off, and using saline solution. Conclusions: Numerous modifiable risk factors for AK were identified, mostly involving hygiene practices. Methods: The mini and semiscleral contact lenses (Procornea) were measured for five powers per design. The NIMO TR-1504 (Lambda-X) was used to assess the power profile and Zernike coefficients of each contact lens. Ten measurements per lens were taken at 3- and 6-mm apertures. Furthermore, the optical quality of each lens was described in Zernike coefficients, modulation transfer function, and point spread function (PSF). A convolution of each lens PSF with an eye-chart image was also computed. Results: The optical power fluctuated less than 0.5 diopters (D) along the optical zone of each lens. However, the optical power obtained for some lenses did not match with its corresponding nominal one, the maximum difference being 0.5 D. In optical quality, small differences were obtained among all lenses within the same design. Although significant differences were obtained among lenses (P<0.05), these showed small impact in the image quality of each convolution. Conclusions: Insignificant power fluctuations were obtained along the optical zone measured for each scleral lens. Methods: Forty patients with myopia aged between 11 and 18 years who wore OK lens continuously for more than 1 year were selected. Refractive error (D), corneal curvature, corneal surface regularity index (SRI), tear breakup time (TBUT), Schirmer I test score, and corneal staining were measured at the following time points: 1 week before wearing the OK lens (T0) and 1 week (T1), 1 month (T2), 3 months (T3), 6 months (T4), and 12 months (T5) after wear. Results: Compared with T0, the average spherical refractions at other time points had become less myopic, flattens the central cornea, the SRI increased, and the TBUT decreased (P<0.01). All the aforementioned variables became stable at T1. Conversely, results of the Schirmer I test showed no significant changes. The number of patients with a corneal epithelium stained with fluorescein increased; staining was mainly of the grade I type. At each time point, the TBUT in patients with grade I corneal fluorescein staining was significantly lower than in patients with grade 0 staining (P<0.05).
