The burden of T2DM with cardiovascular complication is now a huge Palestinian public health challenge increase substantially in the last two decades. The natriuretic peptide is important in controlling blood pressure and saltwater balance. B-Type Natriuretic Peptide (BNP) and N-Terminal pro-B-Type Natriuretic Peptide (NT-proBNP) are small peptides that are either hormones or part of the peptide that contained the hormone at one time. They are continually produced in small quantities in the heart and released in larger quantities when the heart senses that it needs to work harder. The aim of the study was to investigate association between elevated N-terminal pro-brain natriuretic peptide and cardiovascular complication among Type-2 diabetic patients in Gaza strip. Method: The study design was case-control consist of 180 participants aged 45 to 65 years divided into three groups. The first group is 60 healthy participants as control (group 1) attending Al-Remal central clinic, Gaza. The second group was 60 participants with T2DM without CVD (group 2) who also attending at AlRemal central clinic and last group were T2DM with the cardiovascular complication at the Cardio Care Unit (CCU) at ElShifa hospital (group 3). Three groups were matched for age and the period of study was from June to October 2018. All subjects were investigated for biochemical markers, serum NT-BNP were determined By ELISA techniques. Results: The results pointed out that there was a significant difference between three studied groups in the gender, BMI, smoking, and level of hypertension (P<0.05). Also, the results illustrated that patients T2DM with CVD complication were higher statistically significant of duration and age at onset than T2DM without CVD patients. The results of the study showed that the average of NT-BNP level was higher statistically significant in T2DM with CVD compared without CVD and controls. In contrast, no statistically significant for NT-BNP level T2DM with CVD and without CVD. By same away, the mean of cardiac enzymes (LDH, CK & CKMB) were higher statistically significant in T2DM with CVD compared with T2DM without CVD and controls (P<0.001). However, the results showed that there is a positive significant correlation between NT-BNP and BMI, Blood pressure (Systolic and Diastolic), CKMB and LDH (P<0.05). Conclusion: Results of this study recommended that serum NT-BNP not utility as a cardiac biomarker in CVDs predication in T2DM patients. Prevention of pressure injuries in high-risk patients such as those with spinal cord injury, the unconscious, and those with femoral neck fracture is essential. The static air-mattress is one of the pressure relieving devices used to prevent pressure injury in low resource countries. However, some patients develop pressure injuries despite being nursed on the air-mattress. The inflation pressure of the static air-mattress is usually not measured, thus is unknown. The first complete repair of tetralogy of Fallot was successfully performed by C. Walton Lillehi and his team in 1954, but first total correction performed at department of cardiothoracic and vascular surgery Amiri Medical Complex Kabul Afghanistan in 2015, despite some advocates of routine two-stage repair in infancy, during the early 1990s reports documented improve early results with primary repair was associated with improved outcome compared to a two-stage approach. The purpose of this study was to analyze the early postoperative result in total correction for the first time in Afghanistan. Method: The purpose of this study was to evaluate the early outcome after total correction in 180 consecutive patients with a mean age of 5-30 years who underwent total correction surgery in a single center Amiri Medical Complex, Kabul, Afghanistan between August 2015 and October 2018. 8 patients had initial palliative operations (modified BT shunt) in outside centers and referred to us for total correction. The trans annular pericardial patch was inserted in 133(73.8%) patients. 32(17.7%) patients repaired by trans atrial total correction (ventricular septal defect, right ventricular outflow tract muscle band resection and pulmonary valvotomy done through the right atrium ) for 5(2.7%) patients with absent pulmonary valve, monocuspid and bicuspid pulmonary valve reconstructed with a pericardial patch. Result: Mean follow up was (1-3) months postoperatively, the mortality rate was (8.8%). Most of the patients who repaired with trans annular patch had free pulmonary valve regurgitation post-operative period by transthoracic echocardiography 26 patients had the excellent function of their native repaired pulmonary valve and monocusp, bicuspid reconstructed pulmonary valve. The peak gradient of right ventricular outflow tract was between 10 to 35 mmHg postoperatively. 22 patients had small (tiny) residual ventricular septal defect and none of the patients had complete heart block (0%). The results demonstrated that among the 250 patients in our hospital, 131 were male and 119 were female, aged from 20 to 86 years, with a median age of 60 years. It mainly occurred in the stomach (58.4%), and in the small intestine (24.0%). 171 cases of patients with mitotic count <5/50 HPF, and the rest of patients with mitotic count >5/50 HPF. 15 cases of patients with tumor diameter less than 2 cm, 88 cases of patients with 2-5 cm, 110 cases with 5.1-10 cm, and the others with tumor diameter larger than 10 cm. According to modified NIH criteria, the risk was divided into: 15 very low risk patients, 66 low risks, 68 intermediate risks and 110 high risk. The positive rates of immunohistochemical markers CD117, DOG-1 and CD34 were 96.4%, 90.4% and 77.2% respectively. There were significant differences in the expression of CD117 and DOG-1 in different tumor sizes and modified NIH criteria (P<0.05), and the expression of CD34 in different mitotic count (P<0.05). Among the 250 patients, 16 patients were treated with imatinib because of the large size and location of the tumors. 17 patients with metastasis mainly metastasized to the liver, the primary site was the stomach, and most of the metastasis patients had mutations in exon 11 of c-kit. The prognosis of small intestine and rectum after operation is obviously worse than those of stomach. Everyone at the party except 2 people was tested positive for COVID-19. Since then the patient has been under stress and delusional thinking that he will die. For high suspicion of COVID-19, his test was sent and he was started on Azithromycin and Plaquenil. On the second day of admission, the rapid response was called because the patient became unresponsive. He was not responding to painful stimuli and his lower lip was shaking. His blood pressure was highly elevated. There was a concern of stroke, stat CT head was repeated which did not show any acute hemorrhage and infarct, and then Electroencephalogram (EEG) showed seizure spikes. His sodium level was 133 meq/L. He was loaded with Levetiracetam and started on Phenytoin. At this point there was a concern that the patient might have encephalitis secondary to COVID-19, his Lumbar puncture was done but it was a dry tap. MRI of the head did not show any encephalitic changes. The patient slowly regained his consciousness. The next day he developed diarrhea, started complaining of cough, and was fixated that he has developed pneumonia and needed treatment. We repeated his chest x-ray which showed no focal lung infiltrate. Unfortunately, he left against medical advice saying that he does not want to get an infection in the hospital. If his test result comes back positive he should be called. A day later, his test came back positive, he was updated and his family brought him to the hospital. According to the family, the day before coming back to the hospital, he passed out three times. During second admission his chest X-ray showed bibasilar infiltrates, D-dimers were also elevated. With the concern of pulmonary embolism as a complication of COVID-19, his CTA chest was done. CTA of the chest showed pulmonary emboli within the right main pulmonary artery as well as segmental and sub-segmental arterial branches of the right upper, right lower, and left upper lobes. It also showed multiple scattered ground-glass densities bilaterally which more pronounced as compared to the prior imaging on his previous admission. In addition to Levonox, he was restarted on Azithromycin, Plaquenil, and anti-epileptics (Levetiracetum and Lacosamide). EEG was repeated which was now negative for seizure.
