Management of simple low back pain, however, is often inconsistent and insufficient. Popular belief that simple low back pain is self-limiting and recovers within a four-week period has been questioned and proven wrong. Without appropriate treatment, a considerable proportion of low back pain patients continue experiencing pain or discomfort for long periods of their lives. Furthermore, some patients' attitudes and beliefs are changed due to chronic pain, and abnormal illness behaviour develops with its consequences and poorer prognosis for recovery. This paper presents a case of chronic low back pain with signs of abnormal illness behaviour in a young female. It highlights the difficulties that might be encountered in the treatment of this complex condition. Despite a maximum effort, chiropractic treatment might not be sufficient to solve the condition. This finding was found incidentally on lumbar series following minor trauma to his lumbar spine. The issue of diagnostic radiology is hotly debated amongst both the chiropractic and medical communities and it is clear that chiropractors need to start work on producing their own consensus-based guidelines on the role of x-rays in chiropractic practice today.</p><p>OBJECTIVE: To establish the prevalence of the different tupes of significant pathologies seen on lumbar x-rays in patients over 50.</p><p>STUDY DESIGN: Retrospective study of 500 patient files with lumbar spine x-rays.</p><p>SETTING: Anglo European College of Chiropractic (AECC) teaching clinic.</p><p>PARTICIPANTS: 500 randomly selected patient files from the AECC teaching clinic.</p><p>METHODS: Files were selected randomly by choosing every third one that fulfilled the inclusion criteria. Each file's x-ray report was looked at, taking note of age, sex and allsignificant radiographic findings. Data were transferred to Statview™ where numbers and percentages were determined.</p><p>RESULTS: The pathologies that represent absolute contraindications to spinal manipulative therapy made up 3.4% of the sample (abdominal aortic aneurysm [AAA], 2%; new/recent compression fractures, 0.8%; and metastases, 0.6%). Findings of relative contraindications were high; osteopænia, 31.6%; calcified abdominal aortas, 33%; Paget's disease, 1%; and ankylosing spondylitis (AS), 0.6%.</p><p>CONCLUSION: It can be seen that there are a high number of relative contraindications to spinal manipulative therapy (SMT) in the over-50 age group. The limitations of the study, however, make the significance questionable. The findings of 3.4% absolute contraindications are significant in this age group. Differential diagnosis of chest pain, using both the medical-pathologic model and the chiropracticlosteopathic-functional model, is discussed. Attention is drawn to the role of the diaphragm and intercostal muscle dysfunction in patients who do not respond to correction of biomechanical lesions of the spine.
