Data on full-time access and any access to an athletic trainer as well as factors that contributed to accessibility were collected. Twenty-four studies were included in the final review. Two-thirds (66.4%) of the schools in the included studies had any access to an athletic trainer, while only one-third (34.6%) of schools had full-time access. Five common themes arose that diminished a school's ability to access an athletic trainer - smaller school size and lower student athlete numbers, rural school location, private school status, and low socioeconomic status of the region. Athletes and individuals participating in recreational exercise are particularly vulnerable due to increased ventilation during physical activity. The detrimental impacts of various pollutants on cardiorespiratory function, cognition, and sport-specific metrics have been investigated. Therefore, focus within the sports community has shifted to both personal and organizational strategies to mitigate or even prevent these effects. Limited evidence supports reducing exposure time and proximity, transition to indoor activity, precompetition acclimation, monitoring air quality when choosing location, and the use of masks and supplements. In addition, special considerations should be made for the unique exposures and challenges faced by populations, such as warfighters, para-athletes, or those living in disadvantaged communities. This commentary, inspired by real questions encountered in the authors' work, is presented in a Q&A format to simulate a realistic conversation. The authors aim to provide answers to sports medicine physicians and the wider interdisciplinary team by offering a comprehensive overview of key considerations for managing athletes with persistent pelvic floor dysfunction symptoms. However, it is infrequent for pleural empyema to progress to purulent pericarditis and cardiac tamponade. In this report, we present a successful case of a 41-year-old previously healthy Muay Thai fighter who developed purulent pericarditis with cardiac tamponade due to the contiguous spread of infection from an empyemic hydropneumothorax caused by traumatic rib fracture. Emergency pericardial and pleural drainage were performed, and despite negative cultures from both pericardial and pleural fluid, empiric broad-spectrum antibiotic therapy demonstrated significant efficacy.
