This mini review summarizes current evidence regarding p-Tm:YAG physical properties, laboratory results, and clinical outcomes. p-Tm:YAG laser features high peak power, efficient energy use, and a stable pulse profile. In vitro studies have demonstrated effective ablation, minimal retropulsion, and the ability to produce fine stone dust regardless of stone composition. Clinically, flexible ureteroscopy and mini-percutaneous nephrolithotomy using p-Tm:YAG may achieve high stone-free rates and low complication rates. Moreover, comparisons with the other lasers show similar or even better performance in terms of ablation efficiency and operative time. Although these preliminary results are very promising, robust data are needed to validate the versatility of p-Tm:YAG laser for routine use in clinical practice. PATIENT SUMMARY: We summarize current evidence on the new pulsed thulium:YAG laser for the treatment of urinary stones. High stone-free rates and low complication rates have been reported for patients treated for kidney stone disease. Circulating tumor DNA (ctDNA) represents a fraction of cell-free DNA released into the bloodstream by apoptotic cell turnover or cell death in the tumor. We investigated whether ctDNA measurements before and after radical cystectomy are associated with disease recurrence.</p><p><strong>Methods: </strong>We prospectively collected plasma, tumor tissue, and germline samples before and after radical cystectomy. Seventy patients had complete data for both pre- and postsurgery analyses. Commonly mutated genes in plasma and tissue, particularly TERT and TP53, were assessed using the AVENIO ctDNA platform (research use only; Roche, Branchburg, NJ, USA) and then evaluated for a potential association with patient outcome measures, including survival and disease recurrence.</p><p><strong>Key findings and limitations: </strong>In the overall study population, patients with negative ctDNA status in postsurgery plasma had significantly longer recurrence-free survival than those with positive ctDNA status (p = 0.01). The ctDNA positivity fell from 46% before surgery to 23% after surgery, and a positive postoperative result predicted recurrence independently. Given the widespread use of both modalities, comparison of their stone-free rates (SFRs), retreatment rates, complications, costs, and hospital length of stay (LOS) can provide valuable insights for optimizing treatment strategies.</p><p><strong>Methods: </strong>A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, with searches of the PubMed, Cochrane, and Google Scholar databases for relevant studies. The inclusion criteria were comparative studies analysing costs and clinical outcomes of SWL and URS for urolithiasis management. The meta-analysis included 22 studies involving 146 845 patients, of whom 81 772 (55.7%) underwent SWL and 65 073 (44.3%) underwent URS. Statistical analyses were performed using R. Random-effects or fixed-effect models were applied, depending on study heterogeneity. Subgroup analyses by stone size and location were also conducted.</p><p><strong>Key findings and limitations: </strong>URS had a significantly higher SFR (82.2% vs 64.4%; p < 0.001) and a significantly lower retreatment rate (10.6% vs 29.4%; p < 0.001) in comparison to SWL. There were no significant differences between URS and SWL in the overall complication rate (22.6% vs 22.5%), the mean procedural cost ($2546.20 ± $1297.30 vs $2830.40 ± $1652.10), or mean LOS (2.8 d for both). Subgroup analyses confirmed similar trends across stone size and location categories.</p><p><strong>Conclusions and clinical implications: </strong>While direct procedural costs were only slightly lower for URS, this modality had superior efficacy, with higher SFR and lower retreatment rates, suggesting it is a more cost-effective option in the long term.
