Here,nwe set out to identify high-performance predictive biomarkers of DCInand its underlying metabolic disruptions using metabolomics and lipidomicsnapproaches. This single-center prospective observational study enrolledn61 consecutive patients with severe aSAH; among them, 22 experiencedna DCI. Nine patients without aSAH were included as validation controls.nBlood and cerebrospinal fluid (CSF) were sampled within the firstn24 h after admission. We identified a panel of 20 metabolites that,ntogether, showed high predictive performance for DCI. This panel ofnmetabolites included lactate, cotinine, salicylate, 6 phosphatidylcholines,nand 4 sphingomyelins. The interplay of the metabolome and the lipidomenfound between CSF and plasma in our patients underscores that aSAHnand its associated DCI complications can extend beyond cerebral implications,nwith a peripheral dimension as well. As an illustration, early biologicalndisruptions that might explain the subsequent DCI found systemic hypoxiandriven mainly by higher blood lactate, arginine, and proline metabolismnlikely associated with vascular NO and disrupted ceramide/sphingolipidnmetabolism. We conclude that targeting early peripheral hypoxia precedingnDCI could provide an interesting strategy for the prevention of vascularndysfunction.","PeriodicalId":522562,"journal":{"name":"Figshare","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147899384","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
