{"title":"Surgical Approach to Pulmonary Cystic Adenomatoid Malformations: About an Observation","authors":"Meskouri Karim","doi":"10.62046/gijams.2024.v02i02.004","DOIUrl":"https://doi.org/10.62046/gijams.2024.v02i02.004","url":null,"abstract":"Congenital cystic adenomatoid malformation (MAKC) is a congenital anomaly of lung development, and is defined as an adenomatoid proliferation of terminal respiratory structures, bronchial and distal pulmonary cystic dilatation, communicating with the bronchial tree and lacking cartilaginous rings, representing approximately 25% of congenital lung lesions, and in 80 to 85% of cases, the diagnosis is made before the age of 2 and is rarely discovered in adulthood. We report 01 case of MAKC diagnosed at the age of 06 months. The diagnosis was based on clinical elements (episode of bronchiolitis with serious signs at the age of 5 months) and radiological (TLT and CT). The lesion was located in the lower lobe of the right lung. The patient underwent a surgical resection such as a lower right lobectomy removing the lesion in one piece. The discovery of a MAKC must be recognized by clinicians. The clinical diagnosis, strongly oriented by the radiological approach, is confirmed by the anatomical pathology, the therapeutic sanction is surgical in the majority of cases. Our study included all pregnant women admitted to both wards for hypertension during pregnancy during the study period. Age, occupation, risk factors, history, gestation, parity, laboratory tests, complications, and treatment were studied. Out of 2500 pregnant women admitted, 150 had hypertension during pregnancy, i.e. a prevalence of 6%. The age group between 20 and 30 was the most common with 49%. The pre-eclampsia type is the most common subgroup with 46% and the most dreaded because of its high feto-maternal morbidity and mortality. We recorded maternal complications such as eclampsia and HRP. The exact epidemiology of anal pathologies is difficult to define, due to the reluctance of patients to discuss their symptoms and to be examined. In most cases, their diagnosis is delayed and their management is inadequate or inappropriate. Aim: To study anal pathologies during gravido-puerperality at the CS Ref of Kalaban Coro. Methods: This was a single-center descriptive and analytical prospective study with prospective data survey at the CS Ref of Kalaban Coro. The study was conducted over a 9-month period from January 1 to September 30, 2019. Results: During the study period, we identified 151 pregnant women. Of these pregnants, 88 or 58.27% had anal pathology. The different anal pathologies encountered were, hemorrhoidal disease (30.68%), anal fissure (30.68%) and anal incontinence (27.27%). The offending factors were dominated by constipation, a newborn weight > 3500g, a foetal expulsion time > 20 minutes. The clinic was conducted by interrogation and proctological examination. Clinical signs were anal pain, rectorrhagia, and pruritus. The prevalence of uterovaginal aplasia usually reported in the literature is 1/4000 to 1/10,000. Objectives: We tell you about the difficulties of diagnosis with ultrasound, the difficulties of managing our case of vaginal aplasia. Observation: This was a 20-year-old patient saleswoman, single. History of the Disease: was marked by cyclic pelvic headache for 6 months with primary amenorrhea. She has no particular medical-surgical history. On Physical Examination: secondary sexual characteristics were developed. On inspection of the vulva, normal pubic hair was noted. Examination of the vulva showed the labia majora and labia minora present and normal-looking. A hymen could not be identified and there was no visible vaginal opening. Faced with this clinical and paraclinical symptomatology, we made the diagnosis of vaginal agenesis. The ultrasound showed a uterus measuring about 92x71x62 mm, its parenchyma is homogeneous, its contours are regular. The presence in the uterine cavity at the corporeofundic level of a homogeneous echogenic collection of about 66.5 cc (69x35 mm). Note the congenital malformation of the cervix with agenesis). The Surgical Procedure Performed: is vaginoplasty. So the researchers conducted this preliminary statistical study of both creatinine and some electrolytes in the blood serum of a group of healthy males. Years 25 samples of healthy male blood were collected in Sabratha city. Their average age was 13.64, 41.92, The concentration of sodium, chloride, potassium., magnesium ions, and creatinine ions were measured in the blood serum of each volunteer in the hospital the city of Sabratha. The mean and deviation were standard for the concentrations of these chemical components, respectively: (5.15, 140.62 mmol /L, 3.98 (106.80) ) mmol /L, 0.74 4.60 ) mmol /L, 0.18 2.06) milligram / dL ), 0.17 0.84) milligrams / dL. When compared to internationally accepted reference values, the results showed the necessity of working to determine the concentration of creatinine, sodium, and potassium. And chloride through the serum of healthy males in Sabratha and then Libya. In the case of magnesium, it seemed clear that the internationally accepted reference value was sufficient for the purpose. Materials and Methods: We report a retrospective study spread over 6 years to identify 432 patients who benefited from the placement of an implantable catheter port. Patients were followed during hospitalization and at discharge, one month, and one year later. Results: The average age is 67.9 years. There are 284 women and 148 men. The primary cancer requiring the installation of a chamber is essentially represented by cancer and bronchopulmonary cancers. The right internal jugular approach was the most frequently performed. Complications are mainly represented by infection and pneumothorax. Discussion and Conclusion: For prolonged treatments or treatments with aggressive drugs for the veins, a central venous access is preferable to the short peripheral catheter because the repetition of the punctures and the venous irritation of certain injectable drugs expose to pain, thrombosis or even necrosis, peripheral veins. The implantable catheter chamber allows direct access to a vessel to position the end of the catheter at the entrance to the heart, therefore rapid blood dilution. This makes it possible to inject drugs over the long term and/or whose venous tolerance is poor. The diagnosis of toxoplasmosis during pregnancy is often based on maternal serological testing for IgM and IgG Anti- Toxoplasma antibodies. Persistence of IgM for long periods, posses' problems in distinguishing acute from chronic infection. The evaluation of specific IgG avidity enables more accurate dating, since avidity rises progressively during the course of infection. Test that measures immunoglobulin G (IgG) is used to determine if a person has been infected. If it is necessary to try to estimate the time of infection, which is of particular importance for pregnant women, a test which measures immunoglobulin M (IgM) is also used along with other tests such as an avidity test. Toxoplasma-specific IgG antibodies are detectable 1-3 weeks after infection and remain detectable for the life of the individual. Toxoplasma-specific IgM antibodies are also detectable 1-3 weeks after infection but generally decline to nil by one year after infection. The VIDAS® TOXO IgM (TXM) assay is intended for use on the instruments of the VIDAS family (VITEK® Immunodiagnostic Assay System) as an automated enzyme-linked fluorescent immunoassay (ELFA) for the presumptive qualitative detection of anti-Toxoplasma gondii IgM antibodies in human serum. Seventy-six (76) women in the first 16 weeks of pregnancy were tested for VIDAS IgM, IgG antibodies and VIDAS Toxo-IgG avidity. Low avidity antibodies were demonstrated in 2 (33.3%) of 6 sera positive with IgM assay and 4 (12.12%) of sera positive with IgG assay. Low avidity was also detected in 2 (3.27%) of 61 sera negative with IgM. The low avidity suggesting a recent infection, while high avidity in 3 (50%) of the 6 positive IgM and 24 (72.72%) of 33 positive IgG indicating that the Infection acquired in the distant past. Despite the vulnerabilities resulting from the challenges of aging, the elderly are entitled to a dignified and affluent existence. They continue to possess optimism in maintaining their productivity despite their advanced age. In Indonesian culture, there is a prevailing phenomena where regardless of the circumstances, both the elderly and their family have a strong desire to reside together within the community. Hence, the Ministry of Health has initiated an Informal Elderly Caregiver Program. The study aims to analyze the increase of knowledge and abilities among elderly informal caregivers subsequent to their participation in a training program. The study utilized a pre-experimental design, specifically employing a one-group pre- and post-test approach. A total of thirty informal caregivers participated in this study. The mean age of the participants was 42 years (SD = 2.55). The majority of participants (97%) were female. The results show that knowledge and skills were improved. The participants actively engaged in discussion and posed inquiries regarding the cases they frequently encounter. The caregivers' confidence increased in providing care for the elderly being care for.
