Chung","doi":"10.36076/pmcr.2019/3/77","DOIUrl":"https://doi.org/10.36076/pmcr.2019/3/77","url":null,"abstract":"A postdural puncture headache (PDPH) is a\u0000well-described complication after implantation\u0000of an intrathecal drug delivery system (IDDS).\u0000Treatment is typically with supportive management\u0000with the occasional need for an epidural\u0000blood patch. We describe a case of a patient with\u0000refractory muscle spasticity secondary to cerebral\u0000palsy that required a baclofen IDDS implantation\u0000and subsequently developed a PDPH. After failing\u0000conservative therapy as well as an epidural\u0000blood patch, the decision was made to attempt\u0000an epidural fibrin patch, which transiently improved\u0000her headache. Upon return of the patient’s\u0000symptoms, computed tomography myelogram\u0000demonstrated an extensive cerebrospinal fluid\u0000leak with ventral spread into the retroperitoneal\u0000space.
