Though much improvement has been achieved over the last 50 decades, the disease continues to be under-recognized and under-treated, and its mortality remains high. Since the first report, the radiologic examination has been an essential part in evaluating this disease. Chest X-ray radiography and computed tomography are conventional imaging techniques in routine clinical practice. Other image modalities, including lung ultrasound, electrical impedance tomography, positron emission tomography, have demonstrated their respective advantages over recent years but have not yet been broadly applied in clinical practice. Among these modalities, computed tomography and its quantitative analysis have shown an irreplaceable power in diagnosis, intervention evaluation and prognostic prediction. In this review, we briefly introduced the basics of acute respiratory distress syndrome and summarized imaging advances. Demographic, clinical, laboratory results, chest CT findings, and CAC on admission were collected. The primary outcome was in-hospital death and the secondary outcome was composed of in-hospital death, admission to intensive care unit (ICU), and requiring mechanical ventilation. Multivariable Cox regression analysis and Kaplan-Meier plots were used to explore the association between CAC and in-hospital death and adverse clinical outcomes.</p><p><strong>Results: </strong>The mean age was 50 years (SD,16) and 1097 (53.1%) were male. A total of 177 patients showed high CAC level, and compared with patients with low CAC, these patients were older (mean age: 49 vs. 69 years, <i>P</i> < 0.001) and more likely to be male (52.0% vs. 65.0%, <i>P</i> = 0.001). Comorbidities, including cardiovascular disease (CVD) ([33.3%, 59/177] vs. [4.7%, 89/1890], <i>P</i> < 0.001), presented more often among patients with high CAC, compared with patients with low CAC. As for laboratory results, patients with high CAC had higher rates of increased D-dimer, LDH, as well as CK-MB (all <i>P</i> < 0.05). The mean CT severity score in high CAC group was also higher than low CAC group (12.6 vs. 11.1, <i>P</i> = 0.005). Patients showed gastrointestinal symptoms, including nausea, vomiting, or diarrhea. Results of the initial laboratory examination were performed after admission. Chest CT was performed for all patients, with the lower bound including the gastric antrum. The volume of COVID-19 and lungs was segmented, and the ratio was calculated as follows: PV/LV = Volume<sub>pneumonia</sub>/Volume<sub>lungs</sub>.</p><p><strong>Results: </strong>Among the 240 patients, 109 presented with gastric wall edema (edema group), and 131 showed no gastric wall edema (non-edema group); the PV/LV values between the two groups were significantly different (<i>P</i> = 0.002). Univariate analysis revealed the following: fibrinogen (Fib), thrombin time (TT), activated partial thromboplastin time (APTT), and albumin (ALB) significantly differed between the two groups (<i>P</i> < 0.05). Binary logistic regression analysis showed that only APTT had a negative effect on gastric wall edema (<i>P</i> = 0.003).</p><p><strong>Conclusions: </strong>SARS-CoV-2 invades the gastrointestinal tract, gastric wall edema is the primary CT manifestation, and gastric wall edema is more likely to occur with a shorter APTT and severe pneumonia, with a slightly longer hospitalization time.
