{"title":"Integrating mental health services into human immunodeficiency virus clinics: Lessons from task-sharing between clinical and lay healthcare providers in Ethiopia.","authors":"Ismael Ahmed, Teklu Weldegebreal, Alemayehu Mekonnen","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Globally, mental health problems are more common among people living with human immunodeficiency virus (PLHIV) than among the general population. Mental health problems affect human immunodeficiency virus (HIV) treatment adherence and retention. To address this challenge, partners used a task-sharing approach among lay healthcare works and clinicians to integrate mental health services into HIV services at pilot hospitals in the Amhara and Tigray regions of Ethiopia. In this model, trained lay healthcare workers proactively screened patients using a mental health screening tool and subsequently linked potential clients with trained clinicians working at HIV clinics for further diagnosis and treatment.</p><p><strong>Methods: </strong>We retrospectively gathered secondary data, including demographic characteristics and diagnosis information, from mental health clinicians' and case managers' quarterly reports from HIV clinics during the implementation period (January 1, 2013 to March 31, 2014).</p><p><strong>Results: </strong>During the initial three-month implementation period of the project (January to March 2013), case managers screened 5,862 PLHIV for mental health disorders. Case managers referred 687 (11.7%) patients with suspected mental health disorders to clinicians for further evaluation and management. Of the total patients screened by case managers in this period, clinicians confirmed that 454 (7.7%) had a mental health disorder. Overall, the concordance between the case managers' screening results and the clinicians' diagnoses was 67.8% over the 15-month pilot implementation period.</p><p><strong>Conclusions: </strong>Routine screening of PLHIV for mental health disorders helps to proactively identify and manage patients with co-morbidities. Methods: Within the framework of this work, the external and internal environment was assessed based on the main indicators of the Interdistrict Hospital of the Urdzhar district. Also, the management strategy of a medical organization was investigated, the methods of PEST and SWOT-analyzes were used in the work of the organization management leadership. Results: In the course of the study, it was revealed that the Interdistrict hospital of the Urdzhar region» of the East Kazakhstan region of the Republic of Kazakhstan, during the existence and implementation of the components of strategic management, has achieved development in some of the main indicators of the development of a medical institution. This development dynamics can be seen in the way in the interdistrict hospital of the Urdzhar district of the East Kazakhstan region, work is being carried out to achieve the set goals, which is associated with the tasks and mission, as well as the transformation of the organizational structure of the medical organization Conclusions. In the case of monitoring the implementation of strategies, the tasks acquire a very definite specificity, due to the fact that strategic control is aimed at finding out to what extent the implementation of strategies leads to the achievement of the goals of the hospital at the district level. Micro vascular disease significantly contributes to increased morbidity and indirectly contributes to mortality through diabetic neuropathy. Pain may occur with either acute or chronic neuropathy. It results from small fibre damage. Lancinating, neuralgia - type will follow ectopic generation of nerve impulses, whereas chronic, burning pain is due to nerve is chaemia. Therapeutic laser (TL) is gaining much importance in reducing neuropathic pain Objective: To find out the effect of therapeutic laser in the management of diabetic neuropathy pain by comparing with regular medication. Methods: 50 patients were selected by using lottery method of simple random sampling procedure into two groups (Group A 25 and Group B 25) equally. Group A was treated with medications (oral hypoglycemic, analgesics and anti - depressant drugs) for four weeks. Group B was treated with TL for 10mins for 4 times a week for 4 weeks with a dosage of 4J/cm 2 . Their pre & post treatment values were extracted basing on modified Toronto clinical neuropathy score (mTCNS). Results: Both groups pre and post test values were extracted for four weeks and analyzed individually by using Wilcoxon signed rank test. Both groups showed variation in their scores individually. When compared between the groups by using Mann Whitney U - test, group B showed remarkable variation when compared with group A. Conclusion: TL has shown remarkable reduction in pain score of group B when compared with group A. TL is one of the best treatment modality in reducing neuropathic pain. Physiotherapists can gain access with this treatment to proliferate their practice in treating diabetic neuropathic pain. . [ Ethiop.J.
