{"title":"Long-term survival in ROS-1 positive non-small cell lung cancer with development of resistance mechanisms and multiple primary tumours: A case report","authors":"Lotte Duckaert, Annelies Janssens","doi":"10.1016/j.rcro.2026.100619","DOIUrl":"10.1016/j.rcro.2026.100619","url":null,"abstract":"<div><div>ROS1-rearranged non-small cell lung cancer (NSCLC) is a rare molecular subset of lung cancer, most frequently affecting young, non-smoking, female patients. The development of tyrosine kinase inhibitors (TKIs), such as crizotinib and lorlatinib, has greatly improved patient outcomes for this cancer type. Nevertheless, challenges such as acquired resistance, histological transformation, and the emergence of additional malignancies remain significant.</div><div>We report the case of a 31-year-old woman diagnosed in 2018 with stage IV-A mucinous adenocarcinoma of the right lung, harbouring a ROS1-translocation. Initial treatment with crizotinib led to a major partial response, but was later changed to lorlatinib due to suspected progression in 2019. In 2020, a right upper lobectomy was performed due to progressive disease, which showed an adenocarcinoma with focal squamous transformation. Postoperatively, lorlatinib was continued, achieving long-term disease control for several years.</div><div>In 2025, surveillance imaging revealed a new pulmonary lesion and an extrathoracic perineal mass. The pulmonary lesion was treated with stereotactic body radiotherapy, and the perineal mass was surgically removed. Histopathology identified an inflammatory myofibroblastic tumour (IMT) with equivocal ROS1 expression but no clear driver alteration. Given the R1 resection margin and the indolent nature of IMT, a watchful waiting approach was pursued. Both the perineal IMT and the right middle lobe tumour were ultimately treated as primary tumours.</div><div>This case illustrates the long-term complexity of managing ROS1-rearranged NSCLC in a young patient. It demonstrates rare but clinically important phenomena, including histological transformation and the development of multiple primary tumours. It emphasises the importance of multidisciplinary decision-making and vigilant long-term surveillance in oncogene-driven non-small cell lung cancer.</div></div>","PeriodicalId":101248,"journal":{"name":"The Royal College of Radiologists Open","volume":"4 ","pages":"Article 100619"},"PeriodicalIF":0.0,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148640254","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study examined how major policy events have shaped diversity and career timelines within the senior UK radiology workforce over nearly two decades.</div></div><div><h3>Methods</h3><div>All radiologists added to the General Medical Council (GMC) Specialist Register between 2006 and 2023 were identified via a Freedom of Information request. Gender and country of primary medical qualification (PMQ) were analysed, classifying radiologists as UK medical graduates (UKMGs) or international medical graduates (IMGs). The interval between PMQ and specialty registration was a surrogate proxy for overall progression. Temporal trends were assessed using Poisson, binomial logistic, and quantile regression, with interrupted time-series models assessing inflection points at 2012 (MMC), 2016 (Brexit), and 2020 (COVID-19).</div></div><div><h3>Results</h3><div>Between 2006 and 2023, 4409 radiologists entered the GMC Specialist Register; 39.2% were women and 39.8% were IMGs from 80 countries. Annual registrations rose by +3.85%/year (p < 0.001) without significant change in female (β:+0.0045, p = 0.44) or IMG (β: 0.0045, p = 0.44) representation. Median time from PMQ to registration was 10 years (IQR: 9–13), 1 year longer for females (p < 0.001) and 4 years longer for IMGs (p < 0.001). The gender gap remained stable (interaction β:+0.029/year, p = 0.51), while the UKMG–IMG gap widened (interaction β:+0.224/year, p < 0.001).</div></div><div><h3>Conclusions</h3><div>The senior radiology workforce has expanded and remains internationally diverse, though gender representation has plateaued. Female radiologists and IMG radiologists had longer intervals to registration. Further work is needed to understand these differences, which may be due to differences in career breaks, migration and global changes in training pathways.</div></div>","PeriodicalId":101248,"journal":{"name":"The Royal College of Radiologists Open","volume":"4 ","pages":"Article 100471"},"PeriodicalIF":0.0,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147409829","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
