It not only results in weakness of the facial musculature affecting the facial expressions, eye closure, and oral competence, but also causes psychological and emotional trauma to the child and parents. Unilateral palsy is usually idiopathic, whereas bilateral palsy usually has an underlying cause. We report a case of a 4-year-old girl who presented to us with simultaneous facial nerve paralysis where the second side was involved within 7 days of the first, before its complete resolution. Evaluation of the patient and the possible etiologies of bilateral facial palsy have been discussed further.","PeriodicalId":108664,"journal":{"name":"Annals of Otology and Neurotology","volume":"32 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124540373","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Study Design This was a nonrandomized clinical trial. Setting Tertiary referral center. Patients Forty-four patients, who presented to the outpatient department or the emergency room and those who fulfilled the inclusion criteria, were enrolled in this study. The patients were put into two groups: Group IT—that received intratympanic steroid therapy alone and Group IVIT—that received intratympanic as well as intravenous steroid therapies. Intervention Therapeutic—in the form of IVIT steroid therapy. Main Outcome Measure Impact of the steroid therapy in patients with SSNHL was measured objectively with the help of pure tone audiometry (PTA). The improvement in hearing was assessed in terms of decibels gained after the intervention in both the groups. Results The mean improvement in PTA after 3 months in Group IT was 19.78 (±18.918) dB, whereas the mean improvement after 3 months in Group IVIT was 22.29 (±16.147) dB, statistically showing no significant difference between the groups. Conclusion The authors recommend the use of IT steroid therapy alone in the management of SSNHL.","PeriodicalId":108664,"journal":{"name":"Annals of Otology and Neurotology","volume":"98 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"117123463","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Involvement of the inner ear is rare with only case reports published over the past six decades. Methods The occurrence of bilateral sudden sensorineural hearing loss is exceeding rare, with limited published data in the literature. We present a case of a 62-year-old man diagnosed with WM who presented with bilateral sequential sudden sensorineural hearing loss. Results A few months following his WM diagnosis, he experienced sudden hearing loss in his left ear. He was treated with a course of oral steroids with no improvement. Three months following this incident, he experienced a similar sudden loss of hearing in his right ear. Treatment for WM was initiated. A repeat hearing test, done 1 week later, did not show any significant improvement in his right hearing. Conclusion The clinical course of WM is highly variable, with relatively infrequent involvement of lymph nodes, spleen, or liver. The inner ear is rarely involved. In this article, otologic clinical presentation is discussed, along with a review of the literature on hearing loss in WM.","PeriodicalId":108664,"journal":{"name":"Annals of Otology and Neurotology","volume":"61 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132847867","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objective The purpose of this study was to ascertain whether associated signs and symptoms at presentation could help identify a subgroup of high-risk patients. Materials and Methods A total of 66 patients with PT were retrospectively assessed. The diagnoses were classified as group I with an LTC or not in group II. Results There were 4 patients (6%) with a final diagnosis of LTC (group I) and 62 patients (94%) without a final diagnosis of LTC or no diagnosis (group II). The results were not quite statistically significant regarding the trauma and were strongly statistically significant regarding headache and the cranial nerve paralysis. Conclusion The combination of PT with any of the three features of occipital headache, cranial nerve palsy, and recent trauma should alert the clinician to potentially serious causes.","PeriodicalId":108664,"journal":{"name":"Annals of Otology and Neurotology","volume":"135 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121629267","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

MRI scanning has changed from a contraindication to a diagnostic tool. For the first time, a pain-free in vivo evaluation of the cochlea’s fluid state, following the insertion of an electrode, has become possible via MRI scanning. The aim of this study was to evaluate various cochlear implant electrodes’ MRI-specific patterns. Materials and Methods In a retrospective study, we evaluated the MRI pattern of casting iron (CI) electrodes in a 3T T2 turbo spin echo (TSE) sequence after a surgery with Medel Flex 28, Flex 24, AB HFMS, and MRI at 1.5 T T2 TSE with the Oticon EVO array. Results A generally different axial MRI pattern between the “lateral wall” electrodes and the “modiolar” electrodes at the basal turn could be observed. A difference in terms of electrode length/insertion depth could not be found at the present scan resolution. Electrode contacts have an impact on the MRI signal-diminishing pattern. Conclusion At T2 sequences, an electrode design-specific MRI pattern can be observed.","PeriodicalId":108664,"journal":{"name":"Annals of Otology and Neurotology","volume":"36 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128715877","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Cooper Hospital and H.B.T Medical College and establish the fact that high-risk neonates have higher prevalence of hearing impairment compared with normal population. Materials and Methods A prospective observational study was conducted over a period of 1 year starting from March 2016 and involved three-stage screening of 410 neonates admitted to neonatal intensive care unit of Dr. R.N. Cooper Municipal General Hospital and H.B.T Medical College, Vile Parle (west), Mumbai. All enrolled neonateswere screened by a three-stage screening mechanism. First otoacoustic emission (OAE-1) screening was done within 24 to 72 hours of birth. Parents of neonates referred by OAE-1 were instructed to come back within 28 days for repeat OAE test (OAE-2). Those referred by OAE-2 were asked to come back after further 2 months for brainstem-evoked response audiometry. Data collected in the study were processed using Microsoft Excel. Results and Conclusion In the current study, out of 410 neonates who were screened by a three-stage screening mechanism, five including three girls and two boys were found to be suffering from profound sensorineural hearing loss. Observed prevalence of 12.20 (or 12 on rounding off to nearest digit) per 1000 in high-risk neonates is much higher compared with the prevalence of 1 to 6 per 1000 live births in overall population as reported by the American Speech-Language-Hearing Association.","PeriodicalId":108664,"journal":{"name":"Annals of Otology and Neurotology","volume":"36 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125241471","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

S1/2019 2 years from August 2016 to July 2018. Six hundred patients who attended outpatient department (OPD) in the Department of Otorhinolaryngology, with sensorineural hearing loss following inclusion criteria, are included in the study. Detailed history was taken followed by clinical examination. All patients were subjected to complete ENT examination, pure tone audiometric examination was done in all cases (MAICO MA42 audiometer). Height, body weight, and waist circumference (WC) were measured. Results The correlation between obesity and SNHL was found to be more significant in females than males. Obese and diabetic patients of both sexes were found to have higher degree of hearing loss for both high and low frequencies as compared with nonobese patients. Good correlation was found between BMI and degree of hearing loss. Abrupt high tone loss type of audiometric configuration was the commonest finding in both male and females. Conclusion There is a correlation between ages related to sensory neural hearing loss with central obesity and its comorbidities, with waist circumference as the most important indicator. Clinical Significance A careful understanding of the modifiable risk factors for age-related sensory neural hearing loss help in its early detection and prevention.","PeriodicalId":108664,"journal":{"name":"Annals of Otology and Neurotology","volume":"65 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127025501","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

S1/2019 Results Hematoxylin and eosin-stained tissue sections showed stratified squamous epithelium, keratinous contents, ghost squamous cells, and inflammatory cells. The psoriasin expression in cholesteatoma tissues was significantly higher as compared with posterior superior retraction pocket (PSRP). A Psoriasin expression increases with the severity of the disease. The proinflammatory cytokines (IL-6 and IL-1β) were also upregulated along with VEGF and MMP-9 in cholesteatoma cases compared with control and the expression increased with disease severity. TEM images of ultra-thin sections of the tissues showed strips of stratified squamous epithelium layer along with inflammatory and apoptotic cells. A very unique ultrastructural feature of bacterial localization within the membrane pocket was seen, which gave direct evidence of bacterial infection in the middle ear cholesteatoma.
