Introduction: Paragangliomas, also known as neuroendocrine tumors, are rare and can appear in distinct places throughout the human body where chromaffin cells are present. Such examples include blood vessels and nerves. Thirty percent are in head and neck compartments; after carotid paragangliomas, the yugulotimpanic follow in frequency and then the vagal ones by 5%. Onlyone percent of paragangliomas located in the head and neck are functional compared with those located in other parts of the body that tend to be functional, most commonly causing hypertension. Methods: An ambispective, observational, descriptive review was conducted, including patients operated on by paraganglioma from 1987 to January 2023; 99.4% were initially diagnosed as carotid origin. Results: A total of 1070 patients were captured. 998 patients were female (93.2%). 1,063 of paragangliomas were carotid (99.3%), 2 were vagal (0.18%), 1 was yugulotympanic (0.09%) and 3 were in other locations (0.28%). One was reported as a Schwannoma. None of the tumors were reported as being hyperfunctional. Conclusion: The angiographic visualization of these lesions can confuse their diagnosis and cause an erroneous follow-up with a poor prognosis, since it is described that vagal paragangliomas recur in 17%.","PeriodicalId":115205,"journal":{"name":"Journal of Surgery: Open Access","volume":"154 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127341532","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It has been reported to cause inflammation, neuropathy or systemic disorder. It should therefore be detected both correctly and promptly. The author experienced a rare case of a 38-year-old woman presenting with axillary silicone lymphadenopathy due to rupture of a silicone gel-filled breast implant. The patient received primary augmentation mammaplasty using a breast implant at other hospital; the current case highlights the importance of an ultrasoundassisted diagnosis of silicone lymphadenopathy before reoperation in a patient with rupture.","PeriodicalId":115205,"journal":{"name":"Journal of Surgery: Open Access","volume":"52 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123556080","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It has been proposed that pneumoperitoneum in laparoscopic surgery increases post-surgical pain and produces adverse physiological effects in some patients. Objective: Control postoperative pain in surgery by reducing pneumoperitoneum pressure. Material and Methods: Study 3 Hospitals, both public and private, comparing the conventional technique using the original intra-abdominal pressure versus lowering the pneumoperitoneum pressure to 10 mmHg, to reduce pain. Conclusion: Reducing the CO 2 pressure in mmHg up to 10.9 on average of the transoperative pneumoperitoneum, was shown as a safe, uncomplicated pressure and reduced postoperative pain without the need to scale up a greater number of analgesics.","PeriodicalId":115205,"journal":{"name":"Journal of Surgery: Open Access","volume":"16 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130518231","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Peripheral locoregional anesthesia techniques occupy an important place in anesthetic treatment and postoperative resuscitation due to the frequent possibility of cardiovascular, respiratory, renal complications or mesenteric ischemia, which justifies a treatment adapted to the high surgical risk, therefore, postoperative management should generally be in an Intensive Care Unit. Objective: Present the case of a patient with Abdominal Aortic Aneurysm (AAA) with acute abdomen urgently operated in a second-level hospital using mixed anesthesia. Case: Octogenarian patient with a diagnosis of infrarenal AAA, undergoing emergency surgery for abdominal pain, which was under balanced general anesthesia plus epidural block a bifurcated Dacron synthetic graft was interposed, with postoperative management in general surgery hospitalization area because there was no availability in the Intensive Care Unit (ICU), evolving properly. Conclusion: Evolution depends a lot on the anesthetic technique, which must be directed towards a multimodal strategy, where hemodynamic stability is a priority.","PeriodicalId":115205,"journal":{"name":"Journal of Surgery: Open Access","volume":"1987 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131068111","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: This is a retrospective observational study based on patients presenting to a specialist smell clinic being run as a part of long COVID clinic in an ENT department in a regional hospital in UK. Out of all these patients presenting to this clinic, the ones with taste problems with or without smell problems were included in this study and analysed from electronic patient records. Their taste dysfunction was described along with the follow up period to look at the resolution of symptoms and the time taken for the resolution. Results: 70% of the patients had hypogeusia, 22% had dysgeusia and 8% had ageusia. 52% of the patients showed partial resolution of taste dysfunction, 28 % showed complete resolution and 16% had no improvement in a follow up period of 29 months. Overall, the taste problems lasted for an average of 19.3 ± 7.51 months. Conclusions: From our study, we concluded there is a significant burden of taste dysfunction among the long COVID patients in addition to the smell dysfunction, and despite slow recovery, many of the patients are expected to recover months after the initial infection. Nevertheless, this adds to the distress of the patient as they are unable to enjoy their food during this time which can have negative effects on the quality of life of these patients.","PeriodicalId":115205,"journal":{"name":"Journal of Surgery: Open Access","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115839894","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Further molecular pathological assessment detected Epidermal Growth Factor Receptor (EGFR) sensitive mutation, while Programmed Death-Ligand 1 (PD-L1) was highly expressed with a Tumor Proportion Score (TPS) of >70%. Tumor Proportion (TP) regime was the first-line of treatment gained an obvious reduction in tumor size. Until head progression was found, the patient took Osimertinib orally and is currently stable.
