{"title":"[Teaching of endodontics in the 11-member nations of the European Endodontic Society].","authors":"F Harty, G Lavagnoli","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This study was commissioned in 1984 by the Council of the European Society of Endodontology (E.S.E.) in order to discover the extent of endodontic teaching in undergraduate curricula, the methods used in the teaching of the subject and the clinical technique favoured by each school. The survey was conducted with the co-operation of the Country Representatives in eleven out of the fourteen member countries. Each representative was sent a questionnaire and asked to translate it, if necessary, into the appropriate language/languages. These translated questionnaires were sent to the Head of the department responsible for endodontic teaching in each school of that country. The questionnaire inquired into the following subjects: 1) Type of school and teaching arrangements; 2) Teaching concepts; 3) Operative procedures. K-files have been examined for they probably are the most commonly used endodontic instruments. It is particularly important to take into consideration, together with the original quality of the instruments, its possible changes due to the wear from sterilization. {"title":"[Significance of the bacterial flora in the etiology of apical periodontitis. As a matter of fact it has been proved (13) that sterile necrotic pulp tissue is completely unable to cause inflammatory reactions at the periapex. Infection of endodontic origin extends to the supporting tissues of the tooth only in the case of their acute inflammation (e.g. acute apical periodontitis, acute alveolar abscess, phoenix abscess). On the other hand in chronic inflammation bacteria remain confined in the endodontic space. Only few exceptions to this general rule have been experimentally proved. In endodontics we deal with a mixed infection which is composed by obligate anaerobes and by facultative anaerobes. The most frequently found obligate anaerobes are Bacteroides sp. and Fusobacterium sp. (Gram- rods) Anaerobic Diphtheroides (Gram+ rods) Peptostreptococcus sp. (Gram+ cocci) and Veilonella sp. (Gram- cocci). {"title":"[Respecting apical foramina in the endodontic treatment of confluent canals].","authors":"E Berutti","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Many dental components (lower incisors, mesial roots of the lower molars, upper bicuspids, the mesiobuccal root of the upper bicuspids, the mesiobuccal root of the upper first molars) have two root canals at cavity level to which two main canals correspond. These two canals either have two separate apical foramina or they merge in the apical third and have a single apical foramen. The endodontist's failure to recognize the latter anatomical characteristic results in deformation of the shared portion of the root canal and laceration of the apical foramen. Though varying following the different anatomy of the teeth, the opening of the pulp chamber must follow some fundamental rules: no undercuts between the floor of the chamber and the walls of the access cavity, complete removal of the roof of the chamber, unrestricted view of the opening of the canals and of possible accessory canals. The ideal shape of the access cavity should be a funnel with the larger diameter towards the occlusal surface. The study with the longitudinal sections M.-D. and V.-B., stamps and transparent teeth gives us an unquestionable reality. Specially the V.-B. longitudinal sections, which are impossible to observe in the patient with our Rx, are the most instructives.
