This is particularly true in the setting of thoracic and upper abdominal surgery. The clinical and functional preoperative evaluation aims at identifying risk factors for the development of these complications. The available literature allows elaborating recommendations for the evaluation of patients eligible to lung resection surgery. The goal is not only to assess the immediate risk of the surgical intervention but also its long-term functional consequences on the respiratory system. For a long time, measurements of the lung functional parameters are an essential step in this evaluation. However, the assessment of exercise tolerance is now critical in the workout of patients with altered pulmonary function. For other types of surgery, the clinical assessment is as important as the evaluation of pulmonary function. The former aims at identifying risk factors that can be modified in the perioperative period. Although pulmonary functional testing is certainly useful in some conditions - before upper abdominal surgery and thoracic surgery and in patients with identified chronic respiratory diseases -, the currently available literature is insufficient for precisely defining its place.</p></div>","PeriodicalId":100442,"journal":{"name":"EMC - Pneumologie","volume":"2 4","pages":"Pages 177-191"},"PeriodicalIF":0.0,"publicationDate":"2005-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.emcpn.2005.07.003","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"72070080","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is estimated that about 25% of the visits in a pulmonary service are due to a pleural disease. Their management involves various specialists such as pneumologists, radiologists, internal medicine specialists, cardiologists, paediatricians, oncologists, and thoracic surgeons. Located in a confined space, the pleural cavity, pleural diseases may originate from a broad spectrum of diseases, from local diseases of the lung and pleura to systemic conditions such as collagen vascular diseases or drug reactions. Despite modern investigation techniques, identifying the underlying cause is always a clinical challenge. Although the clinical presentations of pleural diseases are usually limited to pleuritic pain and dyspnea, various investigation procedures may be undertaken, always preceded by imaging to show potential effusion, thickening, or pneumothorax. Newer techniques as high resolution CT-scan or ultrasonography can be useful. Percutaneous pleural biopsy is no longer used for pleurisy (except in case of pleural tuberculosis) but thoracentesis using dedicated needle remains the first and best procedure to characterise the effusion (exsudate or transsudate). Currently, the procedure of choice for the management of pleural diseases is the thoracoscopy. By permitting complete exploration of the pleural cavity and many endopleural diagnostic or therapeutic procedures, this technique, significantly less invasive than a thoracotomy, is the cornerstone of the pleural cavity evaluation, constituting a real ‘window’ to the pleural space.</p></div>","PeriodicalId":100442,"journal":{"name":"EMC - Pneumologie","volume":"2 3","pages":"Pages 127-146"},"PeriodicalIF":0.0,"publicationDate":"2005-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.emcpn.2005.07.001","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"72069588","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
