Worldwide, the utility associated with quality of life is expressed in quality-adjusted life years (QALY). The EQ-5D-5L has shown validity and reliability as a measure of quality of life and utility in health, being studied and validated both in primary care and in diabetic patients. Objective: To measure the perceived quality of life in patients with diabetes mellitus type 2 in utility measures, in a second-level hospital during 2018 using the EQ-5D-5L questionnaire Spanish version for Ecuador. Patients and methods: Cross-sectional study with adults older than 40 years with type 2 diabetes mellitus in treatment and ambulatory medical assistant. The QALYs were estimated using the calculator on the NATS-INC website. The results were expressed in means with standard deviation or percentages, and measures of association between variables were performed. Results: 57 participants, mainly women, fulfilled the inclusion and exclusion criteria; with an average age of 67.77 years. Regarding the quality of life measured by the EQ-5D-5L instrument, the most frequent state of health was 11111 (28.1%), which means “no discomfort”. A mean of 74.47 was obtained in the EVA and 0.708 in QALY. On March 11, 2020, COVID-19 was declared as a pandemic by the World Health Organization (WHO). So far, COVID-19 has proven to be a disease with multiorgan involvement, affecting the hematological system as well. Patients with COVID-19, especially those with moderate to severe disease, frequently experience a coagulopathy associated with a high incidence of thrombotic events, which leads to poor outcomes. The pathogenesis of COVID-associated coagulopathy (CAC), is not fully understood yet, although the host inflammatory response to the infection appears to be a crucial element in the development of CAC. IL2, IL-6, IL7, G-CSF, PI10, MIP1, and TNF alpha, among other molecules, act as proinflammatory cytokines that stimulate endothelium damage and alter the coagulation homeostasis. CAC usually manifests as venous thromboembolisms (VTE). While bleeding can also occur, it is a rare form of presentation. Inpatients with COVID-19 must receive thromboprophylaxis, mainly with low-molecular-weight heparin (LMWH); unfractioned heparin can be accepted under certain circumstances. Patients with a diagnosis or high suspicion of VTE should receive the complete doses of anticoagulation treatment and must continue on it for at least three months. Recommendations regarding prophylaxis and treatment may vary among institutions and countries. There is not clear evidence for the regular use of antiplatelet therapy in patients with COVID-19. The occurrence of displacement of foreign bodies in the maxillary sinus is not a common condition and when present, it is associated with car accidents, fire arm accidents, psychiatric disorders and iatrogenies in surgical procedures. The removal of the foreign body is carried out in order to prevent infections, and the permanence of the foreign body may offer immediate or chronic damage. Access to the maxillary sinus is most commonly performed using the Caldwell-Luc access technique. The first case in Ecuador of a young patient with no relevant pathological history, admitted to the internal medicine service of the Enrique Garcés Hospital for presenting cutaneous macula, arthralgias and myalgias, which is complicated with pleural effusion type exudative and poor respiratory mechanics. Extension tests revealed: Acute myeloid leukemia type M2, which is why he was referred to a reference cancer center to complete study and management. Cytogenetic and phenotypic studies corroborated the diagnosis of BPDCN, Treatment with Hyper-CVAD protocol was instituted, however, the patient presented respiratory, renal and haematological commitment that progresses to refractory and death shock. It affects 1 out of 2500 people without considering the age, gender or ethnicity; its etiology is entirely genetic. According to the nerve conduction velocity it is classified in demyelinating or CMT1, axonal or CMT2 and intermediate the same that has the features of the two previous ones. ECMT in majority of cases it is a slowly progressive disease, presenting with characteristic signs of high instep, stork leg, atrophy and decreased muscle strength, altered perception of vibratory stimuli, distal beginning with proximal progression; arreflexia and alteration of march. The diagnosis is based on family history, clinical and physical examination, complemented by electromyography studies, to determine their classification. Genetic tests are taken, based on the type of suspected ECMT, and these will be used for family counseling. Symptomatology is commonly manifested with hemorrhagic episodes and consequent chronic secondary anemia due to deficit. We present the clinical case of a female albino patient of 68 years of age, who came for acute abdominal pain to this we add a chronic rectorrhagia, when performing the physical examination, the presence of venous malformations (MV) characterized by bluish, compressible and heterogeneous nodules, located in the lower lip, chin, perianal region and extremities, as well as pain on palpation in the right hypochondrium. The laboratory analysis revealed low hemoglobin and hematocrit values and other parameters suggestive of iron deficiency anemia, in the colonoscopy report multiple venous malformations (MV) were disseminated, disseminated with rectal predominance, the diagnosis was compatible with Bean Syndrome by the association of MV in skin and Gastrointestinal tract and secondary chronic anemia. The management was clinical symptomatic and palliative surgery, with a favorable evolution. We aimed to investigate the association between waiting time and hepatocellular carcinoma (HCC) recurrence in patients undergoing LTx for HCC. We studied 250 patients who underwent LTx between 2007-2015. Survival and recurrence curves were calculated according to the Kaplan–Meier method and compared by the log-rank test. Univariate hazard ratios for predictors of post-LTx HCC recurrence were determined by Cox proportional hazards regressions. There were no significant differences in recurrence rates when stratified by wait time to transplant. There were also no significant differences in rates of recurrence when the short (< 165 days) and long (> 335 days) wait-time groups were combined, although in this pooled group the 1-year and 5-year cumulative likelihoods of HCC recurrence were higher than in the group with a wait time of 165-334 days. Other predictors of recurrence were microvascular invasion, explant beyond Milan Brazil criteria and tumor diameter ≥ 2.6.
