Utilizing a modified sol-gel process, MSNs were synthesized and functionalized with amino (MSN-NH2) and carboxyl (MSN-COOH) groups to enhance their physicochemical properties and drug delivery efficacy. Comprehensive characterization through scanning electron microscopy (SEM) and Powder X-ray diffraction (PXRD) confirmed the successful synthesis of spherical MSNs with a uniform internal structure and mesoporous nature. Zeta potential measurements highlighted the impact of surface functionalization on the surface charge of the nanoparticles, revealing positive and significantly negative charges for MSN-NH2 and MSN-COOH respectively. This study further investigated the pH-responsive drug release profiles of DOX-loaded MSNs under physiological (pH 7.4) and tumor-mimicking acidic (pH 5.5) conditions. The results demonstrated a controlled release at pH 7.4, minimizing systemic toxicity, and a significantly enhanced release at pH 5.5, targeting the acidic tumor microenvironment for improved therapeutic efficacy. The findings underscore the potential of functionalized MSNs as a versatile nanocarrier system for cancer therapy, offering a promising strategy to increase the therapeutic index of DOX and reduce adverse effects. {"title":"Various Clinical Manifestation of Omphalomesenteric Duct Remnant in a Child: A Case Report","authors":"Jonsinar Silalahi","doi":"10.32996/jmhs.2024.5.2.8","DOIUrl":"https://doi.org/10.32996/jmhs.2024.5.2.8","url":null,"abstract":"Omphalomesenteric duct (OMD) remnant is a condition in which the omphalomesenteric duct fails to obliterate completely or partially. Failure in this step can produce various patterns, such as a vitelline cyst, umbilical sinus, polyp, Meckel’s diverticulum, and patent OMD. Various clinical manifestations of OMD rarely occur in one patient. We present the case of omphalomesenteric duct remnant, manifested as anterior wall abdominal mass, intraperitoneal hemorrhage, and obstructive ileus. Case report: A four-year-old girl was referred to our hospital with obstructive ileus and abdominal wall mass on the left lower quadrant. Contrast-enhanced abdominal computed tomography (CT) scan found an irregular cystic mass in the abdominal cavity that adheres to the anterior wall of the abdomen and bowel structure with a fluid-solid level without enhancement. Further medical history was taken and found a history of intermittent watery umbilical discharge that appears three times in 2 years after birth. Omphalomesenteric duct remnant was suspected, then proceed to exploratory laparotomy. We found intraperitoneal hemorrhage and 5x4 centimeters frail mass adhering to the anterior wall of the abdomen on the left side of the umbilicus. Histopathological examination confirmed intestinal tissue, suggesting remnants of the omphalomesenteric duct and pancreatic tissue with a chronic inflammatory pattern. Diagnosing omphalomesenteric duct remnant could be challenging and should be considered a differential diagnosis of obstructive ileus, particularly if persistent umbilical discharges are also present. Recognizing healthcare professionals as essential stakeholders, SCFHS actively solicited their perspectives to better understand their needs and to collect actionable suggestions for service enhancement. Employing a multifaceted research approach, this study utilized focus groups, a large-scale survey involving over 700,000 healthcare practitioners, and comparative benchmarking against selected G-20 nations. The findings indicate a strong desire among practitioners for a reorganisation of the current registration and classification procedures. Notably, there was a consensus for extending the registration period, with a predominant preference for a five-year duration. Participants also advocated for various improvements, such as an increase in Continuing Medical Education (CME) hours, financial cost reductions, and streamlining of registration processes. Surgical Site Infection (SSI) is one of the problems of healthcare-associated infections (HAIs). Dr. Hospital H. Moch. Ansari Saleh Banjarmasin is a government hospital that provides many orthopedic surgical procedures and has data related to antibiotic use. This study aims to determine the description of the use of antibiotics as preoperative and postoperative prophylaxis. The research method included a descriptive observational study with a cross-sectional design on medical record data of surgical patients at Dr. RSUD. H. Moch. Ansari Saleh Banjarmasin in August–December 2023, which was taken using a purposive sampling technique with a retrospective approach. Results: 133 samples met the inclusion criteria. The majority of patients were aged 17–25 years (25.6%), the gender was predominantly male (62%), the length of stay was at most 2 days (40.6%), the type of surgery was clean (82%), and they did not have comorbidities (83%). Research shows that cefazolin is most widely used as preoperative prophylaxis by 123 people (92%) and postoperatively by 79 people (59.4%). The study concluded that the most widely used antibiotic for patients undergoing orthopedic surgery at RSUD, Dr. H. Moch. A cross-sectional study was conducted in Gulf Medical University (GMU), Ajman, UAE, upon approval from the Ethics Committee. A self-administered questionnaire was given to gather information from 624 students about Restless Legs Syndrome (RLS), covering things like age, gender, and factors associated with it. Among the students (164 boys and 452 girls), we found that 6.2% had restless leg syndrome. The results of the study showed a connection between restless leg syndrome and factors such as magnesium deficiency, vitamin B12 deficiency, anemia, and sleep disturbance. We did not find a significant link between age and restless leg syndrome in our study, as we focused on people aged 18 and older. In conclusion, we identified the most common factors connected to restless leg syndrome, including low magnesium, low vitamin B12, anemia, and sleep problems. Utilizing data from the General Social Survey (GSS), the research explores the relationships between COVID-19 vaccination status and variables such as confidence in medicine, political views, general health condition, income, education level, and marital status. The analysis employs logistic regression models and Chi-Square tests to assess these relationships. Key findings indicate that higher income and education levels, as well as more liberal political views, are positively associated with vaccination uptake. In contrast, marital status presents a more complex picture, suggesting further exploration is needed. The study highlights the multifaceted nature of vaccination decisions and underscores the importance of tailored public health strategies that address the specific needs and concerns of different demographic groups. The research also acknowledges challenges and limitations, including issues related to causality, confounding factors, data quality, generalizability, response bias, and multicollinearity. In this paper, VOSviewer was used to statistically and visually analyze the literature related to traditional Chinese medicine pension in CNKI database from 2013 to 2023. Firstly, we collected and sorted out the research literature on the Chinese medicine pension industry in recent years, and then used bibliometric methods to quantitatively analyze the quantity, quality and research fields of the literature. The study found that the number of research literature on the elderly care industry of traditional Chinese medicine has increased year by year, the research quality has been continuously improved, and the research field has gradually expanded. At the same time, we also found that the research hotspots of traditional Chinese medicine for the elderly care industry are mainly focused on elderly care services, traditional Chinese medicine health preservation, rehabilitation treatment, etc (Ge & Xu, 2023). In addition, this paper also puts forward the shortcomings and development directions of the research on the Chinese medicine pension industry in China and hopes that this study can provide useful reference and guidance for the development of the Chinese medicine pension industry. Based on the China National Knowledge Infrastructure (CNKI) database as the main data source, this paper uses the topic search method in advanced search to search the CNKI database, and confirms that “Chinese medicine for the elderly” is used to read a large number of literature and understand the health care of traditional Chinese medicineAs the theme, the document format was set to see the full text only and Chinese and English extensions, the search time range was set from January 1th, 2013 to December 31th, 2023, the document type was set to academic journals, and the journal source category was set to all journals, a total of 149 articles were obtained, excluding news, publicity, interviews, correspondence and other literature that did not meet the research theme, and finally a total of 149 articles were obtained by manual screening 126 articles in Chinese. VOSviewer is an open-source network analysis tool, mainly used for scientific literature analysis, which can help users carry out visual analysis and network construction and is suitable for analyzing the research status of a certain field. This type of violence is interpreted as a form of gender discrimination and represents a violation of human rights that has a direct impact on women's mental well-being. It manifests itself mainly as an inequality of power between healthcare professionals and pregnant women, whether during childbirth or the puerperium. The World Health Organization has stressed the importance of eliminating these harmful medical practices, urging medical personnel to react appropriately through dialogue and support for pregnant women. A nationally representative sample of women aged 12 years and older from the 2018 National Health and Nutrition Survey was used. Multicollinearity tests were used to avoid redundant information in the models. Next, we used factor analysis to generate an index of obstetric violence and performed linear regressions to observe the correlation between obstetric violence and mental well-being of women in our sample. Finally, we addressed the heterogeneity observed in the proposed relationship by using quantile regression techniques to disentangle the heterogeneous relationship in the distribution of mental well-being and obtain correlation coefficients, with their 95% confidence intervals (95% CI). Based on our findings, we highlight the urgent need to raise awareness and sensitize health professionals about this specific problem, since our main result indicates that there is a misleading correlation between obstetric violence and women's mental well-being. It is crucial to modify those health practices that violate women's rights.
