Since humoral regulation could play a decisive role in the metabolic adaption to hypoglycemia, caused by the enzyme deficiency, we studied insulin-, glucocorticoid-, catecholamine-and somatotropin-secretion in a 27 year old man with a mild glycogenosis type I. Basal and stimulated insulin release was decreased, the glucocorticoid secretion lay in the lowest part of the normal range, whereas catecholamine and somatotropin secretion showed no significant change. {"title":"[New culture method for the diagnosis of bacteriurias].","authors":"M Bacharach, F C Sitzmann, M A Abdou, H B Kastert","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The aim of this study is to check the clinical relevance of a new cultural method for the detection of urinary tract infection especially in urine samples containing residue of antimicrobial agents. The new procedure concerns a urine collecting and cultivating cup (UCCC Biotest Serum Institute), containing a recovery agar (RA5) with incorporated chemotherapeutica antagonists. The efficiency of the UCCC is compared with the familiar dip slide coated with cystein lactose electrolyte deficient (CLED) agar and MacConkey agar. A collective of 305 children is considered in this study. The investigation was carried out before starting, three to five days after starting and three to five days after stopping the chemotherapy. The highest correlation between the microbial count in urine samples and the whole clinical diagnosis was achieved with RA5. The lowest correlation was attained with MacConkey agar. With CLED agar a good correlation was determined before starting therapy, a modest correlation during therapy and very good correlation after stopping therapy. Regarding relapses and reinfections the detection quote of relevant bacteriuria during therapy amounted to 90% as RA5 was used, 62% with CLED agar and 34,5% with MacConkey. {"title":"[ACTH response in the dexamethasone suppression test].","authors":"E Jungmann, F Schulz, W Magnet, K Schöffling","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Despite the great clinical importance of the low-dose dexamethasone suppression test (determination of cortisol after dexamethasone 2 mg p.o. at 11.00 p.m. in the evening before) and of the determination of ACTH in plasma, so far only little is known about the ACTH response to dexamethasone in normal subjects. In the present study cortisol and ACTH were determined by radioimmunoassay in normal subjects at 8.00 a.m., 10.00 a.m. and noon before and after dexamethasone. Additionally the cortisol response to ACTH was tested under both conditions. The ACTH concentrations in the morning show a periodicity quite similar to the well-known circadian rhythm of cortisol secretion. Dexamethasone blocks this periodicity without decreasing ACTH significantly. The cortisol response to ACTH after dexamethasone is normal. In the morning only a basal and presumably biologically inactive ACTH is secreted which can not further be suppressed by dexamethasone.
