The Radiology Puzzler is designed to support the learning of radiological patterns through graphical case retrieval and intended for teaching visual recognition of intracranial lesions on radiological images. Using radiologic feature samplers, the user may define brain lesion characteristics and use visual query when searching for reference cases. A rule-based module generates a hierarchical list of diagnostic hypotheses and provides a feed-back to the user. The Medical Center, which is physically connected to the University of Maryland Hospital, has a bed capacity of about 280 and performs approximately 52,000 radiology and nuclear medicine examinations per year. One of the major advantages of opening a new Medical Center designed for digital imaging was the ability to ensure that the appropriate infrastructure was present within the hospital to facilitate digital imaging. Within the imaging department (radiology and nuclear medicine) in addition to the plans for the Picture Archiving and Communications System (PACS), equipment was purchased specifically to facilitate digital imaging. The radiology/nuclear medicine/cardiology PACS is utilized for 100 % of the imaging studies at the Baltimore VAMC including conventional radiography (computed radiography), ultrasound, CT, MRI, fluoroscopy, nuclear medicine, and the cardiac catheterization lab. Mammograms are obtained conventionally with film and then transferred into the PACS using a film digitizer. The practical experience and data collection at the Baltimore VA Medical Center have demonstrated that the large scale PACS and virtually filmless operation have been very successful. CAD schemes include the automated detection of masses and clustered microcalcifications in mammograms, as well as the detection of nodules, interstitial infiltrates, cardiomegaly, and pneumothoraces in chest radiographs. In this paper, we demonstrate the current performance, in terms of sensitivity, specificity and speed, of a prototype intelligent radiology workstation which includes a laser digitizer, a high-speed computer, and a jukebox library. One of these shortcomings is in limb trauma management, where live animals are used to gain experience treating war wounds. We propose an alternative approach by creating a three-dimensional computer model of the human body that can be used within a virtual environment to learn trauma management techniques. As a first step, we have developed a computer model that represents the anatomy and physiology of a normal and injured lower limb. The population includes all Active Duty Marines stationed at ITS and presenting to the medical clinic for evaluation. The system utilizes a Pictel System 4000 videoconferencing unit and Codec digitizer, with 28 inch color monitor at both ends. A real time image is projected at 384 Kbps. (384,000 bits per second), the slowest speed at which clinical health care interaction is ethical and effective. Overheads were projected on a Cannon RE-650 Video Visualizer. To date 52 Active Duty Marines were evaluated by nine specialists for a total of 63 diagnoses. Patients, referring providers and specialists completed a satisfaction questionnaire. The majority of both patients and providers rated the quality of moving images, volume and ease of use of the control panel as good or excellent. Two specialties (dermatology and radiology) rated the quality for diagnosis as poor. The majority of consulted physicians were satisfied with their telediagnosis and patient disposition. A majority of consulted physicians noted the telemedicine system had either no effect on, or slightly shortened the length of the obstetrics patient encounter.
